PERFORMANCE-BASED WORK STATEMENT (PWS)
FOR
B-2 Specialized Tooling/Special Test Equipment

1. Description of Services:
1.1 The Contractor shall provide all equipment, tooling, and personnel to transport all
B-2 Specialized Tooling/Specialized Tooling Equipment (ST/STE) and storage shelving from the Freeport Center in Clearfield, UT to the Tooele Army Depot
(TEAD) in Stockton, UT. Government personnel will ensure the Freeport Center facility is manned and accessable from 4:30 AM to 12:00 AM Monday through
Friday. TEAD (unloading) personnel will be available from 7:00 AM to 12:00 PM
Monday through Thursday. Completion of transportation of all requirements outlined in this PWS shall be completed no later than (NLT) 15 January 2017.
2. Scope:
2.1 The contractor shall provide the following services for the tooling move:
2.1.1 Loading of B-2 ST/STE
2.1.2 Loading of storage shelving
2.1.3 Blocking, bracing and securing all equipment on transportation vehicles
2.1.4 Delivery of all equipment to designated unloading point at TEAD
3. Basic Services:
3.1 Loading of B-2 ST/STE:
All ST/STE (Freeport Center) will be staged in designated loading zones by
Government personnel. Upon Government personnel staging equipment, the contractor shall utilize contractor owned equipment to load trucks for transportation in accordance with (IAW) para 3.3. Government personnel will ensure that each item or crate will have a DD 1149 and excel spreadsheet (if applicable) affixed to the item or crate prior to movement of the item(s). Contractor personnel shall not load any items, without the proper documentation affixed to each item. Prior to transport, Government personnel will ensure that a “master” DD 1149 is completed identifying all equipment on the load. Contractor personnel shall not begin transport without the
“master” DD 1149 completed and in possession of the designated driver. All ST/STE shall be covered during inclement weather to ensure no environmental damage occurs to ST/STE.

3.2 All current storage shelving located at the Freeport Center will be disassembled.
Upon teardown of the shelving, the contractor shall load and prepare for transport
IAW para 3.3. The shelving removal and transport will be a coordinated effort between Government personnel and contractor personnel. The intent of the shelving removal is to increase designated loading zone space by simultaneously unloading shelving and immediately removing the empty shelving upon removal of equipment from the shelves.
3.3 Blocking, bracing, and securing all equipment on transportation vehicles:
The contractor shall ensure that all equipment will be blocked, braced, and secured in a manner that will protect the equipment during transit. The contractor shall be responsible for the safe keeping and ensuring no damage occurs to equipment once it has been inspected and deemed ready for loading. Any identified damages to
ST/STE shall be annotated, photographed, and recorded prior to loading. The form shall be provided to TEAD personnel.
3.4 Crating or boxing of small ST/STE items:
The government will box or crate all small items prior to movement of items into the loading zone. All boxes and crates shall have a tamper resistant seal affixed to box or crate. All items that are boxed or crated will be inventoried and labeled with a
DD1149 affixed to the container prior to movement to the designated loading zone.
The contractor shall ensure that a DD1149 and tamper resistant seals have been affixed prior to loading onto transport vehicle.
3.5 Delivery of all equipment to designated unloading point at TEAD:
Each load shall be delivered to a specified location at TEAD (approximately 80 miles each way). TEAD personnel will identify the unloading zone prior to the execution of this contract. Once the driver of the load reaches the specified location, TEAD personnel will be standing by to unload the transport vehicle. Prior to unloading the equipment from the transport vehicle, the contractor shall provide all necessary documents (DD1149’s, spreadsheets, etc.) to TEAD personnel for item verification prior to unloading of the vehicle.

