1. DESCRIPTION OF SERVICES

This Performance Work Statement (PWS) defines service requirements to support operation of the hazardous materials (HAZMAT) pour down facility (PDF) in building 256 and the Ogden Air Logistics Complex (OO-ALC). The Contractor shall provide all management, labor, and equipment necessary to minimize HAZMAT usage while ordering, receiving, handling, transporting, distributing, and disposing of HAZMAT. This shall be in accordance with (IAW) all environmental regulations and Air Force Instructions (AFI) listed in Appendix B. This requirement is considered non-mission essential.

2. BASIC SERVICES

These services will serve to 'pour-down' the HAZMAT to size containers usable on aircraft and aircraft component maintenance lines, to distribute these products to the requiring cell activities, and to recycle or dispose of the products to minimize HAZMAT waste. These services require: manual lifting of over 50 pounds (lbs.); operating contractor-furnished general/special purpose vehicles to include an electric forklift and charger rated to National Fire Protection Association (NFPA) 70 Class I, Division 2; if used in Bldg. 256’s south-side staging area or to NFPA 70 Class I, Division 1; if used in Bldg. 256’s north-side mixing area, and delivery/pickup of material in depot supply or other outlying areas to include flight line areas where special licensing is required. The forklift must be able to operate within the building confines that it services.

2.1 HAZMAT PROCEDURES

2.1.1 The Contractor shall utilize the Hill Air Force Base (HAFB) Enterprise Environment Safety & Occupational Health Management System (EESOH-MIS) for ordering, receiving, and/or tracking hazardous materials.

2.1.1.1 The Contractor shall conduct and document monthly inventory on stored material, including hazardous items for various 309th Maintenance Group Complex (MXGC) shops at HAFB.

2.1.1.2 The Contractor shall take appropriate action on all received orders from Hazardous Distribution Service Center (HDSC).

2.1.1.3 The Contractor shall assign a document number for each order utilizing Air Force (AF) Form 2005 “Issue / Turn-In Request”.

2.1.1.4 The Contractor shall review and assign priorities to supply orders.

2.1.2 Pour Down/Delivery. The contractor shall be exposed to and required to work with, but not limited to, the various chemicals listed in Appendix D, B256 Constituent List.

2.1.2.1 The Contractor shall print, associate, and provide Occupational Safety and Health Administration (OSHA) compliant hazardous chemical labels and yellow EESOH-MIS labels on shop day-use and pour-down containers IAW OSHA Hazardous Communication (HAZCOM) Standard (29 Code of Federal Regulations (CFR) 1910.1200). During the labeling process the 75th Civil Engineer Group (CEG) Safety/Bio-Environmental Representative will inspect the required labels for compliance with OSHA standards. See Appendix H for sample of obtaining Globally Harmonized System (GHS) label elements.

2.1.2.2 The Contractor shall remove old labeling from recycled pour-down containers and affix all required labels to new and recycled pour-down containers before delivering to the requesting customer.

2.1.2.3 The Contractor shall print OSHA-compliant hazardous chemical labels for shop day-use and pour-down containers. The Contractor shall also provide OSHA-compliant hazardous chemical labels for shop day-use containers upon customer request. Customer requests are estimated to be 10 - 30% of the overall OSHA-compliant hazard chemical label printing workload.

2.1.3 The Contractor shall deliver pour-downed chemicals to HDSC in the flight line area during all weather conditions, document deliveries, and prepare material cost report. The Contractor shall perform an electronic transfer of chemicals to HDSC via EESOH-MIS.

2.1.3.1 The Contractor shall deliver pour-downed chemicals within one (1) business day after receiving the request in EESOH-MIS unless immediate notification of delays upon receipt of request. Examples of acceptable delays may be due to unforeseen breakdowns, excessive order volume, etc.

2.2 POUR DOWN FACILITY (PDF) MAINTENANCE & MANAGEMENT

2.2.1 The Contractor shall accomplish depot supply material functions IAW all applicable Department of Defense (DoD) and Air Force regulations, including, but not limited to, AFI 32-7086, Air Force Manual (AFMAN) 23-110 Volume III – Part 2, Air Force Material Command Instruction (AFMCI) 21-129, AFMCI 21-130, AFMCI 21-133, AFI 48-106, and their applicable Hill and OO-ALC supplements.

