STATEMENT OF WORK FOR THE REMOVAL AND

DISPOSAL OF ASBESTOS CONTAINING MATERIALS

1.

GENERAL

1.1 DESCRIPTION OF WORK. The work consists of the removal and disposal of Asbestos Containing Material (ACM) located in Building 504 at the Youngstown Air Reserve Station. The ACM to be removed is flooring material, pipe insulation, wallboard and wall tile adhesive in this building. The specific location and scope of work is described in Section 2 of this Statement of Work (SOW) and the attached drawing. Air monitoring will be conducted during abatement activities. The contractor shall provide all labor, materials, equipment, insurance, permits and services necessary to complete this work in accordance with all applicable Federal, State and local regulations.

1.2 LOCATION: The site of work is at the Youngstown Air Reserve Station, U.S. Air Force Reserve, located in Trumbull County, at Vienna, Ohio, about 11 miles north of Youngstown, Ohio.

1.3 GOVERNMENT FURNISHED PROPERTY: Property, such as construction equipment, tools, labor, etc., WILL NOT be furnished by the Government.

1.4 INSPECTION AND APPROVAL OF WORK: All work will be subject to the acceptance by the Contracting Officer. Personnel from the Base Civil Engineering Office, Bldg 510, are designated as having the responsibility for the technical inspection of the contract.

1.5 CONTRACTOR'S SUBMITTALS: Itemized below is a list of materials, drawings, certifications, manufacturers literature, data brochures, technical data and samples as noted to be submitted to the Contracting Officer by the Contractor for approval using AF Form 3000. Approval must be obtained from the Contracting Officer prior to starting any work requiring submittals as specified. All submittals except as noted below shall be made no later than 10 days after receipt of the Notice to Proceed, or as specified by the Contracting Officer. The Contracting Officer will return the submittals approved or disapproved within 10 work days after receipt, by AF Form 3000. If submittals cannot be made within 10 days after Notice to Proceed, or as specified by the Contracting Officer, the Contracting Officer shall be advised of the difficulty and an extension of time requested. Approval of the submittals does not relieve the Contractor from performance in strict accordance with the contract specifications and drawings, nor does such approval constitute "acceptance" under other provisions of this contract. Four copies of all submittals, warranties and operating instructions shall be provided unless otherwise specified. Items requiring submittals are as follows:

1.5.1 Work will not commence until all submittal requirements that are necessary prior to asbestos removal have been submitted and approved. These submittals are denoted by a *.

1.6 Not Used

1.7 PROJECT RESTRICTIONS. The Contractor will be permitted to work in Bldg. 504 Monday through Friday, except on Government holidays.

1.7.1 A burning permit will be obtained from the Base Fire Station prior to any welding or cutting.

SECTION 2 - GENERAL REQUIREMENTS

2.

GENERAL.

2.1

SITE REQUIREMENTS.

2.1.1 Site Investigation.

2.1.1.1 The Contractor shall investigate the site and satisfy himself as to: (a) the conditions affecting the work, including but not limited to physical conditions of the site which may bear upon site access, handling and storage of tools and materials, access to water, electric or other utilities which otherwise affect performance of required activities; (b) the character and quantity of all surface and subsurface materials or obstacles to be encountered in so far as this information is reasonably ascertainable from an inspection of the site, including exploratory work done by the Government or a designated representative as well as information presented in drawings and specifications included with this contract. Any failure by the Contractor to acquaint himself with available information will not relieve him from the responsibility for estimating properly the difficulty or cost of successfully performing the work. The Government is not responsible for any conclusions or interpretations made by the Contractor on the basis of the information made available by the Government.

2.1.1.2 It is recommended that the Contractor become thoroughly familiar with the details of the work and working conditions. The contractor is responsible for verifying all dimensions and estimated quantities of ACM identified in this SOW and attached drawing. No extra compensation will be permitted for differences between actual quantities and government estimates indicated in this SOW unless changes in conditions were not identifiable until the work is underway.

2.1.1.3 Abatement procedures and equipment may vary, depending on material composition and site conditions.

2.1.2.1 Discrepancies with regard to conflicts between the Contract Documents and applicable Federal, State, or local regulations or requirements shall be addressed immediately. Federal, state or local regulations shall govern.

2.1.2.2 Insurance requirements - The Contractor shall purchase and maintain insurance that will protect him from claims that may arise out of, or result from his activities under this Contract, whether those activities are performed by himself or a Subcontractor or by anyone directly or indirectly employed by any of them or by anyone for whose acts any of them may be liable. Contractor's insurance carrier must be informed of the nature of work involved with asbestos abatement activity. Contractor will submit proof of insurance.

2.1.2.2.1 Contractor shall submit proof of coverage under the Worker's Compensation insurance system of the State of Ohio or other similar benefit acts.

