About this file

This is a bid form for the State of Maryland's Western Maryland Hospital Center for testing and inspections of underground fuel oil storage tanks. The contract is a three-year firm fixed price agreement requiring bidders to submit pricing for Year 1, Year 2, and Year 3. The scope of work includes five primary service provisions: testing spill containment on each tank every three years with proper sealing to prevent leakage; inspecting overfill protection devices every three years; completing Underground Storage Tank System Compliance Inspection Reports every three years using the Maryland Department of the Environment Form MDE/WAS/COM.055, signed by a licensed inspector; conducting precision tightness tests of each UST every three years in accordance with COMAR regulations; and testing supply and suction piping on either an annual or biannual frequency as required. Additionally, the contractor must perform annual fuel testing of emergency generator fuel storage tanks using ASTM D975-10 standards, with fuel samples taken from both the main storage tank and day tank, processed by a certified laboratory, and treated if necessary to maintain compliance. The bidder must provide a comprehensive report detailing pre-test, treatment, and post-test results to the Class A facility operator.

Bidders must complete the entire bid form without amendments, alterations, or blank items, with failure to adhere resulting in rejection as non-responsive. The bid requires contractor identification information including FEIN, eMMA number (BPM054721), contact details, and the service location of Hagerstown, Maryland 21742. Bidders must provide a list of major materials and sign the form with an authorized signature and date. All pricing should be entered as a total bid price for the three-year contract period. Extra work beyond the scope must be invoiced separately. The form emphasizes strict compliance with submission requirements and the mandatory submission of all supporting documentation alongside the bid.