2.2.2 The Contractor shall provide maintenance material operations to include being proficient with and utilizing the EESOH-MIS to authorize, order, receive, issue and account for hazardous material. The Contractor shall provide Contracting Officer Representative (COR) with EESOH-MIS Training Certificate.

2.2.3 The Contractor shall pick-up, inventory, and restock reusable containers.

2.2.4 The Contractor shall manage shelf-life items to eliminate hazardous waste disposal IAW Department of Defense Issuances (DoDI) 4140.27-Volumes 1 and 2.

2.2.5 The Contractor shall keep the facility clean and safe according to installation facility requirements and OSHA 29 CFR 1910 Industrial Standards.

2.2.6 The Contractor shall coordinate with the HAFB hazardous materials and hazardous waste management programs. The Contractor shall contact the Hazardous Waste Control Facility for the transportation and disposition of hazardous waste generated in the PDF.

2.3 TRAINING

2.3.1 Contractor personnel shall attend, at contractor expense, all applicable training required by law, Office of Personnel Management, DoD, and Air Force (AF) policy, including Air Force Material Command (AFMC) and local policy. The Contractor shall have available a current roster of personnel trained to provide to the COR.

2.3.2 The Government shall provide training if determined to be in the best interest of the government. Appendix G lists government provided training applicable to this contract.

2.3.3 For mandatory training that is only available from the government, the Contractor shall request to attend training courses by submitting the request no later than (NLT) 30 days after the transition start date, unless mandatory completion date requirement dictates otherwise. For contractor employees that have previously attended the training, the Contractor shall provide documented evidence of attending subject training or equivalent training at start of performance. The Contractor shall ensure that contractor employees are trained in the following subject areas to include, but not limited to: Safety/Fire Prevention/Operational Risk Management (ORM), Security Awareness, Operations Security (OPSEC), Flight-Line Drivers Training, Information Assurance Awareness Training, and OSHA-driven training such as HAZCOM and Hazardous Waste Operations and Emergency Response (HAZWOPER).

2.3.4 For specialized training that is only available from the government, the Government shall provide specialized training when available and in the best interest of the government. Specialized training may include, but not limited to, EESOH-MIS and flight line drivers training.

2.3.5 Personnel required to operate vehicles on the flight line shall be trained, certified, and licensed to do so. The Contractor is responsible for scheduling the training through 309th Maintenance Support Group (MXSG) Training Officer, with adequate lead time not to interfere with the contract schedule. Upon completion of the training, an AF Form 483 (Certificate of Competency) shall be issued by the Airfield Manager’s office, which certifies that the individual has been trained on flight line driving. This certification must be in the possession of the vehicle operator when driving on the flight line IAW AFI 13-213.

3. SERVICE SUMMARY (SS)

2.1.1

The Contractor shall print, associate, and provide OSHA-compliant hazardous chemical labels and yellow EESOH-MIS labels on all pour-down and shop day-use containers.

2.1.2.1 (Satisfactory) Objective: 100% compliance by the contractor.

(Satisfactory) Threshold: The Contractor shall receive less than satisfactory rating if 95% compliance is not met.

The Contractor shall deliver all pour-down requests to the requesting HDSC with OSHA-compliant and EESOH-MIS labels. Remove all old labels from container prior to delivery.

2.1.3 2.1.3.1 (Satisfactory) Objective: The Contractor shall perform delivery within one (1) business day of order receipt. Unless immediate notification of delay upon receipt of request.

(Satisfactory) Threshold: The Contractor shall receive less than satisfactory if the contractor has more than two (2) verifiable late deliveries per quarter.

The Contractor shall provide maintenance material operations to include being proficient with and utilizing the EESOH-MIS.

2.2.2 (Satisfactory) Objective: The Contractor shall provide COR with EESOH-MIS Training Certificate.

(Satisfactory) Threshold: The Contractor shall provide certificate NLT 30 days after contract award and within seven (7) days of any new certification training.

The Contractor shall provide monthly summary reports on hazardous material identifying re-order materials, shelf life, inventory, and backorder status.

5.1.1 (Satisfactory) Objective: The Contractor shall provide the COR with the monthly summary report by the 5th business day of each month (CDRL A/B/C/D/E001).

(Satisfactory) Threshold: The Contractor shall receive less than satisfactory if delivery is late more one (1) time during the evaluated period of performance (POP).

The Contractor shall provide on a quarterly basis an analysis of monthly workload trends by providing charts and graphs.

