Contract Title: Paramedic Services
1. DESCRPITION OF SERVICES.
1.1. General Description. The Contractor shall provide non-personal Paramedic services to provide treatment in emergency ambulance response; in-flight emergency response; non-in-flight emergency response; transfer and transport services for active duty military personnel, their dependents, retirees, their dependents and other eligible beneficiaries, in accordance with the terms and conditions of this contract. The Paramedic shall be required to assist with patient care within the Military Treatment Facility (MTF) when not performing emergency response or transport services. Paramedic shall cover the range of services provided in a civilian medical treatment facility, such as emergency patient care. The Paramedic shall operate under the medical direction of the awarded contractor’s Medical Director and protocols. The protocols will be provided/approved by the contractor’s Medical Director. The Paramedic will be assisted by an emergency medical technician (EMT) from the MTF.
1.2. PARAMEDIC SPECIFIC TASKS.
1.2.1. The Paramedic shall be responsible for creating, maintaining, and disposing of medical documentation as required.
1.2.2. Performs a full range of ambulance services on site at the MTF and in the field when performing ambulance emergency or in-flight response, transfer, or transport services. The paramedics will respond to calls within Moody AFB jurisdiction which includes the main base, Grand Bay, Quiet Pines Housing and Magnolia Grove Housing. The Paramedic shall practice within the guidelines set by the NCOIC, Flight Commander and the contractor’s Medical Director. Provides emergency hospital and pre-hospital care for all types of patients (all ages).
1.2.3. Maintains the government provided (GOV) ambulance according to local policy ensuring preparedness and availability for immediate response. A local checklist will be provided for daily/monthly checks. Shall obtain a valid government driver’s license for on-base response. A flight line driver’s license is not required.
1.2.4. Responds to advance life support (ALS) emergency calls within established time frames according to DoDI 6055.06, Table E3.T1: Aggregate Response Times (ART) for ALS calls will be within twelve (12) minutes for 90% of the calls. 23 CES/CEF Fire Control will be the official time keeper who screens all emergency calls and dispatches them via government issued hand-held radios or radios hard mounted in the ambulances.
1.2.5. Assesses, plans, implements, and evaluates emergency care for patients of all ages. Workload includes the evaluation and treatment of emergent, urgent, and non-emergent patients ranging from infants to the elderly.
Contractor shall adhere to all relevant guidelines established in Rules of Department of Public Health Office of Emergency Medical Services, Chapter 511-9-2, Emergency Medical Services.
1.2.6. Recognizes medical emergencies and responds appropriately. Initiates CPR if required, and administers advanced cardiac life support measures when necessary.
1.2.7. Provides/documents and completes patient exam on approved forms; provides verbal/written reports to receiving physician and medical facility. Performs documentation audit after each response is complete, to ensure documentation compliance with peer review standards. Peer reviews are conducted after each ambulance run. The results which are reviewed for trends and appropriate feedback will be sent to the contractor via the Contracting Officer Representative (COR).
1.2.8. Immediately cleans, sanitizes, and restocks GOV ambulance with MTF supplies according to established policy.
1.2.9. Assisted by EMTs the Paramedic shall be fully integrated into the nursing team, responsible for direct patient care.
1.2.10. Responsible for and assists with orientation and training of newly assigned and inexperienced personnel.

1.2.11. Participates in base exercises, in-services, and continuing education programs. Maintains MTF training record.
1.2.12. Establishes and maintains good interpersonal relationships with co-workers, families, peers, and other health team members.
1.3. CONTRACTOR QUALIFICATIONS.
1.3.1. Knowledge.
1.3.1.1. The following USAF specific knowledge must be completed within 30 days of the contractor start date:
1.3.1.1.2. Emergency medical treatment; Job and Office Safety standards; Aseptic and surgical technique; Infection control, communicable diseases; Medical ethics and legal aspects; Operation and maintenance of therapeutic equipment; Medical terminology; Transportation of the sick and injured; Disaster preparedness procedures, National Incident Management System (NIMS).
1.3.1.1.3. Knowledge of current hospital and pre-hospital protocols within 30 days of the contractor employee start date, in order to assess, plan, implement, and evaluate care of patients in hospital and pre-hospital settings. The Paramedic shall be required to assist with patient care within the MTF when not performing emergency response or transport services.