2.1.3 Licenses and Qualifications.

2.1.3.1 Contractor must be licensed as required by the Ohio Department of Health as an asbestos Hazard Abatement Contractor for the purpose of removal, encapsulation, enclosure, demolition and maintenance of components covered by or composed of asbestos containing materials (Ohio Administration Code, Rule 3701-34-02). Contractor shall submit certification as a licensed Asbestos Hazard Abatement Contractor by the State of Ohio.

2.1.3.2 The Contractor will provide a full-time General Superintendent who is experienced in administration and supervision of abatement projects including work practices, protective measures for building and personnel, disposal procedures, etc. The Superintendent will be accredited by the State of Ohio as an Asbestos Hazard Abatement Specialist. This person is the Contractor’s Representative that is responsible for compliance with applicable federal, state and local regulations. The General Superintendent is to be the Competent Person as required and defined by OSHA in 29 CFR 1926.1101. The Superintendent must be present at all times when abatement activities are being performed. The Superintendent will remain on site until the project is complete. If the Superintendent must leave the site, a substitute must be approved by the Contracting Officer before abatement activities may continue.

2.2

PROTECTION OF GOVERNMENT PROPERTY

2.2.1 The Contractor is responsible to repair or replace any Government property that they damage during this project. This includes, but not limited to suspended ceiling tiles and systems, walls, ceilings and other building components. Any painted surfaces which are damaged will also be repaired by the Contractor to match the existing surface. The repair or replacement of any damaged item must be acceptable to the Contracting Officer.

2.3

SCOPE OF WORK.

2.3.1 This Statement of Work covers the removal of building materials that contain asbestos. The Contractor will furnish all labor, materials, insurance, equipment, and services, in accordance with the requirements of EPA, OSHA and ODOH Standards for the removal of the asbestos containing material. In using the following EPA, OSHA and ODOH Standards, if a contradiction in a particular area exists, the most stringent will apply. Contractor shall also comply with the regulations set by local government and regulations of the waste disposal site used by the Contractor and the governing local regulations of the area where the waste disposal site is located.

2.3.2 The work involves the removal of Asbestos Containing Material (ACM) located in Bldg. 504 as identified in this statement of work and drawing. The Contractor will remove suspended ceiling panels, grids, fiberglass insulation, furniture, lights and other items as needed to gain access to the ACM and reinstall following the abatement. Wall and ceiling penetrations to other areas of the building will be sealed. An Air Filtration Device (AFD) will be used to pull potentially contaminated air out of the work areas. AFD exhaust will be outside the building. Contractor must submit a detailed asbestos abatement plan and a work schedule to the Contracting Officer for approval prior to start of any work. At a minimum, the abatement plan will include the specific removal and encapsulation methods that will be used, type of enclosure, waste collection and disposal methods, number of AFD’s and locations, worker protection methods, and air monitoring requirements for each location included in this project. The method for removing electrical panels and other equipment in Room 100 will be addressed in the plan.

2.3.2.1 Remove all ACM flooring and mastic/glue in Rooms 112 as identified on the attached drawing. Approximately 391 Square Feet (SF) of ACM floor tile and ACM mastic will be removed. The abatement contractor will remove existing carpet to gain access to the floor tile after the area is in containment. Critical barriers will be installed with 6 mil poly sheeting over all openings to other parts of the building. Razor scrape floors to remove as much mastic/glue as possible. An approved solvent may be used to clean the floors but the amount of solvent will be kept to a minimum. Contractor will prevent solvent from entering cracks/seams in the concrete floor and at the base of the walls as much as practical.

2.3.2.2 Remove acoustical tile and mastic from the north, east and west walls in Room 112 as shown on the drawing. The acoustical tile was determined not to contain asbestos but the mastic is ACM. Approximately 600 SF of tile and mastic will be removed.

2.3.2.3 Clean pipe insulation from pipes, floor, wall and ledges between the interior and exterior wall in Rooms 110 and 131 as shown on the drawing. Pipes above these areas were previously covered with asbestos insulation. Pieces of this insulation have fallen into the openings between the walls. It is possible that sections of pipe in these wall openings may still be covered with asbestos insulation. The estimated amount of pipe insulation is approximately 10 linear feet (LF) in each area. Remove interior walls as needed to completely clean the area between the walls. These areas will be sealed off from other parts of the building with 6 mil poly sheeting. The area in Room 110 will be included in the same containment with Room 112. Surfaces will be hepa-vacuumed and wet-wiped. Encapsulant will be sprayed on all surfaces within the opening following the cleaning.

