The contracts identified as past performance must be both RECENT and RELEVANT:

1. RECENT is defined as within the past 3 years.

2. RELEVANT is defined as similar in scope and magnitude to the work required by this solicitation.

3. SCOPE will be judged by comparing the offeror’s past performance to the tasks required in this solicitation’s Performance Work Statement (PWS) with consideration of the horticultural region where the work was performed.

4. MAGNITUDE will be judged by whether the property where grounds maintenance was performed was within the approximate range of 30 to 60 acres (or about +/-30% of 45 acres).

If possible, provide at least one of the three referenced contracts from a publicly owned institution (i.e. town, county, state, and federal) and one from a privately owned institution. At least one of the referenced contracts must be with an institution located in the state of New York.

Please identify contracts in which your firm has completed at least 50% of the work as the prime contractor, specifically work done by your firm’s labor force and/or equipment, but not solely management. Any resultant contract with the Government will incorporate the clause FAR 52.219-14, Limitations on Subcontracting, which requires that the prime contractor perform at least 50% of the total contract effort.

To the maximum extent practicable, identify contracts for properties which are not owned or operated by your firm or by key personnel, including intended subcontractors, of your firm.

INSTRUCTIONS FOR REFERENCE

You have been provided as a past performance reference in response to the solicitation FA8751-08-R-0008 for Grounds Maintenance at the Air Force Research Laboratory’s Rome Research Site, Rome NY. In order for the government to evaluate the past performance of the firm providing you as a reference, please complete Section B, Past Performance Questionnaire, and return it to the Contract Specialist identified above.

The individual knowledgeable of the contractor’s day-to-day operations and overall performance should complete this questionnaire. However, that individual is encouraged to supplement their own knowledge of the contractor’s performance with the judgment of others within their organization, as applicable. Thank you for your assistance.

GENERAL INFORMATION

Per Federal Acquisition Regulation (FAR) 12.206, the Government will consider past performance data from a wide variety of sources both inside and outside the Federal Government in accordance with the policies and procedures contained in FAR subparts 9.1, 13.106, and 15.3, as applicable.

After considering all past performance data gathered by the Government for this solicitation, an offeror’s past performance will be assigned an overall rating of either ACCEPTABLE or UNACCEPTABLE.

The past performance evaluations provided in Section B, Past Performance Questionnaire, will contribute significantly to the Government’s overall rating of an offeror’s past performance. For this part of the past performance evaluation:

(1) An offeror’s past performance will be considered ACCEPTABLE by the Government if most or all of the areas on all three referenced contracts are evaluated as 5 (High Confidence), 4 (Significant Confidence), or 3 (Satisfactory confidence).

(2) An offeror’s past performance will be considered NEUTRAL by the Government if most or all of the areas are evaluated as U (Unknown Confidence). If an offeror’s past performance is considered NEUTRAL, that offeror’s past performance is neither ACCEPTABLE nor UNACCEPTABLE.

(3) An offeror’s past performance will be considered UNACCEPTABLE by the Government otherwise.

SECTION A: Offeror completes this section to identify each past performance reference (3), then returns the forms to the Contract Specialist identified above. By completing this form, the offeror authorizes the past performance reference to complete the Questionnaire.
