APPENDIX G ENVIRONMENTAL TESTING REPORT
January 19, 2018

RE: Asbestos and Lead-Based Paint Testing Results Fairchild Hall Mechanical Rooms United States Air Force Academy Colorado Springs, Colorado 80840 Empirical Project Number 351801
Dear Mr. Merola:
The purpose of this report is to present the results of the Limited Asbestos Containing Material (ACM) Testing and Lead-Based Paint (LBP) Testing conducted by Empirical Environmental, LLC (Empirical) on January 8th and 9th, 2018 in Mechanical Rooms 1B43, 1E9, and 1H15, at Fairchild Hall at the United States Air Force Academy (USAFA) in Colorado Springs, Colorado (subject site). This testing was performed prior to renovation activities. Renovations included, but not limited to, replacement of air handlers and associated piping.
1.0 LIMITED ASBESTOS CONTAINING MATERIAL TESTING
The limited ACM testing included a visual inspection of suspected ACM at the renovation areas, bulk sample collection of suspect ACM, and sample analysis of collected bulk samples. This testing did not include materials that were not going to be impacted during renovation activities such as exterior materials.
A visual survey and select bulk sample collection for suspected ACMs were performed by Ms.
Sheila Sealander, Colorado State Certified Asbestos Building Inspector (Certification Number 8737). A copy of Ms. Sealander’s certification is presented in Appendix A. Additionally, a copy of Empirical’s Asbestos Firm certification is presented in Appendix A. A visual survey and limited sampling for suspect materials were completed on December 1, 2017. Sample nomenclature is as follows: XXX-YY-ZZ where XXX indicates the Mechanical Room; YY indicates the pipe or

Empirical Environmental, LLC 2 associated air handler component where the sample was located; and ZZ indicates the consecutive sample number.
Bulk samples were collected from select materials in the Mechanical Rooms that were reportedly going to be impacted during renovation activities. These samples were submitted to EMSL Analytical, Inc. (EMSL) in Denver, Colorado for analysis of asbestos utilizing Polarized Light Microscopy (PLM). EMSL is an accredited laboratory for the analysis of bulk asbestos samples and is a member of the National Voluntary Laboratory Accreditation Program (NVLAP #200828-0).
EMSL’s NVLAP certification is presented in Appendix A. The laboratory results are presented in Appendix B. This report is based on the bulk sample analysis results. Base site diagrams were supplied by Schendt Engineering Corporation (SEC).
Suspect materials are considered positive for asbestos if the materials contain greater than 1% asbestos. As required by the U.S. Environmental Protection Agency’s (EPA’s) National Emissions Standards for Hazardous Air Pollutants (NESHAP) regulations, point counting analysis should be performed if the analyst detects asbestos in the sample and estimates the amount by visual estimation to be less than 10%, or the owner or operator of the building may either assume the amount to be greater than 1% and treat the material as ACM or require the verification of the amount by point counting.
Mechanical Room 1B43
Ten asbestos samples were collected in Mechanical Room 1B43; five (5) from pipe insulation and five (5) from components on the air handlers or the associated components. Several pipes were labeled as being “Asbestos Free” and were not tested in this testing event. Five (5) pipe insulation and/or wrap were collected in 1B43 at the following locations: sample number 1B43-EGR-01 was collect at the return pipe at the elbow by S31; sample number 1B43-EGS-02 was collected from the supply pipe at the “T” above the elbow by S31;
sample number 1B43-EGR-03 was collected on the return pipe at the jacket at P-B; sample number 1B43-EGS-04 was pipe sealant from the top pipe into S-28; and sample number 1B43- EGR-05 was collected from the bottom pipe at S-28. These samples were reported as not containing asbestos. It should be noted the high pipes on the east side of the room were not sampled since the USAFA representative and the client were not sure the pipes would be removed. If these pipes are slated from removal at a future date, and if they are not labeled as “Asbestos Free”, these pipes should be sampled prior to removal.
Five (5) ACM samples were collected from the air handlers and associated components. The insulation on the air handlers appeared to be fiberglass. Two (2) samples were collected from the insulation covering on the air handlers; sample number 1B43-S28-06 was collected from the

