Summary of FAM-25-039 Attachment N Success Coach Program Overview

This document is a program overview attachment related to the Virginia Department of Social Services (VDSS) Regional Post Permanency Consortia Services Request for Proposals. The Success Coach Service is a voluntary enhanced case management program designed to provide in-home support to eligible families through trained social workers who partner with families for up to two years. Eligible families must have at least one child in the home who was adopted through the foster care system, private or intercountry adoption, or reunified with birth or removal parent(s) through achievement of guardianship or custody from foster care and currently reside in North Carolina. The Success Coach model, developed by Catawba County Social Services in partnership with The Duke Endowment and the National Implementation Research Network, utilizes structured assessment tools and interventions including clinical components, skill building, crisis prevention and intervention, case management, and advocacy. The specific face-to-face contact frequency is negotiated between the coach and family based on identified needs.

The program addresses multiple areas of family need including basic needs (employment, housing, safety, food security), child developmental concerns, mental and behavioral health, parenting support, medical needs, financial assistance and budgeting, natural support system development, educational support, familial bonding and attachment, trauma effects education, service coordination, community referrals, and respite care. Three North Carolina providers currently deliver Success Coach services: Catawba County Department of Social Services, Children's Home Society of North Carolina, and Boys & Girls Home of North Carolina. Referral sources can contact the designated Success Coach provider, who will follow up to obtain additional information and discuss next steps, including the possibility of a joint introductory visit.
