PAST PERFORMANCE QUESTIONNAIRE

When filled in this document is source selection sensitive information IAW FAR 3.104

Please complete this questionnaire based on the following guidance:

1. You are urged to supplement your own knowledge of the offeror's performance with the judgment of others in your activity. In addition to completing the attached questionnaire for the identified program, we solicit your comments on other similar programs for which your activity has or has had contracts with this offeror. If you need more space than the comment sections provided, please attach a separate sheet to the end of this questionnaire.

2. Some of the terms used in this questionnaire are related to government contracting. Commercial firms are requested to answer to the best of your understanding of these terms.

III. EVALUATION:

Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. Provide comments in the block below each item if rated SATISFACTORY or below. This scale is defined as follows:

LEVEL

E EXCEPTIONAL Performance meets contractual requirements with many exceeded to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with no more than a few minor problems for which corrective actions taken by the contractor were highly effective.

G VERY GOOD Performance meets contractual requirements with some exceeded to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with no more than some minor problems for which corrective actions taken by the contractor were effective.

S SATISFACTORY Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor were satisfactory.

M MARGINAL Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.

U UNSATISFACTORY Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

N NEUTRAL Performance was not observed or not applicable to the current effort being reported against.