5.1.2 (Satisfactory) Objective: The Contractor shall provide the COR analysis by 5th business day of each month following the calendar quarter (CDRL A/B/C/D/E002).

(Satisfactory) Threshold: The Contractor shall receive less than satisfactory if delivery is late more one (1) time during the evaluated POP.

The Contractor shall provide monthly cost reports for operations and submit the reports with the monthly billing.

5.1.3 (Satisfactory) Objective: The Contractor shall provide the COR with monthly cost reports by the 5th business day of each month (CDRL A/B/C/D/E001).

(Satisfactory) Threshold: The Contractor shall receive less than satisfactory if delivery is late more one (1) time during the evaluated POP.

The Contractor shall provide reports on internal/external inspections and reconciliation audits. External reports will be provided to COR within one (1) business day of when the inspection occurs/completed. Internal reports shall be submitted monthly.

5.1.4 (Satisfactory) Objective: The Contractor shall provide the COR with reports by one (1) business day for External Inspections and for Internal Inspections submit monthly. (CDRL A/B/C/D/E003).

(Satisfactory) Threshold: The Contractor shall receive less than satisfactory if delivery is late more one (1) time during the evaluated POP.

The Contractor shall respond and take corrective action to the quarterly Hazardous Materials Annual Inventory Reconciliation and Evaluation (HM-AIRE) performed by 75th CEG/CEVC.

5.7.8 (Satisfactory) Objective: 100% compliance by the contractor.

(Satisfactory) Threshold: The Contractor shall receive less than satisfactory rating if 95% compliance is not met and corrections are not made within five (5) business days to bring the contractor intolerance.

4. GOVERNMENT FURNISHED PROPERTY AND SERVICES

4.1 Government Furnished Property (GFP) is not applicable to the performance of this contract. Shared Property of the government is applicable to the performance of this contract.

4.2 Shared Property of the Government (As Is Condition). Shared Property of the government shall remain at all times property of the government. In accordance with government procedures, the contractor is authorized to share property of the government, without cost, with government personnel in government facilities at HAFB, UT for the duration of this contract and only as needed for the performance of this contract. The government maintains accountability for this shared property of the government. This shared property of the government is not considered GFP requiring property administration IAW FAR Parts 45 and 52.245, plus supplements. The government organization accountable for this shared property of the government is also responsible for its disposition.

4.3 Shared property of the government includes engineering drawings and data packages associated with maintaining these systems. Shared property of the government includes, but is not limited to, the pour-down facility at building 256, a Gerber Graphic Printing System with Omega 5 software, two computer systems accessing the base network and a printer, four (4) 55 gallon drum pumps, refrigerators/heaters, basic pour-down equipment and ventilation hoods, shelving for storage, and emergency eye wash equipment.

5. GENERAL INFORMATION

5.1 DATA DELIVERABLES

5.1.1 The Contractor shall provide monthly summary reports on hazardous material identifying re-order materials, shelf life, inventory, and backorder status. The reports shall be due on the 5th business day of each month for services rendered for the previous month, and shall be 100% error free (CDRL A/B/C/D/E001, DI-MGMT-81991). The error rate shall be based on number of findings divided by the number of possible errors, accomplished by COR surveillance on a random basis.

5.1.2 The Contractor shall provide to the COR a quarterly month-to-month workload trend analysis with correlating charts and graphs, to be submitted by the 5th business day of each month following the calendar quarter (CDRL A/B/C/D/E002, DI-MISC-80508B).

5.1.3 The Contractor shall provide a monthly cost report with each monthly billing, to be submitted to the COR by the 5th business day of each month (CDRL A/B/C/D/E001, DI-MGMT-81991).

5.1.4 The Contractor shall provide reports on internal (contractor) and external (government) inspections and reconciliation audits. The reports shall list findings, root causes, and remedies (CDRL A/B/C/D/E003, DI-ADMIN-81313A). External inspection reports shall be provided to the COR within one (1) business day of when the inspection occurs/completed. Internal inspection reports shall be provided to the COR monthly.

5.1.5 The Contractor shall provide from its OSHA 300 information the Total Case Incident Rate/Days Away Restricted or Transferred (TCIR/DART) rates by 15 January of each year to the COR/Safety Office (OO-ALC/SEG) for submission as part of the installation’s annual OSHA Voluntary Protection Program (VPP) self-evaluation report (CDRL A/B/C/D/E004, DI-SAFT-81563).

