SECTION A
1. Complete Section A of this questionnaire (pages 2&3) for each reference.
2. Submit a copy of each Section A to the Contract Specialist with your proposal.
SECTION B
1. Complete Block 2 of Section B (Contract Number and Program Name) for each reference.
2. Submit the Past Performance Questionnaire directly to your references.
3. Request that your reference completes Section B of the questionnaire and then returns it directly to the Contract Specialist.
The offeror is highly encouraged to ensure that the references return Section B to the Contract Specialist by the proposal response date.

2. Contractor provided materials and products that met or exceeded contract requirements.

Contractor completed all efforts with good workmanship in conformance with the specifications. There were minor, if any, punch list items.

Contractor fulfilled all safety regulations without notice, warning, work suspension, or other outside motivation.

Response to spills, mishaps, or other incidents was timely. Reports were made as required. Collateral risk was mitigated promptly. If no incidents for the project, please write N/A.