Empirical Environmental, LLC 3 south side of S28 between the doors, and sample number 1B43-S31-08 was collected from the north side of S31. These samples were reported as not containing asbestos. One (1) sample was collected from the flex connector on S31 (sample number 1B43-FCS31-07). This sample was reported as not containing asbestos. One (1) sample was collected from the gray sealant on E31 between the fan and plenum (sample number 1B43-E31-09). These sealant was reported as not containing asbestos. One (1) sample of insulation paper was collected from inside the exhaust fan at E31 (sample number 1B43-E31-10) and was reported as not containing asbestos.
The laboratory report and Chain-of-Custody (COC) are presented in Appendix B. Site diagrams depicting the approximate sample locations are presented in Appendix C, Figures.
Mechanical Room 1E9
Twenty-six ACM samples were collected in Mechanical Room 1E9. Sixteen of these were collected from the insulation and/wrap on the pipes and associated tanks. Several pipes in 1E9 were labeled as “Asbestos Free” and were not sampled in this testing event. One (1) sample was collected from the insulation on a tank labeled CV8 (sample number 1E9-CV8-23). This insulation and wrap were reported as not containing asbestos. Fifteen samples were collected from the pipe wrap and/or insulation at the following locations: sample number 1E9-EGS-11 was collected from the supply pipe between S-19 and S-20; sample number 1E9-DW-12 was collected from the drinking water pipe by S-19; 1E9-EGS-13 was collected from the supply pipe south of S-19;
sample number 1E9-EGR-14 was collected from the return pipe south of E-19; sample number 1E9-MTHW-15 was collected from the return pipe east of P-69 (according to Mr. Tim Harvey of SEC, this pipe was labeled “Supply”); sample number 1E9-MTHWS-16 was collected from the supply pipe east of P-69; sample number 1E9-MTWS-17 was collected from the supply pipe north of UA-59; sample number 1E9-MTWR-18 was collected from the return vertical pipe from UA-59 by P-69; sample number 1E9-EGS-19 was collected from the supply pipe north of S-12; sample number 1E9-EGR-20 was collected from the return pipe north of S-12; sample number 1E9- CEGS-21 was collected from the chilled supply pipe south of the Chiller; sample number 1E9- CEGR-22 was collected from the chilled return pipe south of the Chiller; sample number 1E9- MTPGS was collected from the supply line off of tank CV8A; sample number 1E9-MTWS-25 was collected from the supply pipe to UH-58; and sample number 1E9-MTWR-26 was collected from the return pipe at UH-58. These samples, with the exception of sample number 1E9-EGS-11, were reported as not containing asbestos. Sample number 1E9-EGS-11 was reported with 2% Chrysotile asbestos in the white sealant over the pipe insulation wrap. A site diagram depicting the approximate sample locations is presented as Figure 3 in Appendix C. Photographs of ACM are presented in Appendix D.
Ten samples were collected from the air handlers and associated components. Three (3) samples were collected from the insulation and covering on the air handlers at the following locations: sample number 1E9-S20-29 was collected from the south side on S-20 on the west

