SECTION SF 30 - CONTINUATION SHEET
A. Change Block 8 of the SF 1449, Solicitation, Contract, Order for Commercial Items from 2:00 PM on 1 May 08 to Read 2:00 PM ON 8 MAY 08.
B. The following have been added by reference:

C. Incorporate Addendum to FAR 52.212-4 as follows:
(u) KEY PERSONNEL (Amendment 0002)
The contractor agrees to assign to the contract tasks all personnel whose resumes were submitted with its proposal for each task order. When requesting a task order proposal, the Government will identify positions that are necessary to fulfill the requirements of the contract as "key personnel". No substitutions may be made except in accordance with the following.
The contractor understands that during the first ninety (90) days of the contract performance period, no personnel substitutions will be permitted unless these substitutions are unavoidable because of the incumbent's sudden illness, death or termination of employment. In any of these events, the contractor shall promptly notify the contracting office and provide the information described in the paragraph below. After the initial ninety (90) day period the contractor must submit to the Contracting Officer all proposed substitutions, in writing, at least fifteen (15) days in advance of any proposed substitution and provide the information required in the paragraph below.
Any request for substitution must include a detailed explanation of the circumstances necessitating the proposed substitution, a resume for the proposed substitute, and any other information requested by the Contracting Officer. Any proposed substitute must have qualifications equal to or superior to the qualifications of the incumbent. The Contracting Officer or the QAP will evaluate such requests and promptly notify the contractor of his/her approval or disapproval thereof.
In the event that any of the identified key personnel cease to perform under the contract and the substitute is disapproved, the task order contract may be immediately terminated in accordance with the Termination clause of the contract.
(v) SECURITY REQUIREMENTS (Amendment 0002):
Specific security requirements in accordance with DD Form 254 will be provided as part of the Request for Quotation (RFQ) for each task order requirement.
D. Change Clause 5352.201-9101 OMBUDSMAN (AUG 2005), paragraph (c) to read as follows:
(c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM ombudsmen, Ms. Deborah Doyle, AFDW/A7K, Phone Number (240) 257-4729). Concerns, issues, disagreements, and recommendations that cannot be resolved at the MAJCOM/DRU level, may be brought by the concerned party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (703) 588-7004, facsimile number (703) 588-1067.
E. Add The following note to NOTICE TO OFFERORS
5. Resultant Award(s) shall be Indefinte Delivery/Indefinite Quantity contract(s) funded by Task Orders issued for requirements as they arise. ***
F: Incorporate the following statement into CLINS 0001, 1001, 2001, 3001, and 4001:
“Payment for FFP CLINS is based on monthly hours expended not the price of the Task Order.”
C. Incorporate the following change to Addendum to FAR 52.212-1, Instructions to Offerors,
(a) Revise Paragraph C(4) to read as follows:
(4) Proposals shall be submitted on 8 ½ x 11-inch paper, double-spaced in three (3) sections. Except for reproduced sections of the solicitation document, text shall be no smaller than 12 point (except for Tables and Graphs (Amenment 0002). Margins on all four sides of each sheet will be at least one inch. All pages shall be printed on one side only. Special consideration will not be given for colors, pictures or unnecessary graphics. Audio and Video recordings, or any other electronic media, to include responses to this solicitation (e.g. CD, tape, electronic transmission) will not be accepted. E-MAIL, GSA E-BUY OR ANY ELECTRONIC RESPONSE WILL NOT BE CONSIDERED FOR AWARD.
(b) Revise Table I to read as follows:

Mission Capability
(Original plus three copies)
a. Maximum of 25 pages to address the basic contract IDIQ requirements for Mission Capability :
1. Technical Qualifications (shall address 52.212-2 paragraph 1.2.1.1) and Management Approach (shall address 52.212-2 paragraph 1.2.1.2); (NO RESUMES) (Amendment 0002) 2. Provide detailed information on meeting security requirements as listed in DD 254 (Attachment 2) – See FAR Clause 52.212-2, paragraph 1.2.1.1.5 for details;
3. Provide your Small Business Subcontracting Plan that demonstrates sound and effective methods to determine subcontracting goals, encourage small business participation, and implement policies that meet the requirements of FAR Part 19 and its supplements. Small Businesses will address how they will manage compliance with the Limitation on Subcontract clause (excluded from the page count limitations); and
b. This Mission Capability Volume is due by no later than 2:00 PM Eastern Daylight Time (EDT) on 8 May 2008. (AMENDMENT 0002)

a. Each offeror shall submit a past performance proposal, containing past performance information on relevant and related references of the same or similar work in terms of size and scope within the past three (3) years with at least 6 months of performance;
b. Offerors shall distribute the Past Performance Questionnaire (Attachment 3) to the reference POCs identified in the Past Performance Section. The reference POCs shall complete and deliver the questionnaire (Attachment 3) to the Contracting Officer identified on the questionnaire;
c. Provide the information requested in Attachment 4 [one (1) page per reference; exempt from double-spaced 12-point requirement (Amendment 0002)] for all performance references. The contractor shall provide information on references and subcontractor consent letters for this part of the Past Performance submission; and
d. This Past Performance Volume is due by no later than 2:00 PM Eastern Daylight Time (EDT) on 8 May 2008. (AMENDMENT 0002)
OFFERORS ARE CAUTIONED THAT THE GOVERNMENT WILL USE DATA PROVIDED BY EACH OFFEROR IN THIS SECTION AND DATA OBTAINED FROM OTHER SOURCES IN THE EVALUATION OF PAST PERFORMANCE.
G. Incorporate the following change to 52.212-2, EVALUATION – COMMERCIAL ITEMS, Paragraph 1.2.1.2.6, Ensure that the Small Business Subcontracting Plan demonstrates sound and effective methods to determine subcontracting goals, encourage small business participation, and implement policies that meet the requirements of FAR Part 19 and its supplements, to include elements listed in FAR 52.219-9, Alternate II, Small Business Subcontracting Plan, (NOT APPLICABLE TO SMALL BUSINESS CONCERNS). Ensure that Small Businesses are in compliance with the Limitation on Subcontract clause. (Amendment 0002)
H. Change the Task Order Procedures
1. Change Paragraph 1.2.2. to read as follows:
“The government intends to award each task order without discussions with respective offerors. Therefore, the offeror’s initial task addendum should contain the offeror’s best terms from a cost or price (at or below the rates provided in the ordering table) (Amendment 0002) and technical standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. If the Government holds discussions, the offeror will be given an opportunity to clarify certain aspects of their proposals (e.g., the relevance of an offeror’s past performance information and adverse past performance information to which the offeror has not previously had an opportunity to respond) or to resolve minor clerical errors.”

c. Provide detailed information on meeting security requirements as listed in DD 254 ; and
d. Provide your Small Business Subcontracting Plan that demonstrates sound and effective methods to determine subcontracting goals, encourage small business participation, and implement policies that meet the requirements of FAR Part 19 and its supplements. Small Businesses will address how they will manage compliance. (Amendment 0002) (excluded from the page count limitations).
3. Remove Paragraph 1.8, entitled OMBUDSMAN, from Task Order Procedures.
I. Delete the following statement from Attachment 1, Performance Work Statement, Paragraph 1.3.1.
“Providing at least one reference letter per candidate. The letter shall be signed by the candidate’s former supervisor addressing work performance.”
J. Change the title of Attachment 7 from “Key Personnel Resume” to read “Task Order Personnel Resume”
K. Incorporate the following changes to Attachment 9, entitled, IDIQ, Total Evaluated Price (TEP):
1. Add the following verbiage:
“FOR EVALUATION PURPOSES ONLY, NOT PART OF RESULTANT CONTRACT”
2. Remove the word Secret at the bottom of Attachment 9
