FOR CONSTRUCTION

SUSTAIN/REPAIR HVAC UNITS 10EA – ARNOLD HALL

Revision 0: 29 March 2017, Initial Release

CENMP PM: David Van Dale Phone: (719) 333-0897

CENMP CM: Jeth Fogg Phone: (719) 333-8397

1. PROJECT INFORMATION:

A. PROJECT NUMBER: XQPZ 12-0114

B. PROJECT TITLE: Sustain/Repair HVAC Units 10EA – Arnold Hall

C. PROJECT LOCATION: 2302 Arnold Hall, USAF Academy, Colorado

2. DESCRIPTION OF PROJECT:

A. PROJECT BACKGROUND: Most of the HVAC systems in 2302 Arnold Hall are original to the building and in operation since approximately 1960. The HVAC consists mainly of air handling units and reheat coils serving spaces. The building is served by the Central Heat Plant which provides high temp hot water of up to 400 degrees. The chilled water is generated at the Harmon/Arnold Hall chiller plant. This project will upgrade HVAC equipment in the north mechanical room consisting mainly of 5 air handling units and associated equipment.

Controls will need to be upgraded on 3 other air handling units and reheat coils.

Most piping and ductwork insulation is original and some contains asbestos.

B. CONSTRUCTION SCOPE: The Contractor shall execute the project as shown in this Statement of Work (SOW), additional project drawings and Specifications.

3. GENERAL PROVISIONS:

A. The Contractor shall supply all labor, materials, transportation, equipment, and supervision to provide a complete, finished, functional product meeting USAFA Specifications.

B. The Contractor must comply with specification sections 01 35 00 USAFA Environmental Standards and 01 35 10 USAFA Erosion Control Revegetation and Tree Care Standards which outlines provisions to help ensure Contractor compliance with environmental law. Include a Spill Prevention and Response Plan with every project.

C. Include in the proposal a Period of Performance (POP) duration including on-site construction time.

D. Pre-Construction Site Survey: Prior to the start of any on-site construction activities, the Contractor shall conduct a condition survey with the 10 CES Construction Inspector, after which the Contractor shall note the condition of trees, shrubs, grassed areas, concrete, and any structural damage that may exist immediately within or adjacent to work site.

E. The work area will be confined to the applicable areas as shown in the design drawings and listed in this SOW. All work must conform to the current provisions of the USAFA Architectural and Engineering Services Guide available on CD from the Project Manager.

F. Submittal Register: A submittal register is provided for this project, and is a supplemental document to the specifications. Provide submittals and maintain the Submittal Register throughout project construction period of performance.

Contractor shall submit the Master Submittal Register to Contracting within 14 days after the Notice to Proceed (NTP).

G. Any brand name items listed in this SOW and drawings are intended as a Basis of Design (BOD) only. These brand names were selected because they address the specific issues that are unique to this project (i.e. complying with USAFA Design Stds, SHPO Review, User requirements, maintenance considerations, Environmental compliance, etc). Because of these unique conditions and requirements, the brand name products have been researched and shown to have a proven record of working when the manufacturer’s recommendations and requirements have been met.

The Contractor may substitute a product, or products, for the brand name items as long as the substituted product(s) meet the minimum performance standards established by the brand name product(s).

The Contractor is responsible for how the substituted product(s) affect the design and schedule of the project. 10 CES will review the substituted products for General Conformance only. All substituted products will be reviewed for compliance with the USAFA Design Standards, any SHPO or historical considerations, Environmental regulation compliance and any other review processes that are applicable to this project.

As a part of the 10 CES review process for substituted products, and prior to the substituted product being used or installed, the Contractor must demonstrate in writing that the substitution will meet all project requirements listed above.

H. HAZARDOUS MATERIALS. All hazardous materials (HAZMAT) brought on site are subject to pre-approval by the USAFA Hazardous Materials Management Program (HMMP) Team (CE, LG, SE & Bio Env). AFI 32-7086, Hazardous Material Management provides approval requirements for contractor used HAZMAT. USAFA may prohibit the use of any HAZMAT it deems to be especially hazardous to human health or the environment or may result in excessive quantities of hazardous waste. The Prime Contractor will submit an Alternative AF Form 3952, Chemical/Hazardous Material Authorization Request (provided by the HAZMAT Tracking Activity (HTA)) for all HAZMAT required for each Project.

The Prime Contractor will also submit HAZMAT requirements for any subcontractors on the Contract. The Alternative AF Form 3952, must fully describe the Project requiring the use of HAZMATs. The Process Name in EESOH-MIS will include the Project Number and Name of the Project. The approval process requires approximately 10 working days. The Contractor will report authorized HAZMAT usage at any time during the execution of the contract or at least monthly to the HTA (email is the preferred method). The report shall include a listing of the assigned barcode label numbers, and date used or disposed. This report may be emailed (LG_Hazmat@us.af.mil) or hand delivered to the HTA.

I. For all projects that have a Period of Performance that is 90-days or longer in length, the Contractor shall prepare and submit a DRAFT DD Form 1354, Transfer and Acceptance of Military Real Property, for all installed property (by building number if multiple buildings are involved in the project/task order) to the CO. The DRAFT 1354 shall be submitted no later than 30-days after the Notice to Proceed. This DRAFT 1354 will be reviewed by Real Property to resolve any issues and provide clarity to the Contractor for inclusion in the FINAL DD form 1354. Please request a sample document if needed.

4. SPECIFIC REQUIREMENTS:

A. CLIN Schedule:

1. CLIN0001: Provide all work for XQPZ12-0114 Sustain/Repair HVAC Units 10EA – Arnold Hall, described in this SOW, and on the plans and specifications. This work is considered funding code 52200 and 52400.

B. Work Sequencing:

1. All required ACM and lead containing paint abatement shall occur first, before additional demolition or construction is commenced.

2. Demolition and construction of AHUs, minus new CHWS/R connections, shall occur so that only one AHU is down at any time. Sequence shall begin with AHU6 and proceed in numerical order for AHU7 thru AHU10.