Empirical Environmental, LLC 4 end; sample number 1e9-S17-31 was collected from the north side of S-17 on the east end; and sample number 1E9-S17-32 was collected on the south side of S-17 on the east end. These samples were reported as not containing asbestos. Three (3) samples were collected from the insulation and associated covering; sample number 1E9-S19-27 was collected from the large duct at S-19 (this sample also included a brown putty); sample number 1E9-S18-33 was collected from the west side of the duct at S-18; and sample number 1E9-S11-36 was collected from the south side of the north duct at S-11. These samples were reported as not containing asbestos. Gray duct sealant was observed on the seams of the ducting. Two (2) samples were collected from gray duct sealant; sample number 1E9-S19-28 was collected by the flex connector at S-19 (this sample also contained some white sealant), and sample number 1E9-E18-30 was collected from the north side at E-18. These samples were reported as not containing asbestos. Flex connectors were observed at the air handlers. Two (2) samples were collected. Sample number 1E9-E11- 34 was collected from the east side at the exhaust on E-11, and sample number 1E9-E10-35 was collected from the flex connector on the vertical duct at E-10. These samples were reported as not containing asbestos. EMSL laboratory results and Chain of Custody are presented in Appendix B. Figure 4 in Appendix C depicts the approximate sample locations for the air handlers and associated components.
Mechanical Room 1H15
Twenty ACM samples were collected in Mechanical Room 1H15. One (1) of these samples was collected from tank insulations and wrap at CV5, sample number 1H15-CV5-46. This sample was reported as not containing asbestos. Eleven samples were collected from the pipe insulation and/or wrap. As in the other Mechanical Rooms several pipes were labeled “Asbestos Free” and were not included in this testing event. The pipe insulation and/or wrap samples were collected from the following locations: sample number 1H15-MTHW-47 was collected at UH-49; sample number 1H15-P60-48 was collected from the discharge pipe at Pump 60; sample number 1H15- P60-49 was collected from the suction side at Pump 60; sample number 1H15-EGR-50 was collected from the horizontal return pipe at S-4; sample number 1H15-EGS-51 was collected form the vertical supply pipe at S-4; sample number 1H15-ABDR-52 was collected from an abandoned return pipe north of S-4; sample number 1H15-ABDS-53 was collected from an abandoned supply pipe north of S-4; sample number 1H15-MTWS-54 was collected from the supply pipe to UH-50 above the back flow prevention or blue area; sample number1H15-MTWR-55 was collected from the return pipe at UH-50 above the back flow prevention area; sample number 1H15-DW-56 was collected from the drinking water line by S-5; and sample number 1H15-P1-57 was collected from the east side of the pipe from Pump 1. These samples were reported as not containing asbestos.
Figure 5 in Appendix C depicts the approximate location of the samples.
Nine (9) samples were collected from the air handlers and associated components that were to be impacted during renovations. One (1) sample was collected from the east wall plenum, south

Empirical Environmental, LLC 5 of S-7, sample number 1H15-S7P-37. This sample was reported as not containing asbestos.
Two (2) samples were collected from the insulation and covering on the air handlers. These samples were collected from the south side of S-6 (sample number 1H15-S6-38) and from the north side of S-4 above the west door (sample number 1H15-S4-44. These samples were reported as not containing asbestos. Two (2) samples of gray duct sealant were collected, sample numbers 1H15-S5-39 and 1H15-E4-43. These samples were collected from the west side of S- 5 at the duct and from the west side of E-4 at the duct, respectively. These samples were reported as not containing asbestos. Gray putty was observed on the east side of the duct connection at E-4. This putty was sampled, sample number 1H15-E4-42, and was reported as not containing asbestos. Two (2) samples were collected from duct insulation and covering. Sample number 1H15-S5-40 was collected from the west duct at S-5 and sample number 1H15-S4-45 was collected from the west side of the round duct at S-4. These samples were reported as not containing asbestos. One (1) sample of flex connector was collected from the east side on E-6, sample number 1H15-E6-41. This sample was reported as not containing asbestos. Sample locations are depicted on Figure 6 in Appendix C. EMSL laboratory report and COC is presented in Appendix B.
2.0 LIMITED LEAD-BASED PAINT TESTING
This limited lead-based paint (LBP) testing was performed on January 9, 2018 by Ms. Sheila D.
Sealander, (Colorado Lead Risk Assessor Certification number 8950). This testing was performed prior to renovation activities at the subject site. This testing was not intended to be a LBP Inspection or Risk Assessment as defined by the U.S. Department of Housing and Urban Development (HUD).
Testing was performed using a Niton XLi-702a Series Lead Analyzer (Serial Number 14719) that utilizes X-Ray Fluorescence (XRF) technology. The XRF’s testing mode was set at K & L Spectra Mode during this testing and the assay (source) date was February 1, 2015. This type of sampling is non-destructive. Empirical calibrated the XRF according to manufacturer’s directions prior to and at the end of the testing. Substrate correction is not recommended for this XRF instrument.
The Niton XRF does not have inconclusive readings or ranges, therefore paint chip sampling is not necessary.
Fifty-eight XRF readings were taken during this testing event: seven (7) were for calibration purposes; two (2) were null or incomplete readings; and 49 were at select locations at the subject site.
Twenty XRF readings were taken in Mechanical Room 1B43. Eighteen of these XRF readings were below the EPA’s standard for lead-based paint of 1.0 milligram per square centimeter (mg/cm2) and would not be considered LBP. Two (2) XRF readings were above 1.0 mg/cm2 and