3.6 The contractor shall develop the following schedules throughout the life of the contract:
3.6.1 The contractor shall provide and estimated “Master Schedule” of the estimated time of the contract from start to finish. The Contractor shall provide the
Master Schedule within 10 calendar days of contract award to the Contracting
Officer (CO) and Government subject matter expert (SME) at site facility
(Freeport Center). I.E. If contract award is 1 August 2016, the contractor shall provide the Master Schedule to the CO and SME NLT 10 August 2016.
3.6.2 The contractor shall provide a “Daily Delivery Schedule” for all scheduled loads NLT three (3) days prior to the actual delivery of each daily load. The
Daily Delivery Schedules will be communicated to the TEAD movement point of contact (POC), by the CO or SME, in order to comply with the interagency
Statement of Work.
3.6.3 If any load requires specialzed equipment (i.e. a crane), the contractor shall identify the need NLT 10 days prior to delivery of the specified load. All oversized/overweight deliveries are shall also be identified 10 days prior to delivery of specified load. Oversized/ overweight deliveries fall within the specialized equipment perameters. Any identified specialized equipment requirements, will be communicated to the TEAD movement point of contact
(POC), by the CO or SME, in order to comply with the interagency Statement of Work.
3.6.3.1 Determination of oversized and/or overweight loads are as follows:
Oversized: Tooling assets greater than 180 inches in width and/or height
Overweight: Tooling assets weighing in excess of 14,000 pounds

5. General Information
5.1 Safety: In performing work under this contract, the contractor shall:
5.1.1 The contractor shall be knowledgeable of and comply with all applicable
Interstate, Federal, State, and Local laws, regulations, and requirements regarding transportation requirements.
5.1.2 Provide all personal protective equipment (PPE) required for their employees.
5.1.3 Conform to the safety requirements contained in this contract for all activities related to the accomplishment of the work.
5.1.4 Comply with additional, immediate precautions that the CO may require for safety and mishap prevention purposes.
5.1.5 Provide protection to government property to prevent damage during the period of time the property is under the control, or in possession of, the contractor.
5.1.6 Some of the equipment is identified as “asbestos HAZMAT”. The identified equipment does not pose a HAZMAT risk in its current state. The only potential asbestos risk is if the equipment were modified by sanding, grinding, milling, etc.
5.1.7 In the event of an accident or mishap, the contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.
5.1.8 Record and report promptly (within one hour) to the CO and/or authorized
Technical Program Manager (TPM) or above, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.
5.1.9 If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.
5.2 Insurance
5.2.1 The contractor shall provide insurance to cover all ST/STE during all times that
ST/STE is in contractor control. The contractor shall ensure that insurance documentation is in the possession of drivers at all times.
5.3 Physical Security
5.3.1 The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, property, and materials shall be secured.
5.3.2 The contractor shall provide a list of all drivers that have a bona fide need to access the TEAD location upon contract award. The contractor shall provide current visas for any personnel that are not United States citizens. Upon receipt of list and visas (if applicable), TEAD personnel will provide access to facility to all identified contractor personnel. The contractor shall provide all requirements in this section to the CO within 10 calendar days of contract award.
5.4 Hours of Operation
5.4.1 See Descriprion of Services PWS para 1.
5.5 Recognized Holidays
5.5.1 The contractor is not required to provide service on the following days: New
Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving
Day and Christmas Day. If the holiday falls on Saturday, it is observed on
Friday. If the holiday falls on a Sunday, it is observed on Monday.
5.6 Government Observations
5.6.1 The Contracting Officer Representative (COR) is the authorized individual who will perform assessments of the contractor’s performance.
5.6.2 The COR will inform the CO when discrepancies occur and will request corrective action. The COR will make a notation of the discrepancy on their assessment checklist with the date and time the discrepancy was noted and will request the CO or authorized representative, to initial the entry on the checklist.
5.6.3 Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO and not the COR.
5.6.4 The services to be performed by the contractor during the period of this contract shall at all times and places are subject to review by the CO and or authorized
COR.
5.6.5 Government personnel, other than CO and COR, may from time-to-time, with
CO coordination, observe contractor operations. However, these personnel may not interfere with contractor performance. During the period that the contractor is working on this contract, trained personnel from the 309th CMXG, will oversee the work and will make the final acceptance.
6. Contractor Requirements
6.1 Communication
6.1.1 The contractor shall keep government management informed of problems and risks that could impact the mission or organization as discussed in the weekly meetings or as needed. Cooperates and coordinates with others within the assigned organization, and functions well as a member of a project or government product team.