1.3.1.1.4. Knowledge and skill in cardiopulmonary resuscitation and advanced life saving measures used to initiate resuscitation measures when necessary and assist unit in providing support measures.
1.3.2. Education. Graduation from an accredited EMT-Paramedic certification program; a current, valid paramedic license and certification from the National Registry of Emergency Medical Technicians (NREMT); or State of Georgia Office of Emergency Medical Services (OEMS); Current certification in Basic Life Support (BLS);
Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS); and Hazard Materials Awareness. Certificate/license provided at time of contractor award or prior to start date for a replacement employee. An updated copy will be kept in their Competency Assessment Folder.
1.3.3. Experience. One (1) year of full-time experience having performed as a paramedic within the last three (3) years. Experience in personnel management is highly desirable.
1.3.4. Language Skills. The Paramedic must be able to read, write, and speak fluent English. All documentation shall be completed in English.
1.3.5. Driving Skills. Drivers must not have any reckless or dangerous driving citations within the previous twenty-four-(24)-month period and have no Driving While Intoxicated (DWI) or Driving Under the Influence (DUI) convictions in the previous five (5) years. Drivers can have a maximum of six (6) points per year assessed under the state of Georgia driving infraction system. Certificate/license provided at time of award or prior to replacement employee start date. An updated copy kept in their Competency Assessment Folder.
1.3.6 Physical Abilities. The Paramedic must be able to run 100 yards and lift 175 lbs with assistance. Verification provided at time of award or prior to replacement employee start date. An updated copy will be kept in their Competency Assessment Folder.

3. GOVERNMENT-FURNISHED EQUIPMENT, FACILITY, FORMS/PUBLICATIONS, MATERIALS,
PROPERTY, SERVICES, TECHNICAL ORDERS, AND TRAINING.
3.1. General Information. The Government will provide the following facilities, equipment, materials, services, technical order, publications and forms. If facilities, equipment, materials or services are not listed as Government-furnished or Government-shared, they shall be Contractor-furnished.
3.2. Government-Furnished Facility. The Government will furnish the following: Office and rest room at Building 900, 3278 Mitchell Blvd, Moody AFB GA for 1 employee. Dormitory capabilities to include sleeping quarters, showers, and restrooms for 1 employee at Building 621, 8169 Apron A Road, Moody AFB, GA.
3.3. Government-Furnished Equipment/Office Furniture. The Contractor shall have joint use of all available equipment for performing services required by this contract. Office space, furniture, standard desktop computer, telephone with DSN capability, fax, and reproduction capabilities will be available. Office supplies, necessary computer hardware and software programs, e-mail with af.mil address, computer technical support, and administrative assistance for travel requirements will be after approval from CO and CA.
3.3.1. Government-Furnished Supplies. The MTF will provide medical and non-medical supplies commonly used in the facility for the care and management of patients. Commonly used supplies are those routinely stocked by the
MTF.
3.3.2. Personal Protective Equipment (PPE). The Government will furnish the Contractor with appropriate PPE, e.g.: gloves, masks, gowns, equipment, etc. The Government will be responsible for any repair, cleaning, and inventory required for the PPE. This does not include any type of uniform, laboratory coat, or respirator devices.
3.3.3. Computer Equipment. The MTF will provide computer equipment required to schedule, check in, document, order ancillary services, and maintain appropriate electronic medical information that support the hard copy medical record. The MTF will provide required training for these systems. The Contractor is required to use the computer systems that are standard for the support of health care delivery at the MTF. All software and hardware, passwords/access to military Internet/e-mail and applicable databases will be available within the scope of this contract. The Contractor shall comply with local, AF and DoD policies concerning Information Management and Information Technology applications and connectivity to military systems.
3.3.4. Vehicles. The Contractor shall have use of a Government-supplied modular ambulance for emergency response according to the terms of this contract. The vehicle is a Standard Modular Type One Ambulance meeting DOT specifications. Must comply with licensing requirements of the State and local motor vehicle laws and shall be certified, by the Contractor and at the Contractor’s expense, as being fully qualified to operate the vehicles/equipment to which they are assigned in accordance with AFI 24-301, VEHICLE OPERATIONS, Section 8E, Licensing for Federal Civilian Employees, Contractor Personnel, and Federal Prison Camp Inmates, and paragraph 8.10, Licensing Contractor Personnel.