2.3.2.4 Remove all asbestos containing wallboard (transite) from the walls and ceiling in Room 100. Approximately 275 SF of transite will be removed from the ceiling and 553 SF of transite from the walls. The transite on the ceiling is above a non-ACM plaster with wire mesh. The plaster on the ceiling will be removed while the area is in containment and disposed of as asbestos waste. Any loose insulation above the transite will also be removed and disposed of as asbestos waste. Transite will be carefully removed from the walls to minimize damage to the drywall below the transite.

2.3.2.5 Remove two sections of pipe with asbestos insulation between Room 100 and Room 103. Cut pipe in each room beyond where the asbestos insulation is encapsulated. Pull pipe and insulation through the block wall. Bag or wrap pipe and insulation with poly for disposal. Clean rewetabble cloth and encapsulant from the wall in both rooms. Spray encapsulant in the openings on the wall.

2.3.3 Removal of asbestos containing materials shall be performed using wet methods. All asbestos contained and contaminated material shall be properly containerized and processed for disposal in accordance with Section 5 and Section 7.

2.3.4 The contractor will remove all surface mounted fixtures/objects that may interfere with the removal of asbestos containing material and reinstall upon completion of abatement. The contractor will remove and/or protect all equipment within the abatement area prior to the removal of asbestos. The contractor will clean and reinstall items upon the completion of the abatement activity. Remove any containment barriers, decon unit, and engineering controls upon completion and visual acceptance of work.

2.3.4.1 Equipment currently secured to the transite in Room 100 will be removed after the room is in containment to the maximum extent practical. Live electrical panels and other equipment may be removed before the room is in containment. However the Contractor will provide qualified personnel and equipment to immediately cleanup any debris that comes loose when this equipment is removed from the walls.

2.3.4.2 Contractor shall adequately protect electronic equipment in Room 101. The electronic equipment in Room 101 must continue to operate during the asbestos removal in Room 100. Ensure electronic equipment in Room 101 has sufficient ventilation to continue operating.

2.3.5 The Contractor shall provide all necessary personal air monitoring and sampling for accomplishment of this abatement activity in accordance with Section 9. Sampling and analysis shall be accomplished through an independent testing laboratory and air sampling professional approved by the Contracting Officer.

2.4

WORK REQUIREMENTS.

2.4.1 The work specified herein shall be the removal of asbestos containing materials by competent persons trained, knowledgeable and qualified in the techniques of abatement, handling and disposal of asbestos-containing and asbestos contaminated materials and the subsequent cleaning of contaminated areas, who comply with all applicable Federal, State, and Local regulations and are capable of and willing to perform the work of this contract.

2.4.2 The Contractor shall supply all labor, materials, services, insurance, permits and equipment necessary to carry out the work in accordance with all applicable Federal, State, and Local regulations.

2.4.3 Utilities: All reasonable quantities of available utilities, electricity and water at the job site will be provided the contractor without charge. The source of utilities shall be from existing locations and any temporary connections or lines shall be installed, maintained and removed by the contractor at his own expense and in a manner acceptable to the Contracting Officer. Contractor will install backflow devices on water connections.

2.5

APPLICABLE STANDARDS AND GUIDELINES.

2.5.1 General Requirements.

2.5.1.1 All work under this contract shall be done in strict accordance with all applicable Federal, State, and Local regulations, standards and codes governing asbestos abatement.

2.5.1.2 The most recent edition of any relevant regulation, standard, document or code shall be in effect. Where conflict exists among the requirements or with these specifications, the most stringent requirements shall be utilized.

2.5.2 Specific Requirements.

SUBMITTALS AND NOTICES.

2.6.1 Contractor shall:

2.6.1.1 Prior to Commencement of Work:

2.6.1.1.1 Contractor will send written notification and fee in accordance with Ohio Administrative Code at least ten (10) days prior to the commencement of any on-site project activity to the Ohio Department of Health (ODH) and the Ohio Environmental Protection Agency (OEPA). A copy of the notifications and proof of receipt by ODH and OEPA will be submitted to the contracting officer.

2.6.1.1.2 Submit certification that the landfill to be utilized is permitted to accept asbestos contained material and verification of acceptance for asbestos disposal under this contract.

2.6.1.1.3 Submit certification of training to the Contracting Officer that the Contractor's employees, including foremen, supervisors, and any other company personnel or agents who may be exposed to airborne asbestos fibers or who may be responsible for any aspects of abatement activities, have received adequate training. Submit a brief description of the Supervisor’s experience in asbestos abatement.

2.6.1.1.4 Submit documentation that personnel have received medical monitoring as required in OSHA Title 29, CFR l926.1101.

2.6.1.1.5 Submit manufacturer's certification that HEPA vacuums, negative pressure ventilation units and other local exhaust ventilation equipment conform to ANSI standards.

2.6.1.1.6 When rental equipment is to be used in abatement areas or to transport asbestos contaminated waste, a written notification concerning intended use of the rental equipment must be provided to the rental agency with a copy submitted to the Contracting Officer. If no rental equipment will be used, submit a written statement that rental equipment will not be used on this project.