5.1.6 The Contractor shall submit mishap notification NLT three (3) calendar days after a mishap. Additional information shall be provided NLT 20 calendar days after the mishap (See Appendix C) (CDRL A/B/C/D/E004, DI-SAFT-81563).

5.1.7 The Contractor shall submit a hazardous materials management program plan NLT seven (7) calendar days after contract (DAC) award (CDRL A/B/C/D/E005, DI-MGMT-81398C). This data item is critical and requires advance written approval by the procuring agency before the contractor can proceed.

5.1.8 The Contractor shall submit a safety plan NLT fifteen (15) calendar DAC award (CDRL A/B/C/D/E006, DI-SAFT-81626). This data item is critical and requires advance written approval by the procuring agency before the contractor can proceed.

5.2 CONTRACTOR BASE/NETWORK ACCESS SECURITY

5.2.1 Contractor Base Access Security. Defense Biometric Identification System (DBIDS) Badge/CAC/Controlled/Restricted Authorization Badge (CAB/RAB) IAW HAFB Instruction 36-812 and Hill Integrated Defense Plan (IDP) IC 1. The Contractor must meet the same security requirements as government employees for access to the installation, Site Air Force Base’s (AFB) network or Automated Information Systems.

5.2.1.1 Contractor Identification Badges. DBIDS card may be issued after contractor submits a completed AFMC Form 496, Application for AFMC Identification Card to the COR requesting a DBIDS. The COR completes and forwards the Form 496 to the Security Manager (SM) for signature and processing. The SM will notify the COR after the Form 496 is approved with instructions for obtaining a DBIDS. Employee will return the completed 496 to issuing SM to begin the in-processing. Contractors with derogatory information on their Form 496 will be disapproved for DBIDS and will not be granted access to the installation. NOTE: Escort authority will not be authorized and Force Protection “Bravo” is the minimum established by Security Forces Squadron (SFS). Contractors with a current DBIDS have authority to sponsor (not escort) onto the installation and must follow installation procedures for doing so. Contractors who do not require access to DoD Information Technology (IT) systems will be issued a DBIDS only.

5.2.1.1.1 Contractor employees approved for DBIDS will require sponsorship onto the installation to SFS (building 408) to be issued a DBIDS. Employees going to SFS without being sponsored are considered to be on the installation illegally.

5.2.1.2 A Common Access Card (CAC) will be issued at the task order level if access to government IT systems is required to perform the job. CAC cards will not be issued for training purposes only IAW AFBI 36-812. CAC cards will not be issued to foreign nationals unless they meet the requirements of AFI 31-113, Installation Perimeter Access Control (FOUO).

5.2.1.2.1 Contractor employees requiring access to government computer systems will be required to pass a Local File Check (LFC), positive fingerprint results, a completed Form 306, Declaration of Federal Employment and a minimum of a Tier 1 (T1) investigation through the Group security office. If all forms are returned with no derogatory information and the T1 investigation is opened with Office of Personnel Management (OPM) then an interim CAC card may be issued by the respective Group SM. If the T1 is returned with less than favorable results the contractor employee will have their CAC card revoked and any access to government IT systems. A local adjudication will be conducted and if found unfavorable the employee will be removed from the installation immediately.

5.2.1.2.2 Contractor employees will be given two (2) weeks to complete the on-line investigation questions (e-QIP) prior to being submitted for a T1. Employees that refuse to complete the questionnaire, or fail to meet the deadline, will be removed from the installation. On-line questionnaire will be initiated by the Group Security Manager (SM).

5.2.1.2.3 Closed investigations, with favorable results, must be within last two (2) years from previous government employment, service or appointment to be used in lieu of submitting for a new background check or security clearance.

5.2.1.2.4 Contractor employees that do not meet the minimum requirements for a CAC will be removed from the Installation immediately. All government credentials will be recovered and returned to the issuing SM the same day.

5.2.1.2.5 Contractor employees requiring a higher level of investigation must be submitted for a Tier 3 (initial) or a Tier 3R for (Reinvestigations) if over 10 yrs. A Single Scope Background Investigation (SSBI) for Top Secret, must be submitted IAW AFI 16-1405, Personnel Security Management Program, AFI 33-282, Computer Security. The Field Security Officer (FSO) for the Contractor is responsible for initiating the required level of investigation above a T1 background investigation. The Contractor shall meet the same security requirements as Government employees for access to the site AFB network and connections to an office printer, office supplies and any other items necessary to complete the assigned tasks. The Contractor will only have access and use of the office equipment mentioned above. The Government supervisor or equipment custodian for the organization will monitor the accountability of the GFP provided for the contractor employee’s use.

