STATEMENT OF WORK

Project No: EVKP 15-4409

Project Title: Repair Typhoon Damaged Doors, CF (204 UN)

Location: Camp Foster, Okinawa, Japan

CE Project Manager: Mrs. Michiyo Matsunaga

1.0 General Scope:

The Contractor shall furnish all tools, equipment, materials, testing, labor and personnel necessary to plan, manage, supervise and construct the project to repair/replace typhoon damaged doors at stairway exit and electrical room entrance at Camp Foster towers 377, 2602 and 2604. The Contractor shall determine the best technical design solution in conformance with current codes and regulations, not specifically stated elsewhere in this project statement of work to provide a complete and useable facility upon completion. The scope of work is further defined in the

Description of Work section.

2.0 Project Schedule and Time Constraints:

2.1 Completion of Work: The Contractor shall be required to complete the entire work, ready for use, not later than 130 calendar days after the date of Notice to Proceed (NTP). The Contractor shall submit a Project Schedule at the pre-design conference and shall resubmit updates as necessary until project completion.

2.2 Construction Schedule: The Contractor shall determine the most efficient work plan to minimize inconvenience to the user.

2.3 Start Work Notification: Before starting on-site work the Contractor shall notify the Contracting Officer and the Project Manager in writing, thirty (30) calendar days prior to on-site work starting. After approval of on-site work, the Contractor shall provide written notice to the Housing Manager and Military Family Housing Occupants affected by the Task Order within a minimum of Fourteen (14) calendar days before starting on-site work.

2.4 Construction Access: The Contractor shall be allowed to work from 0800 to 1630, Monday through Friday, excluding U.S. Federal holidays, and other days specifically designated by the Contracting Officer.

(9) 4th Thursday of November, Thanksgiving Day

2.6 Performance Time: The Contractor shall begin performance within five (5) calendar days after receiving the

Noticed to Proceed (NTP) for construction.

Schedule Shall Be Completed Within

a) Shop Drawing/Material Submittals: 14 calendar days

b) Government Material Review: 10 calendar days

c) Procurement of Materials: 60 calendar days

d) Construction Time: 40 calendar days

e) Unfavorable Weather 6 calendar days

Construction Subtotal 130 calendar days

3.0 Construction Requirements:

3.1 Construction shop drawings shall be provided for the completion of the work identified on this Statement of

Work within the contract price and schedule. All shop drawing plans, material and work shall conform to Unified

Facility Criteria (UFC), National Fire Protection Association (NFPA), National Electric Code (NEC), Japan

Environmental Governing Standards (JEGS), Japanese Industrial Standards (JIS) and all other applicable criteria.

The Government shall not be responsible for any additional costs which could have been avoided through an appropriate contractor’s planning. The shop drawings and materials submittals must be coordinated and approved by

718CES/CENMA. Required Submittals shall be identified on an AF Form 66, Material Submittal Log by the government, submittals shall be transmitted provided an AF Form 3000.

3.2 As-built drawings will be provided to the Contractor for reference in their present form. The Government bears no responsibility for the accuracy of these drawings. Actual field investigation and survey of the facility must be conducted by the Contractor to ascertain existing conditions.

3.3 Construction Project Signs: The Contractor shall provide and maintain construction signs on project site.

Signs shall be installed on project site by the contract start date and shall be removed by the contract completion date.

Location of project signs will be coordinated with the Facility Manager, and Project Manager. Contractor may install company signs at temporary site office locations. Request for company sign will be submitted to Contractor Officer and the Project Manager in writing for approval.

3.4 Protection of properties: The Contractor shall protect, with approved protective materials, the area surrounding their work, to include the site and facility as well as anything remaining within the facility (e.g. equipment and furniture). Any items moved by the Contractor shall be properly stored and protected and shall be returned to the original place by the Contractor. The contractor shall protect adjacent property, buildings and their contents from dust, dirt, or other materials. The contractor shall wet down dry materials, settle debris and prevent blowing dust. The contractor shall maintain work areas in a neat, clean and safe condition. Work areas must, at a minimum, be cleaned daily.

3.5 Documents Required: All documents required under this Contract, including but not limited to all correspondence of any nature whatsoever, any written communications, documents, drawings, material submittals, reports, etc. shall all be written in English or translated in English before being provided to the Government by the

Contractor. Delays encountered due to a translated copy were not submitted shall be at the Contractor’s expense.

3.6 Availability of Latrine: Latrine is not available in the work area. The Contractor shall be responsible for providing latrines for his personnel.

3.7 Daily Reports: Contractor shall provide daily progress reports to the Government. Photos of this project are permissible. Photo or media recording of the project must be by approval and coordinated with 718 CES/CENMA.

3.8 Any work that closes or prohibits or limits facility access shall be coordinated with facility manager through the 718CES/CENMA.

3.9 The Contractor shall provide a weather delay request within 48 hours of inclement weather that caused the work delay.