2.6.1.1.7 Submit NIOSH approvals for all respiratory protective devices utilized on site.

2.6.1.1.8 Submit documentation of respirator fit-testing for all Contractor employees and agents who must enter the work area. This fit-testing shall be in accordance with procedures as detailed in Appendix C to CFR l926.1101, Qualitative and Quantitative Fit Testing Procedures - Mandatory.

2.6.1.1.9 Submit respiratory protection program as required in Section 9.

2.6.1.2 During Abatement Activities:

2.6.1.2.1 Maintain and submit a daily job progress report. This will be in a narrative form which details the daily activities. The names of workers and any visitors that entered the regulated area will be included in this report.

2.6.1.2.2 Submit copies of all transport manifests, trip tickets, disposal receipts and waste shipment records for all asbestos waste materials removed from the work area during the abatement process. The manifests will be signed by the contractor, transporter(s), and the disposal facility in accordance with Section 7.

2.6.1.2.3 Submit results of all air sampling data collected during the course of the abatement.

2.7

SITE SECURITY.

2.7.1 The work area is to be restricted only to authorized, trained, and protected personnel. These may include the Contractor's employees, employees of subcontractors, Government employees and representatives, State and local inspectors and any other designated individuals.

2.7.2 Entry into the work area by unauthorized individuals shall be reported immediately to the Contracting Officer or Representative by the Contractor.

2.7.3 Contractor shall have control of site security during abatement operations in order to protect work efforts and equipment.

SECTION 3 - MATERIALS AND EQUIPMENT

3.

MATERIALS AND EQUIPMENT

3.1 Materials.

3.1.1 General.

3.1.1.1 Deliver all materials in the original packages, containers, or bundles bearing the name of the manufacturer and brand name. Store all materials subject to damage off the ground, away from wet or damp surfaces, and under cover sufficient enough to prevent damage or contamination. Replacement materials shall be stored outside of the work area until abatement is complete.

3.1.1.2 Damaged, deteriorating or previously used materials shall not be used and shall be removed from the worksite and disposed of properly.

3.1.1.3 Polyethylene sheeting for floors, temporary enclosures and stationary objects shall be a minimum of 6 mil. Glovebags shall be made of 6 mil thick polyethylene and shall be seamless at the bottom.

3.1.1.4 Method of attachment may include any combination of duct tape or other waterproof tape or other effective procedures capable of sealing polyethylene to dissimilar finished or unfinished surfaces under both wet and dry conditions including the use of amended water. All joints will be sealed with tape or other approved means to prohibit contamination of adjoining areas. Adhesive will be cleaned from building or equipment surfaces.

3.1.1.5 Disposal bags shall be of 6-mil polyethylene, pre- printed with labels as required by EPA Regulation Title 40, CFR 61.152(b)(i)(iv), or OSHA Title 29 CFR 1910.1200(f) of OSHA's Hazard Communication Standard and contain the information per OSHA Title 29, CFR 1926.58(k)(2).

3.1.1.6 Warning labels as required by OSHA Title 29, CFR 1910.1101(k)(7) and applicable DOT regulations.

3.1.1.7 Disposal drums, if required, shall be metal or fiberboard with locking rings. Stick on labels for disposal drum will meet EPA and OSHA regulations.

3.1.2 Removal.

3.1.2.1 Surfactant (wetting agent) shall be a 50/50 mixture of polyoxyethlylene ether and polyoxyethlylene ester, or equal, mixed in a proportion of 1 fluid ounce to 5 gallons of water or as specified by the manufacturer. .

3.1.3 Encapsulation.

3.1.3.1 Contractor shall submit a copy of the Safety Data Sheet (SDS) for the encapsulant, spray adhesive and any other hazardous material to be used on the project.

3.2

EQUIPMENT.

3.2.1 General.

3.2.1.1 Full body disposable protective clothing, including head, body, and foot consisting of material impenetrable by asbestos fibers (Tyvek brand or equivalent) shall be provided to all workers and authorized visitors in sizes adequate to accommodate movement without tearing.

3.2.1.2 Additional safety equipment (e.g., hard hats meeting requirements of ANSI Standard Z89.1-1981; eye protection meeting the requirements of ANSI Standard Z87.1-1979; safety shoes meeting the requirements of ANSI Standard Z41.1-1967; and disposable PVC gloves), as necessary, shall be provided to all workers.

3.2.1.3 Non-skid footwear shall be provided to all abatement workers. Disposable clothing shall be adequately sealed to the footwear to prevent body contamination.

3.2.2 Removal.

3.2.2.1 A sufficient supply of scaffolds, lifts, ladders, lifts and hand tools (e.g., scrapers, wire cutters, brushes, utility knives, wire saws, etc.) shall be provided by the Contractor as needed.