Empirical Environmental, LLC 6 would be considered LBP. These readings were on the dark red belt guard at S30 and the beige fan housing on S30. Like paints on like surfaces should be presumed to be LBP if they were not tested. XRF results are presented in Appendix E. Figure 7 in Appendix C depicts the approximate locations of XRF readings.
Sixteen XRF readings were taken in Mechanical Room 1E9. Eleven of these XRF readings were below the EPA’s standard for lead-based paint of 1.0 mg/cm2 and would not be considered LBP.
Five (5) XRF readings were above 1.0 mg/cm2 and would be considered LBP. These readings were located on the beige plenum door at S-19, the beige fan housing at S-20, the beige door on S-17, and the orange belt guards at E-17 and S-10. Like paints on like surfaces should be presumed to be LBP if they were not tested. The approximate XRF reading locations are presented in Figure 8 in Appendix C. The XRF results are presented in Appendix E.
Thirteen XRF readings were taken in Mechanical Room 1H15. Twelve of these XRF readings were below the EPA’s standard for lead-based paint of 1.0 mg/cm2 and would not be considered LBP. Two (2) XRF readings were above 1.0 mg/cm2 and would be considered LBP. These readings were on located on the orange belt guard at E-6 and on the red P-60 pump. Like paints on like surfaces should be presumed to be LBP if they were not tested. The approximate XRF reading locations are presented in Figure 9 in Appendix C. The XRF results are presented in Appendix E.
Disturbance of lead-based paint or lead-containing paint will need to comply with the Occupational Safety and Health Administration’s (OSHA’S) Lead Standard, 1926.62.
The XRF readings and locations are enclosed in Table 1, Appendix D. The Lead Risk Assessor’s certification and Empirical’s Firm certification are included in Appendix A.
3.0 CONCLUSIONS AND RECOMMENDATIONS
Upon completion of the limited ACM testing and review of the sample results, asbestos containing materials were reported in the sample collected from the insulation pipe wrap on the supply pipe Mechanical Room 1E9 between S-19 and E-20.
Lead-based paint was detected on the belt guards in the Mechanical Rooms, the fan housing at S-30 in 1B43, the plenum door at S-19 in 1E9, the fan housing at S-20 in 1E9, the door at S-17 in 1E9, and the pump P-60 in 1H15.

4.0 LIMITATIONS
Empirical has performed limited ACM and LBP testing on the materials that were reportedly going to be impacted during renovation activities in Mechanical Rooms 1B43, 1E9, and 1H15 at Fairchild Hall at the USAFA in Colorado Springs, Colorado. Empirical used analytical results to assess whether ACMs were present in the samples and XRF results to determine if the paint was LBP. This limited testing is not a guarantee that ACMs or LBP are limited to those areas where identified in this report. This sampling event was limited to those areas that were visible or visibility was easily attained on the days of sampling. Other sampling data at a different time or location may provide different results. The limitations of this limited testing should be recognized as conclusions are formulated on the environmental risks associated with this property. Empirical does not warrant the work of regulatory agencies or other third parties supplying information which may have been used during the preparation of this report.
This limited ACM and LBP testing report was prepared for the exclusive use of SEC for specific application to the subject property. No warranties, either expressed or implied, are made or intended. The material contained herein shall not, in whole or in part, be disseminated or conveyed to any other party or be used or relied upon by any other party without Empirical’s written consent. In the event that any changes in the nature or location of suspected ACM or LBP as outlined in this report are observed, the conclusion and recommendations contained in this report shall not be valid unless these changes are reviewed and the opinions of this report are modified or verified in writing by Empirical.