3.3.5. Information. The Government will provide manuals, texts, briefs and other materials associated with the execution of all contract requirements. All available documentation for current systems/training materials associated with this contract will be available for contract personnel. All provided documentation shall remain the sole property of the Government and will be returned to the Contracting Officer’s Representative (COR) upon completion of this contract. Information provided by the Government and/or developed during the performance period of this contract shall not be disseminated without approval by the COR/CO.
3.3.6. Equipment Accountability. The Contractor shall be liable for loss or damage to Government-furnished property and equipment beyond fair wear and tear in accordance with the clause of the contract. The Contractor shall report all lost, damaged or destroyed Government furnished equipment to the Contracting Officer (CO) within seventy-two (72) hours of discovery. In the case of loss or damage beyond economical repair to property and equipment, the CO shall determine the amount of the Contractor’s liability.
3.4. GOVERNMENT-FURNISHED SERVICES.
3.4.1. Telephone Services. The use of Government-supplied telephones is provided solely for official Government business and shall not be used to transact personal business by Contractor or his employees. The Contractor shall contact the COR for repair service. The Government will not be responsible for charges for long distance telephone calls made or accepted by Contractor personnel. Telephone services will be subject to the standard monitoring requirements of the Government telephone network.
3.4.2. Base Distribution. The Contractor may use the Base Information Transfer Center (BITC) to send mail on base.
3.4.3. Refuse Collection. The Contractor may use the Base existing dumpsters for refuse resulting from performance under this contract.
3.4.4. Infectious Waste. Will be disposed of IAW MDGI 44-7, Surveillance, Prevention, and Control of Infection.

3.4.5. Recycling Program. The Contractor will be required to participate in the base-recycling program.
3.4.6. Emergency Medical Service. 23d Medical Group, Moody Air Force Base, GA, will provide emergency (cases where loss of life, limb, sight or hearing is possible) medical services, if required, on a reimbursable basis.
The Contractor shall be responsible to reimburse the Government.
3.4.7. Utilities. The Government will furnish reasonable amounts of utilities to include electricity, water, sewage, and heating. The Contractor shall exercise responsible use of all utilities.
3.4.8. Housekeeping Services. The Government shall be responsible for housekeeping services in buildings and areas pertaining to this contract with the exception of the dormitory room assigned to the Paramedic staff
3.5. GOVERNMENT-FURNISHED PUBLICATIONS AND FORMS.
3.5.1. Publications and forms that apply to the PWS are listed below. The Contractor shall maintain current copies of forms and publications via the Internet at http://www.e-publishing.af.mil/ for the documents required to provide Paramedic services.

3.5.2. The Government will notify the Contractor as follow-on requirements changes occur. Supplements or amendments to listed publications from any organization level may be issued during the life of the contract. The Contractor shall immediately implement those changes that result in a decrease or no change in the contract price and notify the CO in writing of such change. Should a decrease in contract price result, the Contractor shall provide a proposal for a reduction in the contract price to the CO. Contract Title: Paramedic Services increase in contract price, the Contractor shall submit to the CO a price proposal within thirty (30) days following receipt of the change by the Contractor. Upon completion of the contract, the Contractor shall return to the Government all issued publications and forms.
3.5.3 Operating Instructions/Orientation. Operating instructions and all forms/publications will be available to contract personnel. Initial familiarization with the local installation/organization will be provided.
3.6. GOVERNMENT-FURNSIHED TRAINING.
3.6.1. MTF Requirements. The Contractor shall complete all MTF-specified orientation program(s), initial and all required annual training, and comply with all MTF policies, procedures, productivity standards and instructions as provided by the MTF.
3.6.2. Health Insurance Portability and Accountability Act (HIPAA) OF 1996:
All individuals performing services at an Air Force MTF are required to complete an initial and annual refresher HIPAA Privacy and Security Rule training as provided by the MTF for its personnel and will be held accountable for complying with health information privacy and security policies and procedures. Reference Appendix D, Section H, Special Contract Clauses, for detailed HIPAA language.
3.6.3. Periodic Continuing Medical Education (CME) may be conducted at the MTF and will be available, at no cost, to any healthcare provider desiring to attend.
4. GENERAL INFORMATION.
4.1. CONTRACTOR PERSONNEL.
4.1.1. Contract Manager. The Contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of the contract manager and an alternate(s), who shall act for the Contractor when the manager is absent, shall be designated in writing to the Contracting Officer (CO) prior to contract start date. The contract manager is responsible for both the contract Paramedic personnel and the contracted Medical Director.