5.2.1.3 Controlled/Restricted area badge requests shall be completed by the group SM, who shall complete a LFC and an AF Form 2586, Unescorted Entry Authorization Certificate for each employee requiring entry into controlled/restricted areas. The Contractor employee shall carry the Form 2586 to Security Forces (building 408-West side). The Form 2586 will be returned to the issuing SM for accountability. NOTE: CAB/RABs will not be issued for the inconvenience of having to be escorted.

5.2.1.3.1 Contractor employees requesting unescorted access into a controlled area will have a valid DBIDS or CAC card. If the employee has a current DBIDS then a new 2583 will be done if more than 30 days has passed since the issuance of the DBIDS.

5.2.1.3.2 The Contractor shall complete, annually, Security Education and Motivation Phases I and II prior to submitting for a Form 2586 and any additional required training, based on access requirements.

5.2.1.3.3 The Contractor shall provide written justification with employee full name, Social Security Number, Height, Weight, Eye Color, Hair Color and contract number with expiration date and areas needing access to the issuing SM to complete a Form 2586.

5.2.1.3.4 Contractor employees must meet the minimum background requirements prior to submitting for a CAB/RAB: positive LFC, have a current investigation or current security clearance verifiable through Joint Personnel Adjudication System.

5.2.1.3.5 Contractor employees shall display the CAB in a badge holder between the shoulders and above the waist and in the front of the body whenever inside an identified restricted or controlled area. NOTE: When outside restricted/controlled areas the CAB will be placed out of sight and not displayed.

5.2.1.3.6 Contractor employees transferring between contracts, groups or companies will return DBIDS or CAC, CAB/RAB and vindicator card and will follow paragraph 5.2.1.5 for in/out processing. New credentials will be issued by the receiving organization. NOTE: contractor employee will be required to be escorted while on the installation until new credentials are issued.

5.2.1.4 As required, the contractor employee shall also obtain a Building ID and/or vindicator access card from the appropriate Building Facility/SM. Access to controlled/restricted areas will not be granted without a CAB/RAB badge and proper authorization for required areas.

5.2.1.5 Contractor employees will complete the same in/out processing checklist from the assigned organization. Employees removed from the contract will be required to return all issued government credentials and vindicator key to the issuing office within the same day of separation. If employee is unable to return the credentials and vindicator key, then the supervisor will ensure that all credentials are returned to the issuing SM the same day.

5.2.1.6 When the POP has ended or the task is considered complete, contractors shall have one (1) working day to terminate all their network user accounts and to return all access cards and base identification badges to COR or Group Security Manager.

5.2.1.7 During performance of the contract and at the completion or termination of the contract, the Contractor shall be responsible for prompt return of credentials to the appropriate security manager for any employee who no longer requires access to the work site, and for obtaining required identification of newly assigned personnel. Lost badges shall be reported to the Maintenance Group Security Officer within one (1) working day of loss. After receiving a Lost Identification Letter from the issuing security office, the contractor is required to fill out an AF Form 1168 through the Security Forces Office. The contractor must bring AF Form 2586 to the Security Forces Offices to be issued a new badge then return the form to the Group Security Office.

5.2.1.8 Should a security incident occur, the Contractor shall: immediately report to the site location Group Security Manager all available facts related to each instance; take such precautions as the Security Manager may reasonably require for security purposes; take reasonable and prudent action to establish control of the scene, prevent further violation, and to preserve evidence until released by proper authority; and cooperate fully and assist government personnel as the government conducts an investigation of the violation. Participation shall last until the investigation is completed.

5.2.1.9 The contractor shall create an Incident/Mishap report when a contract employee loses a CAC or Restricted/Controlled Area Badge Line badge. The Incident/Mishap report must be submitted NLT one (1) business day.

5.2.2 Contractor Network Access Security. Access to the HAFB computer network will be required. Comply with local unit in and out processing procedures for all Contractor personnel to ensure proper security related briefings and debriefs are accomplished. Comply with all security, force protection, and Antiterrorism requirements enforced at each installation.

5.2.3 Comply with local unit in and out processing procedures for all Contractor personnel to ensure proper security related briefings and debriefs are accomplished. Comply with all security, force protection, and Antiterrorism requirements enforced at each installation.