3.2.2.2 Brushes utilized for removing loose asbestos containing material shall have nylon or fiber bristles, not metal.

3.2.2.3 A sufficient supply of HEPA filtered vacuum systems will be provided for cleaning.

3.2.3 Encapsulation.

3.2.3.1 Encapsulants shall be sprayed using airless spray equipment. Nozzle pressure should be adjustable within the 400 to 1500 psi range depending on the encapsulants viscosity and solids content. Tip size shall also be specified based on manufacturer’s recommendations.

3.2.3.2 The nature of the encapsulant may affect the requirements for respiratory protection. Vapors that may be given off during encapsulant application must be taken into account when selecting respirators if types other than air supplied are used.

3.2.4 Enclosure.

3.2.4.1 Hand tools equipped with HEPA filtered local exhaust ventilation shall be utilized during the installation of enclosures and supports if there is any need to disturb asbestos containing materials during this process.

3.3

SUBSTITUTIONS.

3.3.1 Approval Required:

3.3.1.1 The contract is based on the materials, equipment and methods described in the contract documents.

3.3.1.2 The Contracting Officer will consider proposals for substitutions of materials, equipment and methods only when such proposals are accompanied by full and complete technical data and all other information required by the Government to evaluate the proposed substitution.

3.3.1.3 Do not substitute materials, equipment or methods unless such substitution has been specifically approved for this work by the Contracting Officer.

3.3.2 "Or Equal"

3.3.2.1 Where the phrase "equal" or "equal as approved by the Contracting Officer" occurs in the contract document, do not assume that materials, equipment, or methods will be approved by the Contracting Officer unless the item has been specifically approved for this work by the Contracting Officer.

SECTION 4 - EXECUTION

4.

EXECUTION.

4.1

PREPARATION.

4.1.1 Work Areas.

4.1.1.1 Post DANGER signs meeting the specifications of OSHA Title 29, CFR 1926.1101(k)(6) Communications of Hazard, at any location and approaches to a location where airborne concentrations of asbestos may exceed ambient background levels. Signs shall be posted at a distance sufficiently far enough away from the work area to permit an employee to read the sign and take the necessary protective measures to avoid exposure.

4.1.1.2 Provide temporary power and lighting, if required. Insure safe installation of temporary power sources and equipment by compliance with all applicable electrical code requirements and OSHA requirements for temporary electrical systems.

4.1.1.3 The Government will provide water for abatement purposes. Contractor shall provide connections to existing system. The Contractor is required to supply a water heater, as needed, for showers. Contractor will provide and use backflow prevention devices on temporary water connections.

4.1.1.4 Pre-clean all surfaces in the work area using HEPA filtered vacuums and/or wet cleaning methods, as appropriate. Do not use any methods that would raise dust such as dry sweeping or vacuuming with equipment not equipped with HEPA filters. Do not disturb asbestos containing materials during the precleaning phase.

4.1.2 Worker Decontamination.

4.1.2.1 Worker decontamination system will be provided by the contractor. The type and location(s) of the decon will be included in the Contractor’s Abatement Plan.

4.1.3 Commencement of work shall not occur until:

4.1.3.1 Enclosure systems, if required, have been constructed and tested.

4.1.3.2 Negative pressure ventilation systems if required, are functioning adequately.

4.1.3.3 All pre-abatement submissions, notifications, postings and permits have been provided and are satisfactory to the Contracting Officer.

4.1.3.4 All equipment for abatement, cleanup and disposal is available on site.

4.1.3.5 All worker training and certification is completed.

4.1.3.6 The Asbestos Pre-Removal Checklist is completed and submitted to the Contracting Officer's Representative. The attached checklist (Appendix “A”), or approved equal, shall be utilized.

4.1.4 Alternative Procedures:

4.1.4.1 Procedures described in this Statement of Work and the approved asbestos abatement plan are to be utilized at all times.

4.1.4.2 If specified procedures cannot be utilized, a request must be made in writing to the Contracting Officer providing details of the problem encountered and recommended alternatives.

4.1.4.3 Alternative procedures shall provide equivalent or greater protection than procedures that they replace.

4.1.4.4 Any alternative procedure must be approved in writing by the Contracting Officer prior to implementation.

4.2 Not Used

4.3

PERSONNEL PROTECTION REQUIREMENTS.

4.3.1 Contractor will provide the following:

4.3.1.1 Prior to commencement of abatement activities, all personnel who will be required to enter the work area or handle containerized asbestos containing materials must have received adequate training.

4.3.1.2 Special on-site training on equipment and procedures unique to this job site shall be performed as required.

4.3.1.3 Training in emergency response and evacuation procedures shall be provided.

4.3.2 Respiratory Protection.

4.3.2.1 All respiratory protection shall be provided to workers in accordance with the submitted written respiratory protection program, which includes all items in OSHA Title 29, CFR 1910.134(b)(1-11).