4.1.1.1. The contract manager or alternate(s) shall have full authority to act for the Contractor on all contract matters relating to the daily operation of this contract.
4.1.1.2. The contract manager or alternate(s) shall be available during standard duty hours within sixty (60) minutes to meet on the installation with Government personnel designated by the CO to discuss problem areas. After standard duty hours the manager or designated alternate shall be available within two (2) hours.
4.1.1.3. The contract manager, alternate(s) and employee performing services shall be able to read, write, speak, and understand English.
4.1.2. Contractor Employees. The CO may restrict the employment under the contract of any contractor employee, or prospective contract employee who is identified as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population.
4.1.2.1. Contract personnel shall present a neat appearance and be easily recognized. The Contractor shall provide uniforms bearing the name of the company to identify contract employees and insignia, such as an Paramedic pin, to identify the employee as a medical professional.
4.1.2.2. The Contractor shall ensure that employees have the following current and valid professional certifications before starting work. Vehicle operators shall have a valid Georgia driver's license and shall obtain a valid government driver's license upon request. In the event that the contractor's employee resigns and a new person must fill the position, the new employee must provide current and valid professional certifications before starting work.

4.1.2.3. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person could create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DOD 5500.7-R Joint Ethics Regulation (JER), Chapter 2. In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities, paragraph 3.
4.1.2.4. The Contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment. Military Reservists and National Guard members may be subject to recall or active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform. Their absence at any time shall not constitute an excuse for nonperformance under this contract.
4.1.3. Medical Director. The contractor shall provide the services of a Medical Director who provides medical control to the paramedic contracted personnel IAW Chapter 511-9-2, Rules Of Department of Public Health Office of Emergency Medical Services. "Medical Control" means the clinical guidance from a physician to emergency medical services personnel regarding the pre hospital management of a patient. Care will be rendered utilizing approved medical protocols. "Medical Protocol" means a prehospital treatment guideline, approved by the contracted Medical Director, used to manage an emergency medical condition in the field by outlining the permissible and appropriate medical treatment that may be rendered by emergency medical services personnel to a patient experiencing a medical emergency. The contracted Medical Director must be available for telephonic or radio consultation/guidance 24 hours a day/7 days a week.
4.1.3.1. Duties of the local Medical Director shall include but are not limited to the following:
(a) The approval of policies and procedures affecting patient care;
(b) The formulation of medical protocols and communication protocols;
(c) The formulation and evaluation of contracted paramedic training objectives;
(d) Continuous quality improvement/quality assurance of patient care, to include medical peer review.
4.1.4. The contractor will provide liability insurance for their staff members/employees.
4.2. QUALITY CONTROL.
4.2.1. The Contractor shall provide a comprehensive Quality Control Plan (QCP) for all work required in this PWS.
The written QCP shall be submitted to the CO for validation no later than the pre-performance conference. The CO will notify the Contractor of validation or of any modifications that may be required to be made to the plan.
4.2.1.1. The plan shall include, at a minimum:
4.2.1.1.1. A description of the inspection system to cover all services listed on the Services Summary (SS). The description shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, submission of QC schedule and the title and organizational placement of the quality control inspectors (QCIs). Additionally, control procedures for any Government-provided keys or lock combinations shall be included.
4.2.1.2. A description of the methods to be used for identifying and preventing defects in the quality of service performed.
4.2.1.3. A description of the records will be kept to document inspections and corrective or preventive actions taken. This includes the use and distribution of required forms and reports.

4.2.1.4. The records of inspections shall be kept and made available to the Government throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.
4.3. GOVERNMENT QUALITY ASSURANCE.
4.3.1. The Government’s COR may evaluate the Contractor’s performance at any time during the duration of this contract. Inspections will be conducted in accordance with (IAW) the Quality Assurance Surveillance Plan with the intent of verifying compliance with contract requirements and performance within the performance thresholds included in the SS. The assessment/evaluation methods identified in the Plan, in concert with the Contractor’s quality control procedures, will also assist in evaluating the Contractor’s success in delivering the level of performance required in the contract. The Government COR may use the Contractor’s Quality Control Plan (QCP) as the basis for their evaluations. Following contract award, the QASP will be reviewed to ensure it will work cooperatively with Contractor’s QCP, but not duplicate its provisions. The COR will record all surveillance/assessment observations. When observation indicates defective performance, the COR will require the contract manager or representative at the site to initial the observation. This initialing of the observation(s) does not necessarily constitute concurrence with the observation, only acknowledgment that the Contractor’s representative has been made aware of the defective performance. If performance defects are identified and validated by the COR, the Contractor shall be provided the opportunity to re-perform. Disputes between COR and Contractor personnel over performance standards shall be referred to the CO for final determination. The Government reserves the right to inspect tasks not shown on the SS, as well as anything on the contract.