5.3. PHYSICAL SECURITY, KEY CONTROL, AND OPERATIONS SECURITY

5.3.1 Contract personnel will be working in building 256 which is behind the flight line fence line (Pacer Protect area). Therefore all contracted personnel will be required to obtain flight line badges and will be issued a vindicator card for electronic key access to designated gates and facilities behind the flight line.

5.3.2 During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel. At the completion of the contract or at the termination of any employee, the contractor shall return credentials to the Contracting Officer (CO) and Unit Security Manager.

5.3.3 The Contractor shall be responsible for safeguarding all government shared property provided for contractor use. At the close of each work period, property and materials shall be secured.

5.3.4 Key Control. The Contractor shall establish and implement methods of ensuring that all keys/key cards issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons. No keys issued to the contractor by the Government shall be duplicated. NOTE: All references to keys also include key cards (vindicator cards).

5.3.5 The Contractor shall immediately report the occurrences of a lost or duplicate key to the appropriate Security Managers. The contractor shall, upon written direction of the contracting officer, rekey or replace the affected lock or locks; however, the government, at its option, may replace the affected lock or locks or perform rekeying. When the replacement of locks or rekeying is performed by the government, the total cost of rekeying or the replacement of the lock or locks shall be deducted from the payment due the contractor.

5.3.6 The Contractor shall prohibit the use of keys issued by the government by any persons other than the contractor’s employees. The Contractor shall prohibit the opening of locked areas by contractor employees to permit entrance of persons other than contractor’s employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Security Managers.

5.3.7 OPSEC. The Contractor shall familiarize all new employees and conduct refresher sessions in the areas of Operations Security (DoD Contract Security Classification Specification) addendum. Operations Security (AFI 10-701 Operations Security as supplemented, and AFI 16-1404 Information Security Program, Standards for Storage and Handling Classified Material as supplemented).

5.4 HOURS OF OPERATION

5.4.1 Normal Hours of Operation. The Contractor shall have access to perform services during normal working hours up to Force Protection Condition (FPCON) Bravo. Normal shop working hours at HAFB are typically 6:00AM – 2:30 PM local time Monday through Friday. The contractor may work, with prior approval of the COR, extended hours to ensure timely completion of work at no additional cost to the government.

5.4.2 Recognized Holidays. The Contractor is not required to provide services on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day, and Christmas Day. If the Holiday falls on a Saturday, it will be observed on Friday. If the Holiday falls on a Sunday, it will be observed on Monday.

5.4.3 In the event that the contractor is prevented from performance as the result of an Executive Order or an Administrative leave determination that applies to the using activity, such time may not be charged to the contract.

5.5 GOVERNMENT OBSERVATIONS

5.5.1 The Government will periodically evaluate the Contractor’s performance through inspections IAW with FAR 52.246-4 Inspection of Services. Government personnel, other than CO/COR may from time-to-time, observe contractor operations. Government personnel may not interfere with contractor performance, and only the CO may make changes to the contract.

5.6 SAFETY REQUIREMENTS

5.6.1 The Contractor shall follow Appendix C and adhere to the Air Force as part of OSHA.

5.6.2 System Safety Program Plan. The Contractor shall implement a safety program that ensures protection of government personnel and property (See Appendix C). As part of the Safety Program the contractor shall establish a Safety Plan IAW Appendix C of this PWS. The contractor shall submit the Safety Plan within fifteen (15) calendar DAC award. (CDRL A/B/C/D/E006, DI-SAFT-81626).

5.6.3 Voluntary Protection Program (VPP). The contractor is required to attain familiarization with the requirements of VPP. Information on the VPP can be accessed at http://www.osha.gov/dcsp/vpp/index.html. VPP requirements apply to contractors whose employees work more than 1,000 hours per quarter. The contractor shall flow down these requirements to all subcontractors who qualify as applicable contractors under this contract.

5.6.4 Total Case Incident Rate/Days Away Restricted or Transferred (TCIR/DART). The Contractor shall provide from its OSHA 300 information the TCIR/DART rates (CDRL A/B/C/D/E004, DI-SAFT-81563) by 15 January of each year to the COR/OO-ALC/SE for submission as part of the installation’s annual OSHA VPP self-evaluation report.