Dear Mr. Merola:
The purpose of this addendum is to present the results of the additional Asbestos Containing Material (ACM) Testing conducted by Empirical Environmental, LLC (Empirical) on February 20th, 2018 in the Mechanical Room 1E9 at Fairchild Hall at the United States Air Force Academy (USAFA) in Colorado Springs, Colorado (subject site). This additional testing was performed after one (1) asbestos sample was reported as being above 1% during the initial testing phase in January (see Empirical report dated January 19, 2018).
1.0 LIMITED ASBESTOS CONTAINING MATERIAL TESTING
This additional ACM testing included a visual inspection and sample collection of suspected ACM at the hanger areas on and around the EGR and EGS pipes. This sampling was performed by Ms. Sheila Sealander, Colorado State Certified Asbestos Building Inspector (Certification Number 8737). A copy of Ms. Sealander’s certification is presented in Appendix A. Additionally, a copy of Empirical’s Asbestos Firm certification is presented in Appendix A. Sample nomenclature is as follows: XXX-YY-ZZ where XXX indicates the Mechanical Room; YY indicates the consecutive sample number; and ZZ indicates which pipe where the sample was collected.

The samples were submitted to EMSL Analytical, Inc. (EMSL) in Denver, Colorado for analysis of asbestos utilizing Polarized Light Microscopy (PLM). EMSL is an accredited laboratory for the analysis of bulk asbestos samples and is a member of the National Voluntary Laboratory Accreditation Program (NVLAP #200828-0). EMSL’s NVLAP certification is presented in Appendix A. The laboratory results are presented in Appendix B. This addendum is based on the bulk sample analysis results. A site diagram depicting the approximate sample locations is presented in Appendix C.
Suspect materials are considered positive for asbestos if the materials contain greater than 1% asbestos. As required by the U.S. Environmental Protection Agency’s (EPA’s) National Emissions Standards for Hazardous Air Pollutants (NESHAP) regulations, point counting analysis should be performed if the analyst detects asbestos in the sample and estimates the amount by visual estimation to be less than 10%, or the owner or operator of the building may either assume the amount to be greater than 1% and treat the material as ACM or require the verification of the amount by point counting.
During the first testing event sixteen samples were collected from the piping in Mechanical Room 1E9. One (1) sample was reported as containing asbestos. This sample was collected at a pipe hanger. In an effort to delineate where the ACM was located, thirteen additional ACM samples were collected during this sampling event at hanger locations. Seven (7) of these samples (sample numbers 1E9-1-EGS, 1E9-2-EGS, 1E9-3-EGS, 1E9-4-EGS, 1E9-5-EGR, 1E9-6-EGR, 1E9-7-EGS, and 1E9-9-EGR) were reported to contain <1% to 3% Chrysotile asbestos in the mastic on the pipe insulation. These samples were located the EGS and EGR pipes in the north end of the mechanical room. One (1) sample in the approximate location of the ACM was reported as not containing asbestos, sample number 1E9-8-EGR. Four (4) samples were collected at pipe hanger locations in the south end of the mechanical room, sample number 1E9-10-EGS, 1E9-11- EGR, 1E9-12-EGR, and 1E9-13-EGS. These samples were reported as not containing asbestos.
2.0 CONCLUSIONS AND RECOMMENDATIONS
Upon completion of the additional ACM testing and review of the sample results, asbestos containing materials were reported in the samples collected from the mastic on the pipe insulation at the pipe hangers between S-19 and south of S-20. A site diagram is presented in Appendix C.
It is recommended the piping at the hangers be abated by a Colorado Asbestos Abatement Contractor if these areas will be disturbed during renovations.
3.0 LIMITATIONS
Empirical has performed additional ACM testing at hanger locations on the EGS and EGR pipes in Mechanical Room 1E9 at Fairchild Hall at the USAFA in Colorado Springs, Colorado. Empirical

Empirical Environmental, LLC 3 used analytical results to assess whether ACMs were present. This limited testing is not a guarantee that ACMs are limited to those areas where identified in this addendum. Empirical does not warrant the work of regulatory agencies or other third parties supplying information which may have been used during the preparation of this report.
This limited ACM testing addendum was prepared for the exclusive use of SEC for specific application to the subject property. No warranties, either expressed or implied, are made or intended.