4.4. RE-PERFORMANCE.
4.4.1. Re-performance of Service. When service performed does not conform to contract requirements, the Contractor shall re-perform the service in conformity with the contract specifications at not additional cost to the Government. Re-performance of the service(s) shall commence immediately upon notification by the COR, and the Contractor shall have a sufficient workforce dedicated to ensure corrections are accomplished by noon the next day without degradation to standard services.
4.4.2. Acceptable Performance. Acceptable performance is achieved when the Contractor’s performance meets the minimum the contract standard(s).
4.4.3. Unacceptable Performance. Unacceptable performance exists when the Contractor does not meet the minimum contract performance standard(s).
4.4.3.1. When the Contractor’s performance is unacceptable, the COR will determine the cause of the unacceptable performance. If any Government action, or lack of action, caused or contributed to the unacceptable performance, the unacceptable performance will not be counted against the Contractor. The COR will take action to ensure Government action, or lack of action, does not interfere with the Contractor’s performance in the future. The COR will completely document the circumstance accordingly.
4.4.3.2. When the unacceptable performance is not the result of Government action, or lack of action, COR shall promptly notify the Contractor and may issue a *Corrective Action Report (CAR) to the Contractor to assure corrective action is taken. By initialing and dating the CAR, the Contractor representative is acknowledging that he/she has been informed of the unacceptable performance and is not necessarily agreeing with the performance assessment. The COR shall direct the Contractor to re-perform the service, if possible, without additional cost to the Government. If the Contractor challenges the validity of the COR unacceptable assessment finding, and the COR and Contractor cannot come to an agreement, the matter shall be referred to the CO for resolution. The Contractor shall be required to return all completed CARs to the COR no later than the suspense date indicated on the CAR.
*Note: The COR should discuss with the CO before issuing a CAR.

4.4.3.3. Unacceptable performance re-performed by the Contractor and acceptable to the COR shall not be considered a defect. However, unacceptable performance not corrected or re-performed in the timeframe required and still considered unacceptable by the COR will be considered “a defect”.
4.4.4. The COR shall maintain a CAR log of all CARs issued to the Contractor (a locally devised form may be used). The CAR log shall contain the CAR number, date issued, Contractor, description of unacceptable performance, suspense date, and closeout date. All CARs and CAR logs shall be maintained by the COR for the life of the contract.
4.5. HOURS OF OPERATION
4.5.1. Standard Duty Hours. The Contractor shall maintain services twenty-four (24) hours/day; 365 days/year.
4.5.2. Federal Holidays. The Contractor is required to provide services on the following days:

4.5.2.1. On work days that have been declared “family days,” the Base may be minimally manned with many offices and facilities closed. On these days, the COR shall ensure that the Contractor can gain entrance to the work site.
4.6. SECURITY REQUIREMENTS.
4.6.1. Pass and Identification. Pass and identification items, required for contract performance, shall be obtained for Contractor personnel and non-Government-provided vehicles. Procurement of the appropriate passes and identification items are the responsibility of the Contractor.
4.6.2. The Contractor shall obtain personnel pass application forms from the Contracting Office’s Contract Administrator. The Contractor employee shall complete and carry the forms to the Security Forces, Pass and Registration Office. Vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles to be registered.
4.6.3. Retrieving pass and identification items shall be accomplished by the contract manager upon the departure from employment, prior to contract expiration, of any Contractor employee and turned in to the Visitors’ Center, Pass and Identification Office.
4.6.4. Key Control. The Contractor shall establish and implement methods of ensuring all keys and vault combinations issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons.
4.6.4.1. In the event keys, other than master keys, are lost or duplicated, the Contractor may be required, upon written direction of the CO, to re-key or replace the affected lock or locks without cost to the Government. The Government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the Contractor. In the event a master key is lost or duplicated, the

Government shall replace all locks and keys for that system and the total cost deducted from the monthly payment due the Contractor.