5.6.5 The Contractor shall ensure their employees are made aware that the performance of these services will occur in industrial areas. The pour-down facility workload and the industrial complex at HAFB has the potential to expose workers to hazardous materials which may include, but is not limited to, methyl ethyl ketone, toluene, isopropyl alcohol, and ethyl acetate as constituents of cleaning solvents; and hexavalent chrome and cadmium as byproducts of paint.

5.6.6 The Contractor shall mitigate the operational risk hazard identified with unattended personnel performing actual pour-down activities, such as by camera surveillance or by accomplishing tasks via a two-person ‘buddy’ system. While the contractor will not perform duties in a marked and signed regulated area (where exposures are expected to exceed the permissible exposure limits set by 29 CFR 1910), there may be contact hazards with these materials during cleanup. The Contractor shall ensure proper protective measures and training are taken to protect contracted employees from these hazards. The 75th Air Base Wing (ABW) Bioenvironmental Engineering Flight has produced industrial hygiene assessments of these areas which are attached.

5.7 HAZARDOUS MATERIAL AND ENVIRONMENT CONTROLS

5.7.1 The Contractor shall implement a Hazardous Materials Management Program (HMMP) Plan as described in paragraph 4.3 of National Aerospace Standard (NAS) 411 (CDRL A/B/C/D/E005, DI-MGMT-81398C). The Contractor’s HMMP shall help minimize its operations’ impact to the environment and to comply with applicable laws, regulations, and other environmentally oriented requirements. The Contractor shall ensure their employees are made aware that the performance of these services will occur in industrial areas. The Contractor shall ensure proper protective measures and training is taken to ensure contracted employees are protected from these hazards. Basic HMMP training shall be completed prior to contractor personnel transporting or using any potentially hazardous materials on the installation. The Contractor shall provide employees with initial 40-hour Hazardous Waste Operations Emergency Response, annual 8-hour Hazardous Waste Operations Refresher per 29 CFR 1910.120 and 29 CFR 1910.1200. Contractor personnel shall obtain training specified in the contractor’s Health and Safety Plan.

5.7.2 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change (such as the transition to Global Harmonization System [GHS] of Chemical Classification and Labeling) during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.

5.7.3 The Contractor and its employees shall comply with all Federal, State and local regulations when transporting and delivering hazardous materials outside of HAFB (i.e. Little Mountain, Utah Test and Training Range (UTTR), etc.) including manifesting and personnel Department of Transportation certifications where applicable. The Contractor shall maintain and store all Hazmat transportation documentation (i.e. manifests) on-site and will have this documentation available for immediate inspection.

5.7.4 Notification of Environmental Spills. If the contractor spills or releases any hazardous chemical into the environment, the Contractor or its agent shall immediately clean-up incidental spills in accordance with established procedures, or if spill is significant, report the incident to HAFB Fire Department at 911 and evacuate according to established emergency action plans. The liability for the spill or release of such substances rests solely with the contractor and its agent.

5.7.5 Safety Data Sheets (SDSs). A hazardous material control program shall be in effect to inform employees of potential hazards if and when chemicals are used. The Government will provide and maintain access to applicable SDSs IAW OSHA 29 CFR 1910.1200 and Hill AFB Instruction (HAFBI) 48-106 for review of safety and health information.

5.7.6 Material Storage and Use. The Contractor shall store hazardous material in the process and manner for which it is intended by the manufacturer and as authorized by the HDSC. The Contractor shall follow manufacturer’s guidelines for control of humidity, temperature, cleanliness, and materials handling. This includes the handling, tracking, and storage of hazardous materials, to include flammable and combustible liquids as identified in AFI 32-7086, 29 CFR 1910 Subpart H, AFI 91-203 Chapter 22, and NFPA 30. The Contractor shall maintain housekeeping/storage methods and procedures of material/work residue to ensure items do not migrate out of the immediate work area to become a potential Foreign Object Damage (FOD) or fire hazard, and shall inventory all hazardous material lockers for expired materials on a monthly basis. Expired materials shall be managed in IAW the 309 Maintenance Wing (MXW) Hazardous Materials Shelf-Life Management Policy.

5.7.7 The Contractor shall collect spent, used, and expired hazardous material to a container that is managed IAW Resource Conservation and Recovery Act (RCRA) and the HAFB Hazardous Waste Management Plan. In this effort, the Contractor shall comply with 40CFR hazardous waste management regulations as specified in the RCRA.