4.3.2.2 Workers shall be provided with personally issued, individually identified (marked with waterproof designations) respirators.

4.3.2.3 The use of engineering controls such as negative pressure ventilation units and HEPA vacuums and good work practices such as the wetting of asbestos containing material prior to abatement (when applicable), misting the work area to help fibers settle out, removal in small sections, use of glovebags and proper cleanup and containerization, all help to reduce airborne fiber levels in the work area. It is imperative, however, that adequate air monitoring of fiber levels and a well designed respiratory protection program in accordance with OSHA Title 29, CFR 1910.134 be implemented. Key points of the respirator program include proper selection of respirator type and size, training of personnel in the proper inspection, donning, use, cleaning and maintenance procedures for the respirators selected including their use limitations, and a good fitting and fit testing program to provide proper protection.

4.3.2.4 Fit Testing.

4.3.2.4.1 Workers must perform positive and negative air pressure fit tests each time a respirator is put on, whenever the respirator design so permits. Powered air-purifying respirators shall be tested for adequate flow as specified by the manufacturer.

4.3.2.4.2 Workers shall have been fit tested in accordance with procedures detailed in the OSHA Title 29, CFR 1926.1101, Appendix C, Qualitative and Quantitative Fit Test Protocols, for all respirators to be used on this abatement project.

4.3.2.4.3 Documentation of adequate respirator fit must be provided to the Contracting Officer.

4.3.2.5 No one wearing a beard shall be permitted to don a respirator and enter the work area.

4.3.3 Protective Clothing.

4.3.3.1 Disposable clothing, including head, foot, and full body protection, shall be provided in sufficient quantities and adequate sizes for all workers and authorized visitors.

4.3.3.2 Launderable clothing, if used, shall be provided in sufficient quantities and adequate sizes for all workers and authorized visitors.

4.3.3.3 Hard hats, protective eyewear, gloves, rubber boots and/or other footwear shall be provided as required for workers and authorized visitors. Safety shoes will be required.

SECTION 5 - REMOVAL PROCEDURES

5.

REMOVAL PROCEDURES

5.1 Requirements.

5.1.1 Isolate the work area in accordance with Section 4 and the approved asbestos abatement plan.

5.1.2 Wet all asbestos containing materials with an amended water solution using equipment capable of providing a fine spray mist, in order to reduce airborne fiber concentrations when the material is disturbed. Saturate the material to the substrate, however, do not allow excessive water to accumulate. Keep all removed material wet enough to prevent fiber release. Wetting procedures are not equally effective on all type of asbestos containing materials but still shall be used in all cases.

5.1.3 Containers (6-mil polyethylene bags or drums) shall be sealed when full. Double bagging of waste material will be used. Bags shall not be over filled. They should be securely sealed to prevent accidental opening and leakage by taping in goose-neck fashion. Do not seal bags with wire or cord. Bags will be placed in drums for staging and transportation to the landfill if required by landfill for disposal. Bags shall be decontaminated on exterior surfaces by wet cleaning and HEPA vacuuming before removal or being placed in clean drums sealed with locking ring tops.

5.1.4 Components removed intact will be wrapped in two layers of 6-mil polyethylene sheeting secured with tape for transport to the landfill.

5.1.5 Asbestos containing waste with sharp-edged components (e.g., nails, screws, metal lath, tile) that will tear the polyethylene bags and sheeting shall be placed into drums for disposal.

5.1.6 After completion of all stripping work, surfaces from which asbestos containing materials have been removed shall be wet brushed and sponged or cleaned by some equivalent method to remove all visible residue.

5.1.7 Cleanup shall proceed in accordance with Section 6.

SECTION 6 - CLEANUP PROCEDURES

6.

CLEANUP PROCEDURES.

6.1

6.1.1 Remove any poly sheeting from the walls, floors and ceilings.

6.1.2 Remove all containerized and wrapped waste from the work area on a daily basis. No waste will remain in the building overnight.

6.1.3 Inspect the work area for visible residue. If any accumulation of residue is observed, it will be assumed to be asbestos and the cleaning cycle repeated.

6.1.4 The work area shall be cleaned until it is in compliance with State, local, and Federal requirements with the most stringent criteria applying to the Contractor. Additional cleaning cycles shall be provided, as necessary, at no cost to the Government until these criteria have been met.

6.1.5 A final visual inspection shall be made by the Contracting Officer or representative to verify that no visible contamination remains in the work area. Unsatisfactory conditions may require additional cleaning and air monitoring at the contractor’s expense. (See Section 8-Re-Establishment of the Work Area)

SECTION 7 - DISPOSAL PROCEDURES

7.