3.10 Cleanup: The contractor shall collect all trash, debris, refuse, garbage, etc., which the contractor generate and place it in appropriate containers with lids or approved covers daily. These materials must be hauled from the site by appropriate means daily, unless otherwise approved by the contracting officer. Disposal must be outside the limits of

EVKP15-4409 Page 3 of 5 government property. Disposal must be at a municipal solid waste landfill or by other approved methods and must conform to all local, state and federal guidelines, criteria and regulations. The time for construction completion includes final clean-up of the premises.

3.11 Joint job site inspection shall be conducted by the project manager, user, contractor, and 18 CONS/LGCC representatives.

3.12 The Contractor shall provide barriers/signs to keep unauthorized persons from entering working area.

3.13 Warranty: The construction contract requires the Contractor to warrant all workmanship, materials, and equipment for a period of one year from the date of substantial completion or beneficial occupancy, whichever occurs first times.

4.0 Special Requirements:

4.1 Asbestos Containing Materials (ACM) and Lead Based Paint (LBP) survey: Follow the latest JEGS requirements.

a. Available ACM and LBP results on record will be provided to the Contractor for reference only. The

Contractor shall remain responsible for additional testing of existing materials that will be affected by the scope of work.

b. Prior to field sampling, the following must be submitted to 18 AMDS/SGPB (Bioenvironmental

Engineering) and 718 CES/CEIE (Environmental) for review and approval: Certificate of Training, Sampling Plan and Protective Equipment Plan.

c. Building access must be coordinated with the proper agencies through the 718CES/CENMA.

d. Building must be visually surveyed for existing finish, materials and painted surfaces which are suspected containing ACM or LBP.

e. Floor plan (small scale) of the building shall be submitted showing specific locations where samples were taken; e.g., duct flange gasket, duct flexible connection, paint on ductwork, walls, ceilings, interior and exterior paint, etc.. Test results shall be submitted to the Government.

f. Destructive sampling shall be taken from areas that are not easily visible and shall be patched to match existing finish.

g. Protection of properties: The Contractor shall protect all equipment, furniture and other items not removed in the work area with approved protective materials while accomplishing sampling.

4.2 Asbestos Containing Materials (ACM) and Lead Based Paint Abatement:

a. HEPA Vacuum Cleaner: Where ACM or LBP is found after testing, the Contractor shall apply the

HEPA vacuum cleaner for a drilling work of existing wall, floor and ceiling for new piping and conduit installation to avoid escape of the waste media into the surrounding environment, and the waste media shall be disposed in accordance with all applicable Federal, FEGS, GOJ, JEGS, Prefectural and local regulations.

b. Abatement work shall be completed and the waste media shall be disposed in accordance with all applicable Federal, FEGS, GOJ, JEGS, Prefectural and local regulations.

4.3 Cost Breakdown: The Contractor’s cost proposal shall include the cost for abatement of ACM and LBP.

5.0 Description of Work: The work consists of the following elements.

5.1 Demolition Work:

1. Remove existing typhoon damaged doors including door frames, closers, glasses, hardware, locksets, door stops and all other related items.

2. 718 Housing maintenance personnel will collect onsite usable parts from the removed doors to be used as replacement parts for their maintenance work. Contractor shall meet with the 718 Housing maintenance personnel thru project manager prior to start work to identify what items need to be salvaged.

3. Contractor shall remove glass on demolished doors and dispose. All hardware and related items that remained onsite after 718 Housing maintenance personnel collected all the usable parts shall be dispose by the contractor.

4. Contractor shall deliver all demolished aluminum doors and frames at the Kadena Recycling Center, B851.

5.2 Civil/Architectural Work:

1. Provide new half glazed single aluminum door including door frame, safety glass, closer, panic hardware, lockset, door stop, typhoon dead bolts with thumb turns and all other related necessary items at 1 st floor south stairway exit (1).

2. Repair and repaint all surface areas that were affected during construction and shall match existing conditions.

1. Provide new half glazed single aluminum door including door frame, safety glass, closer, panic hardware, lockset, door stop, typhoon dead bolts with thumb turns and all other related necessary items at roof top stairway exit (1).

2. Provide new double steel door including door frame, closers, panic hardware, lockset, door stops, typhoon dead bolts with thumb turns, and all other related necessary items at 1 st floor electrical room entrance (1). This door should be fire rated. Provide temporary partition wall or barrier during non-working hours until new doors are installed. Provide new warning sign on the outside of the door.

Note:

The new doors shall conform to UFC 3-301-01, UFC4-010-01, NFPA 101 & 80 and NEC.

Existing cylinders for the locksets shall be the same as the new ones due to emergency personnel access. All doors shall be keyed to match existing building master/common area key (same key cut). Provide at least 3 copies of spare keys.

Provide label on the door for the typhoon locks to indicate “Typhoon Use Only”. Also indicate which is locked/unlocked position.