5.7.8 The Contractor shall respond and take corrective action within five (5) business days to the quarterly Hazardous Materials Annual Inventory Reconciliation and Evaluation (HM-AIRE) performed by 75th Civil Engineer Group/Environmental Division (CEG/ CEVC).

5.7.9 Identification of unauthorized materials and missing High Vapor Pressure Solvent (HVPS) materials, as well as HAZMAT management practices, will be audited. The criteria and evaluation factors are as follows:

5.7.9.1 The Contractor must meet 5% or less error rate. If the contractor does not meet the initial 5% or less error rate, the Contractor must make corrections within five (5) calendar days. The error rate percentage (%) is calculated as follows:

5.7.9.1.1 The Initial Error Rate % is equal to the 'Total Number of Findings' divided by the 'Total Number of Possible Errors'. The ‘Total Number of Findings’ is the sum of the number of container errors from Finding 1 and Finding 2 (this does not include containers identified as EESOH-MIS errors). Finding 1: Material handed-out physically, but not issued electronically as directed by AFI 32-7086 (i.e. material not issued properly). Finding 2: Issued material was physically returned to HDSC, but not electronically turned-in (i.e. material not turned-in properly). The ‘Total Number of Possible Errors’ is equal to the sum of the number of containers in the electronic and physical inventory of HDSC256.

5.7.10 The Contractor shall implement procedures consistent with the 309th MXSG Memorandum, Hazardous Materials Shelf Life Management Policy.

5.8 MANPOWER REPORTING

5.8.1 The Contractor shall report ALL contractor labor hours (including sub-contractor labor hours) required for performance of services provided under this contract via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil/.

5.8.2 Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the Contractor Manpower Reporting Application (CMRA) help desk.

5.8.3 Uses and Safeguarding of Information. Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link at http://www.ecmra.mil/.

5.8.4 Unit Identification Code (UIC): FFB4G0

5.9 FOREIGN OBJECT DAMAGE (FOD) AND TOOL CONTROL

5.9.1 The Contractor shall follow the applicable guidance in chapters/paragraphs/ sections of Air Force Instruction (AFI) 21-102, Air Force Material Command (AFMC) Supplement to 21-102, Air Force Sustainment Center Manual (AFSCMAN) 21-102, Chapter 10, Ogden Air Logistic Complex (OO-ALC) supplements to the AFI 21-102 concerning base tool (chapter 10), and FOD control policies (chapter 14, and sub paragraphs designated as 41), daily. Cloth rags are considered tools and will follow the same regulations and procedures as tools.

5.9.2 If tools, and/or equipment are lost, the Contractor shall report the loss to the COR NLT two (2) hours after discovery. If tools are lost or not accounted for, then the Contractor shall implement the lost tool procedures of AFMC supplement to AFSCMAN 21-102, Chapter 10, section B.

5.9.3 Contractors performing functions in industrial areas on the center shall have a method for controlling and accounting for all tools used. All contractors working on the center shall inventory their tools at the beginning and end of each shift and at the end of each task to check for any lost or missing tools.

5.9.4 The Contractor shall follow control/restrictions of cell phones and personal electronic devices within designated high potential FOD areas (reference chapter 1 sub paragraph 35, Communication) of AFI 21-102 and Headquarters (HQ) AFMC, HAFB, and OO-ALC supplements.

5.10 PUBLICATIONS:

5.10.1 Compliance with all publications, regulations and operating instructions provided by the government is required when:

5.10.1.1 They pertain to the procedures for materials expediting herein and where the contractor is authorized by the PWS to accomplish the work specified in the publication, regulation or operating instructions.

5.10.1.2 The publications prescribe AF policies, use of materials, procedures and processes applicable to the work requirements.

5.10.1.3 The contractor is required to acquire and work on the latest version of the publication.

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

The contractor will implement a safety program plan that ensures protection of Government personnel and property. As part of the Contractor Safety and Health Program, the contractor shall assign in their plan, by name and phone number, a person who will be the primary point of contact for safety and health issues for the on-site operation. The program will consist of, as a minimum:

1. Mishap reporting, as defined in paragraph B1 below.

2. A Safety Plan that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and will be used during the performance of the work described in the contract. The Safety Plan will be accepted by OO-ALC/SE (Safety Office) prior to commencement of any work described in this contract.

3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

4. Competent personnel to provide surveillance of the Safety Plan.

5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.

6. All contractor personnel shall be trained and qualified to perform their duties safely.

7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.
