52.212-1 ADDENDUM, INSTRUCTIONS TO OFFERORS – COMMERCIAL ITEMS (APR 2014)
Only electronic proposals will be accepted. Proposals shall be received no later than 0800 Central European Summer Time (CEST) 14 Aug 2015 by email. E-mail submissions are subject to size and type restrictions (typically limited to less than 2 megabyte in size and only non-executable attachments such as .doc or .pdf files) as well as any other appropriate network security measures. It is the offeror's responsibility to verify the e-mail was received and can be viewed with the contracting point of contact. Emails shall be submitted to the Contracting Officers, Leslie Knight, Trevor Talamantez, and a copy to the Contract Specialist, Christoph Kehrein at leslie.knight.9@us.af.mil, trevor.talamantez@us.af.mil, and christoph.kehrein.de@us.af.mil.
52.212-1 (b)(4) is tailored as follows:
(4). In accordance with (IAW) Factor 1, the proposal shall at a minimum:
(a) Provide written evidence, limited to no more than a total of 3 pages, of offeror’s past experience in performing inspection, testing, maintenance and repair of Aircraft and Structural Live Fire Trainers.
Experience can be commercial and/or governmental, and experience can be demonstrated by one contract or multiple contracts. Each individual contract must have a minimum annual value of $100,000 and must have been performed entirely in the past three years from the date of solicitation release.
The offeror shall include the following information for each contract offered as evidence of experience:
Contract Number (for subcontracts, provide the prime contract number and subcontract number), Total Contract Dollar Value, Description of Requirement; Period of Performance; Contracting Agency/Customer.
(b) Provide a narrative, limited to no more than a total of 3 pages, outlining the offeror’s ability to provide inspection and preventative maintenance on structural and aircraft liquid propane gas (LPG) - fueled fire trainers (mobile and fixed facility) in accordance with (IAW) the PWS.
52.212-1 (b)(6) is tailored as follows:
(6) Price and any discount terms in accrodance with (IAW) Factor 2;
(a) Offeror shall utilize the schedule above as the proposal pricing schedule. The offeror shall complete the UNIT PRICE and AMOUNT in US Dollars for each service to be provided at each required location. The AMOUNT shall equal the sum of the UNIT PRICE. The offeror shall also provide a total price for the base period, each option period and the total price proposed for the five year period of performance. The total proposed price shall equal the sum of all services at all required locations and will confirm the extended amount.
52.212-1 (c) is tailored as follows:
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers.
52.212-1(f)(2)(i)(A) through 52.212-1(f)(2)(i)(C) are hereby replaced with the following: Your quote or offer was received in the designated Government email inbox and could be opened and viewed (to include any attachments).
No other Government point of entry, to include any server, will be acceptable for receipt.
INTRODUCTION
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Government will utilize simplified procedures in accordance with Subpart 13.5. This announcement constitutes the only solicitation; proposals are being requested and a separate solicitation will not be issued.
(ii) The solicitation number is FA5641-15-R-0009, and the solicitation is issued as a Request for Proposal (RFP).
(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83.
(iv) This is not a set-aside. The North American Industry Classification System (NAICS) code for this solicitation is 922160, Fire Protection.
(v) A list of contract line item numbers (CLINs) and items, quantities and unit of measure, including all options, is listed in the ADDENDUM to Federal Acquisition Regulation (FAR) 52.212-1.
(vi) United States Air Forces in Europe (USAFE) has a requirement for the inspection, testing, maintenance and repair of the USAFE structural and aircraft Liquid Propane Gas (LPG) –fueled fire trainers (mobile and fixed facility) in a manner that will ensure their serviceability and prevent deterioration in operational effectiveness. Attachment 1, Performance Work Statement, outlines the full details of the requirement.
(vii) Place of performance and acceptance is on site at Aviano Air Base (AB), Italy; Moron AB, Spain;
Lajes AB, Portugal; Ramstein AB, Germany; Spangdahlem AB, Germany; Royal Air Force (RAF) Fairford, United Kingdom (UK); RAF Lakenheath, UK; RAF Mildenhall, UK; RAF Croughton, UK, RAF Molesworth, UK. The period of performance (POP) will include one six month base period, four 12-month option periods and one six month option period commencing on 1 Oct 2015.
