SUBJECT: Request for Past Performance Evaluation – FA7014-08-R-0023 1.

You have been identified as a point of contact for a contract that is considered relevant to the requirements of an Air Force solicitation for Professional Defense Research, Science, & Engineering Technical Advisory & Assistance Support (A&AS). Past performance evaluation of the firm listed on the attached questionnaire is hereby requested. This firm is currently being considered for a contract with Air Force District of Washington Acquisition Division, Bolling AFB DC.

We request that you complete the attached Performance Questionnaire. The information that you provide regarding the contractor’s performance is considered sensitive and cannot be disclosed to others.

Please submit past performance information regarding subject contractor by completing the attached questionnaire and returning it either via E-mail to marion.faulhaber@afncr.af.mil and michael.nannery@afncr.af.mil or facsimile to (202) 767-8107 by 17 April 2008.
