About this file

This document is a Prohibition Regarding Foreign Countries of Concern form issued by the City of Gainesville's Procurement Division, implementing Section 287.138 of Florida Statutes effective January 1, 2024. The form requires bidders on city contracts that would grant access to personal identifying information to submit an affidavit attesting that they do not have connections to specified foreign countries of concern, including China, Russia, Iran, North Korea, Cuba, Venezuela, and Syria. The affidavit must be signed by a company officer under penalty of perjury, confirming the bidder is not owned by, controlled by, or principally located in these countries.

The document provides a standardized affidavit template for bidders to complete, which includes spaces for the bidder's name, title, and a notarized declaration that they do not meet the specified criteria related to foreign country ownership, control, or business location. This requirement appears to be part of a broader governmental effort to restrict potential security risks in public procurement by limiting interactions with entities from certain geopolitically sensitive nations. The form must be completed and notarized for a bidder to be considered eligible for city contracts involving sensitive information.