DISPOSAL PROCEDURES.

7.1 General Requirements.

7.1.1 As work progresses, sealed and labeled containers of asbestos containing waste shall be removed and transported from the project location, to prevent exceeding available storage capacity on-site.

7.1.2 Disposal must occur at an authorized site in accordance with regulatory requirements of NESHAP and applicable State and local guidelines and regulations.

7.1.3 All dump receipts, trip tickets, transportation manifests or other documentation of disposal shall be delivered to the Contracting Officer's Representative. A recommended record keeping format utilizes a chain-of-custody form which includes the names and addresses of the Contracting Officer or Representative, Contractor, pickup site, and disposal site, the estimated quantity and type of the asbestos waste and the type of containers used. The form shall be signed by the Contractor, Transporter(s) and the Disposal Site Operator as the responsibility for the material changes hands. If a separate transporter is employed, his name, address, telephone number and signature shall also appear on the form.

7.1.4 The contractor shall use the Ohio EPA Regulated Asbestos Material Waste Shipment Record, or approved equal, for the transport of asbestos containing material to the disposal facility. This document shall be utilized for the record of transport and disposal. The shipping document will record all transportation of the waste from the time it leaves the Base. The Contracting Officer shall receive the original copy of these documents, with a signature of the landfill agent, upon completion of disposal and acceptance by the disposal facility.

7.2 Transportation to the Landfill.

7.2.1 Once drums, bags, and wrapped components have been removed from the work area, they shall be loaded into an enclosed truck or trailer for transportation. The truck or trailer will have required markings while the ACM is being loaded.

7.2.2 When moving containers, utilize hand-trucks, carts, and proper lifting techniques to avoid back injuries. Trucks with lift-gates are helpful for raising drums during truck loading.

7.2.3 The enclosed cargo area of the truck, trailer or box shall be free of debris and lined with 6-mil polyethylene sheeting to prevent contamination from leaking or spilled containers. Floor sheeting shall be installed first and extend up the sidewalls. Wall sheeting shall be overlapped and taped into place.

7.2.4 Containers shall be placed on level surfaces in the cargo area and packed tightly together to prevent shifting and tipping. Large structural components shall be secured to prevent shifting and bags placed on top. Do not throw containers into truck cargo area, trailer or box.

7.2.5 Any debris or residue observed on containers or surfaces outside of the work area resulting from cleanup or disposal activities shall be immediately cleaned up using HEPA filtered vacuum equipment and/or wet methods as appropriate.

7.3 Disposal at the Landfill.

7.3.1 Upon reaching the landfill, trucks are to approach the dump location as closely as possible for unloading of the asbestos containing waste.

7.3.1.1 Bags, drums, and components shall be inspected as they are off-loaded at the disposal site. Material in damaged containers shall be repackaged in empty drums or bags as necessary. (Local requirements may not allow the disposal of asbestos waste in drums. Check with appropriate agency and institute appropriate alternative procedures.)

7.3.1.2 Waste containers shall be placed on the ground at the disposal site, not pushed or thrown out of trucks (weight of wet material could rupture containers).

7.3.1.3 Not Used.

7.3.1.4 Following the removal of all containerized waste, the truck cargo area shall be decontaminated using HEPA vacuums and/or wet methods to meet the no visible residue criteria. Polyethylene sheeting shall be removed and discarded along with contaminated cleaning materials and protective clothing, in bags or drums at the disposal site.

SECTION 8 - RE-ESTABLISHMENT OF THE WORK AREA AND SYSTEMS

8.

RE-ESTABLISHMENT OF THE WORK AREA AND SYSTEMS.

8.1

8.1.1 Re-establishment of the work area shall only occur following the cleanup process and after air monitoring has been performed and documented to the satisfaction of Contracting Officer’s Representative.

8.1.2 The Contractor and Contracting Officer or his representative shall visually inspect the work area for any remaining visible residue. Evidence of contamination will necessitate additional cleaning requirements at the contractor’s expense.

8.1.3 Additional air monitoring shall also be performed in accordance with Section 9, at the contractor’s expense if additional cleanup is necessary.

8.1.4 Contractor will repair all areas of damage that occurred as a result of abatement activities and restore the work areas to the condition prior to abatement activity. This includes cleaning or repainting walls, floors, or equipment that have been damaged or oversprayed with encapsulant or adhesive. This also includes damage to the suspended ceiling system including light fixtures, detectors, etc..

SECTION 9 - SUPPORT ACTIVITIES AND PERSONNEL

9. SUPPORT ACTIVITIES AND PERSONNEL.

9.1

TRAINING:

9.1.1 Training shall be provided by the Contractor to all employees or agents who may be required to disturb asbestos containing or asbestos contaminated materials for abatement and auxiliary purposes and to all supervisory personnel who may be involved in planning, execution, or inspection of abatement projects.