4.6.4.2. The Contractor shall prohibit the use of keys or vault combination, issued by the Government, by any persons other than the Contractor’s employees and the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in performance of contract work requirements in those areas.
4.6.5. Traffic Laws. The Contractor and its employees shall comply with base speed limits and other traffic laws.
4.6.6. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately-owned vehicle while on Moody AFB GA.
4.6.7. Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
4.6.8. Controlled Areas. The Contractor shall implement local base procedures for entry to Air Force controlled areas where Contractor personnel shall work
4.6.9. Suitability Investigations. Contractor personnel shall successfully complete, as a minimum a National Agency Check (NAC), before operating unclassified automated information systems (AIS), for unescorted entry into restricted areas, or for access to unclassified sensitive information or sensitive equipment. The Government shall submit these investigations, at no additional cost to the Contractor. The Contractor shall comply with the requirements of DoDR 5200-2, Personnel Security Program, and AFI 33-119, Electronic Mail (E-Mail) Management and Use.
4.6.10. For Official Use Only (FOUO). The Contractor shall comply with DoD 5200.1R, Appendix C, Section 2, Information Security Program requirements. This regulation sets policy and procedures for marking, handling, transmitting, and safeguarding FOUO material.
4.7. SAFETY REQUIREMENTS.
4.7.1. Safety of personnel and performance shall conform to Office of Safety and Health Administration (OSHA) requirements as specified in AFI 91-301, Air Force Occupational Safety, Fire Protection and Health Program (AFOSH), paragraph 9, page 20.
4.7.2. Cell Phone Usage. Cell phone use is prohibited while driving on base, unless a completely “hands-free” device is utilized.
4.8. CONTINUATION OF ESSENTIAL CONTRACTOR SERVICES DURING CRISIS.
4.8.1. All services or any specifically designated portions of the functions accomplished under this contract shall be performed during any wartime or crisis operations. Wartime operations are those actions, including contingency planning, which would be required to support current or future United States Air Force wartime requirements.
Notwithstanding any other provision of this contract, the Contractor agrees that, in the event of the occurrence of any wartime-related event, crisis or contingency, the Government shall have the unilateral right to extend performance of this contract as long as that contingency/crisis exists. Contract payment will be in accordance with the existing contract rate. The Contractor shall be notified by email, telephone, in person or all three, within four (4) hours.
4.8.1.1. The Contractor shall provide the CO at the pre-performance conference the names and title of all employees who shall be responsible to work during such wartime operations.

4.8.1.2. The Contractor is responsible for providing the requested information as required in the DFARS Clauses 252.237-7023 and 252.237-7024 of this contract.
4.9. Listing of Employees. The Contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number, and type of investigation if contract work involves access to Government automated information systems (AIS), unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander, or for access to unclassified sensitive information or sensitive equipment.
The list shall be provided to the CO and the sponsoring agency’s Security Manager. An updated listing shall be provided when an employee's status or information changes.
4.10. GOVERNMENT PERFORMANCE OF SERVICES DURING LABOR STRIKES.
4.10.1. The services called for under this PWS are of critical importance to the Air Force, and the Government reserves the right to take over performance in the event of a labor strike by the Contractor’s employees which impairs the Contractor’s ability to satisfactorily perform the contract. In such event, the Government may elect to perform services with exclusively Government personnel in accordance with AFI 64-106, Air Force Industrial Labor Relations Activities, paragraph 2.2.
4.10.l. Under such circumstances and at the direction of the CO, the Contractor agrees to remove its non-striking force from the performance site and not to interfere in any way with Government performance.
4.10.2. Contractor further agrees under such circumstances to permit the Government to use any essential Contractor-furnished property. The Government will equitably compensate the Contractor for use of such property.
4.10.3. Contractor shall not be entitled to payment for any performance period or parts thereof during which the Government assumes performance.
4.11. CONTRACTOR EMPLOYEE TRAINING.
4.11.1 Medical Response Certification. At a minimum, the Contractor shall maintain current certification in either the American Heart Association Basic Life Support (BLS) (Course C) or the American Red Cross CPR/BLS Course.
The Contractor will also maintain certification in Advanced Cardiac Life Support (ACLS) and Pediatric Life Support (PALS).
