MEMORANDUM FOR 4 CES/CEOER
ATTENTION: CUSTOMER SERVICE-MS. HASTINGS &

FROM: 4 CES/CEOER-Abaternent Shop
SUBJECT: Facility inspection/ Assessment Letter
1. The following information is provided per your request for asbestos and or lead paint inspection and or sampling in multiple facilities 3603, 3605, 3606, 3607, 3613, 3614, 3615, 3616, 3651 and 3652. Multiple DSW's were opened to support this request. This requests supports project VKAG l 1=1084P2, "Repair Dorm Cameras Phase 2".
2. Two prior assessments and the current was accomplished May 31, 2016. The cameras will be installed on walls and installation will require drilling into or through interior and exterior wall material. The exterior walls are brick, block or texture materials. Interior walls are plaster, texture and sheet rock. There are three prior positive samples: one in facility 3603-joint compound from 3rd floor day room wall, one from 3607-ceiling patch material in the 2nd floor hallway and one from facility 3616-texture material on the concrete decking. The samples contained 3%-4% chrysotile asbestos. We are unsure if these materials are remaining in the facilities. All other prior mentioned materials are non-asbestos containing. Paint contains .004%-.007% lead by weight. No other suspect asbestos or lead containing materials were identified for sampling or analysis.
3. These facilities may not be clear of asbestos or lead in paint as identified above. Your request is approved with the following requirements. Personnel disturbing asbestos or lead must have prior training and PPE. You must fo llow the OSHA Lead in Construction regulations. You may contact OSHA @ www.osha.gov, Kim Morton @ 919-790-8096, NC HHCB@ 919-707-5950 or Abatement Shop for further asbestos or lead guidance.
4. This information is in reference to the materials and areas identified above. It does not cover materials or areas not identified or accessible for inspection or sampling. Other materials may contain asbestos or lead and must not be disturbed until an inspection can be performed to determine the potential health hazard. This assessment is valid for one year or invalid if the original scope of work changes.