9.1.2 Training shall be provided and certified in accordance with the Ohio Department of Health.

9.2

MEDICAL MONITORING.

9.2.1 Medical Monitoring must be provided by the Contractor to any employee or agent that may be exposed to asbestos in excess of background levels during any phase of the abatement project. The purposes of a medical monitoring program, in addition to meeting the requirements of the law, are to document the state of health of workers for workman's compensation and to determine work related disease as well to ensure fitness for duty, particularly ability to wear a respirator. Smokers should be made aware of the synergistic effects of cigarette smoking and asbestos exposure. The medical monitoring program provides the appropriate setting to share this information. Employers should also be aware of the potential cost of this additional risk. Medical monitoring shall include at a minimum, the requirements of OSHA Title 29, CFR 1926.58(m).

9.3 RESPIRATORY PROTECTION PROGRAM: The Contractor’s Industrial Hygienist shall establish in writing and implement a respiratory protection program in accordance with OSHA Title 29 CFR 1926.58 and 1910.134. The Contractor’s respiratory protection program shall include, at a minimum, the following elements:

9.3.1 The company policy, used for the assignment of individual responsibility, accountability, and implementation of the respiratory protection plan.

9.3.2 The standard operating procedures covering the selection and use of respirators. Selection shall be determined by the hazard to which the employee is exposed.

9.3.3 Medical evaluation of each user to verify that the worker may be assigned to an activity where respiratory protection is required.

9.3.4 Training in the proper use and limitations of respirators. Training in putting on and removing respirators.

9.3.5 Respirator fit-testing, i.e., quantitative, qualitative and individual functional fit checks.

9.3.6 Regular cleaning and disinfection of respirators. Routine inspection of respirators during cleaning and after each use when designated for emergency use.

9.3.7 Storage of respirators in convenient, clean, and sanitary locations.

9.3.8 Surveillance of work area conditions and degree of employee exposure as determined through air monitoring.

9.3.9 Regular evaluation of the continued effectiveness of the respiratory protection program.

9.3.10 Recognition and procedures for the resolution of special problems as they affect respirator use ( e.g., no facial hair that comes between the respirator face piece and face or interferes with valve function, prescription eye wear usage, prohibition of wearing contact lenses, etc..

9.4

ASBESTOS PROJECT MANAGER.

9.4.1 The Asbestos Project Manager shall be the Contractor or a designated representative. This individual will serve as the competent person and will be certified as an Asbestos Hazard Abatement Specialist by the State of Ohio. A competent person will be on-site at all times during this project.

9.5 AIR SAMPLING

9.5.1 The Air Sampling Professional (ASP) shall conduct all air sampling for ACM removal. The ASP shall be knowledgeable and experienced about the methods for asbestos air sampling and be able to select representative numbers and locations of samples.

9.5.2 The ASP shall conduct air sampling for asbestos in accordance with the NIOSH Standard Analytical Method for Asbestos in Air, P & CAM 239 and/or Method 7400 and Method 7402.

9.5.3 It is required that the following schedule be utilized for air sampling during the project (in addition to any OSHA compliance monitoring):

9.5.3.1 Background Sampling: None Required

9.5.3.2 Asbestos sampling during the abatement project.

9.5.3.2.1 The following schedule of samples is the minimum required on a daily basis, when asbestos abatement activities are conducted. The size of the work area will impact on the number of samples necessary to adequately monitor the Contractor's activities. Decisions on the number of samples and blanks should be made with the advice of the Air Sampling Professional. The Government may collect air samples, in addition to these, which will be analyzed for quality control purposes.

9.5.4 Post-abatement (clearance) air sampling shall be conducted in Room 100 and Rooms 110/112 after all ACM has been removed and following the cleaning phase of the work, once the no visible residue criteria has been met. A minimum of 3 samples shall be collected to determine post-abatement air concentrations in each regulated area. An adequate volume of air to provide accuracy to 0.01 fibers/cc is required. The work areas will be considered clear when all samples are below 0.01 f/cc.

9.5.5 Air samples collected in accordance with these requirements will be analyzed by Phase Contrast Microscopy (PCM). The analyst must have taken a NIOSH course for sampling and evaluating airborne asbestos dust or an equivalent course and be a satisfactory participant in the AIHA Proficiency Testing Program (PAT), AIHA Asbestos Analyst Registry or approved equivalent. Verification of proficiency and the NIOSH training certificate for the ASP will be submitted to the Contracting Officer prior to start of work.

9.5.5.1 On site analytical capabilities for PCM analysis are required for immediate results of sampling. This provides the Contracting Officer with a timely review of Contractor performance and a more rapid awareness of hazardous exposure conditions which can be corrected. Test results, including sample numbers and descriptions, will be submitted to the Contracting Officer.