4.11.2. Continuing Medical Education (CME) Requirements. The Contractor shall continue to meet the minimum standards for CME to remain current as prescribed in AFI 41-117, Medical Service Officer Education, paragraph 4.6, shall be obtained at no additional cost to the Government and shall be reported to the COR and MTF Credentials Committee annually on the first standard duty day in January for the previous calendar year.
4.11.3. The Contractor shall train, qualify and certify personnel for specific job tasks to be performed in Emergency Medical Services. Records of training, qualifications and certification shall be maintained by the Contractor on file and made available for Government review upon request.
4.11.4. The Contractor shall fund any additional training required to meet the scope of this contract and to maintain the proficiency of contract personnel during the performance period. If trained personnel resign prior to the end of this contract, the Contractor, not the Government, shall be responsible for all training costs for replacement personnel.
4.12. The Business Associate Agreement will be an attachment to the contract and will be filled out at time of award. The Business Associate Agreement states that the contractor shall comply with HIPAA rules.

APPENDIX A
DEFINITIONS
General Definitions
Contract Management. Implementation of the Quality Assurance Surveillance Plan (QASP) after contract award by the multi-functional team (MFT). It integrates the goals and objectives identified by the MFT oversight, performance management, and performance measurement, in accordance with the QASP.
Contracting Officer (CO). The only person who can bind, limited to the extent of the authority delegated by the head of the agency, through a warrant or certificate of appointment. The CO enters into, administers, or terminates contracts and makes related determinations and findings.
Contracting Officer’s Representative (COR). Individuals designated to perform quality assessment functions, and oversee performance in accordance with a Quality Assurance Surveillance Plan (QASP) They serve as on-site technical managers assessing Contractor performance against contract performance standards. Personnel in this area have many titles, such as Quality Assurance Evaluator (QAE), Quality Assurance Specialist (QAS), Functional Area Evaluators (FAEs), and Contracting Officer Technical Representatives (COTR). However, at Moody AFB GA they are called CORs.
Defective Service. Any service output that does not conform to contract requirements as stipulated in the Services Summary.
Functional Commander/Director (FC/FD). The person responsible for a functional area at the Squadron level.
For example, the Civil Engineer Squadron Commander is the Functional Commander for the Civil Engineer performance-based work statements.
Market Research. A continuous process for gathering data on service characteristics, suppliers’ capabilities, and current business practices plus the analysis of that data to make acquisition decisions.
Multi-Functional Team. A teams of stakeholders responsible for a service acquisition. Includes not only representatives of the technical and procurement communities, but also stakeholders of the service acquisition and the contractors who provide the services. This team could be a Business Requirements Advisory Group (BRAG), a Mission Area Acquisition Team (MAAT), an Integrated Product Team (IPT), working group, and A-76 IPT or Steering Group, etc. At Moody AFB GA this team is called the “MFT”.
Performance Assessment. A process that measures success towards achieving defined performance goals or objectives defined within the performance thresholds in the service summary or the process of assessing progress towards achieving the objectives/goals developed in a QASP.
Performance Management. The use of performance measurement information to effect positive change in organizational culture, systems, and processes, by helping to set agreed upon performance goals, allocating and prioritizing resources, informing managers to either confirm or change current policy or program direction(s) to meet those goals, and sharing results of performance in pursuing those goals.
Performance Objective. The outcome associated with successful contract performance in a specific area. This is a critical success factor in achieving the organization’s mission, vision and strategy which, if not achieved, would likely result in a significant decrease in customer satisfaction or risk mission failure. Obtaining multi-services/sub-services performed at a certain measurable standard and consistently ensures success in achieving the objectives critical to the mission.
Performance Threshold. The minimum performance level/standard of a performance objective required by the Government.
Quality Assurance Program. A program that is developed, planned, and managed to carry out cost-effectively all efforts to affect the quality of services. The QA Program includes performance measurement and performance management by the multi-functional team (MFT).
Quality Assurance Program Coordinator. Mission support group or AFMC/AFSPC Center level individual, normally from the contracting activity, selected to coordinate and manage the Quality Assurance Program.
Quality Control. Those actions taken by a Contractor to control the performance of services so that they meet the requirements/standards of the PWS.
Services Summary (SS). A summary of the performance objective(s) and performance threshold(s) required by the Government in contractor performance. The items listed in the summary are mission essential
