1. SUMMARY OF EXPECTATIONS
1.1. Existing light fixtures noted on the construction drawings in Bldgs 450, 850, 870, 1145, 1448, 1845, 3080 and 3081 will be removed and replaced with more energy efficient light fixtures as defined in the table below.
1.2. The contractor shall provide all items, articles, materials, operations, or methods listed, mentioned or scheduled and/or as specified herein required to replace the lighting fixtures in the buildings to a more energy efficient lighting fixture based on the specification on the drawings and detailed below.
1.3 Once the new fixtures have been installed, the contractor shall test each buildings’ lighting system in the presence of the Contracting Officer’s Representative to ensure the new lighting is working correctly.
1.4 Operations and Maintenance Manuals: One complete set of instructions, in post-form binders, containing the manufacturer’s operating and maintenance instructions for each type of lighting fixture installed. shall be furnished to the Contracting Officer. In addition, (2) CD-ROM discs containing the same information as the hard copy shall be provided. Where information or instructions are included for more than one model, the Contractor shall clearly mark the model actually installed on the job. The manuals shall be sectionalized using dividers with labeled tabs identifying the product/item behind the dividers. One complete set shall be furnished at the time test procedures are submitted, and the remaining sets shall be furnished before the contract is completed. Furnishing operating and maintenance instructions does not relieve the Contractor of the responsibility of furnishing framed instructions and schematics where specified. Each binder shall be indelibly marked on the exterior front and edge with the project name, project number, contract number, name of contractor and the date.
1.5 Guarantees: Provide a list of all equipment items which are specified to be guaranteed, accomplished by a copy of each specified guarantee therefore. For each specific guaranteed item on the list shall include the name, address, and telephone numbers of the subcontractor who installed the item, the supplier, or distributor and the manufacturer. The completion data of the guarantee period shall correspond to the applicable specification requirements for each guarantee item.
1.6 Submittals: Contractor shall submit three copies of all submittals using AF IMT Form 3000, to be recorded on AF Form 66. Submittal items, including samples, shall be marked with a number corresponding to the item number on the AF Form 66, and the page and paragraph number of the specification requiring the submittal. Contractor shall hi-lite specifics when submitting catalogs, etc. It is the responsibility of the contractor to complete column 13, “Required Submission Date”, in the submittal form and insert dates for each submittal item not already designated by the Government as having a mandatory due date. The contractor shall schedule and submit all required items for major systems at the same time to enhance review of interrelated components. It is the Contractor’s responsibility to ensure the dates provided on the form do not create any delays in construction. Provide the completed form to the Contracting Officer at or prior to the Pre-Construction meeting. The completed form will then be used by the Government for tracking submittal progress. If the contractor is proposing to use an exact brand name or product identified, a submittal for that item is not required. In lieu of a submittal, the contractor shall submit a letter to the Contracting Officer outlining each item that will be provided exactly as specified. These items will also be annotated as such on the AF Form 66. Even though a specified product is provided, in some circumstances a submittal may still be required. If a recycle initiative product is specified and the contractor wishes to provide a substitute product, the Contractor shall request a substitution.
1.7 Maintenance of Air Force Operations: It is required and essential that the primary use of the facilities continue uninterrupted throughout the contract period. Work, once commenced, shall be carried to completion with a minimum of interruption to Government operations.
2. HOURS OF OPERATION
2.1. Normal Hours. The contractor shall perform all the routine services required under this contract during the following hours: 0700 through 1600 Monday through Friday, with the exception of Federal Holidays, or when the base is officially closed by the Base Commander.
2.2. Recognized Holidays. The contractor is not required to provide routine service on the following days: New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. NOTE: Normally, if the holiday falls on Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.
3. CONTRACTOR REQUIREMENTS
3.1. The contractor shall provide all items, articles, materials, operations, or methods listed, mentioned or scheduled and/or as specified herein, including all labor, materials, equipment, and incidentals required to replace the lighting fixtures in the buildings with more energy efficient lighting fixtures based on the specification on the drawings and detailed below.
3.2. The Building Lighting to be provided and installed by the contractor is described below. All new light fixtures are to be installed at locations of existing fixtures. Locations shown on plan sheets are all approximate and shown for information only. All removed fixtures shall become the property of the contractor and shall be disposed of properly.

Light fixture requirements:
· Induction type light fixtures shall have a diffuser cover, UL Listed, 120V, minimum 5 year warranty
· LED Light fixtures shall be DLC premium certified and UL listed, diffused lighted panel or diffused cover, 120V, minimum 80 lumens per watt, minimum 25000 hours life, minimum 5 year warranty
· T5 light fixtures shall have a diffuser cover, UL listed, 120V, minimum 5 year warranty
CONTRACTOR EMPLOYEES
4.1. Security. The contractor shall not employ persons for work on this contract if such employee is a potential threat to the health, safety, general well-being or operational mission of the installation and its population.
4.2. Contract Manager. The Contractor shall appoint a Project Manager (PM) to provide daily supervision and quality control, and who shall represent the Contractor and be responsible for the performance of the work throughout the term of the contract. The name of this person, and an alternate(s) who shall act for the Contractor when the manager is absent, shall be designated in writing to the CO and the Quality Assurance Personnel (QAP).
4.3. Safety: The Contractor shall comply with all existing Occupational Safety and Health Act (OSHA) standards of safety at all times in the performance of this contract. Hazards to the safe use of the premises due to the Contractor’s work and/or equipment shall be suitably marked at all times. Pedestrian and vehicle traffic ways shall be kept clear and unobstructed.
MISHAP NOTIFICATION AND REPORTING
5.1 The contractor shall immediately report any and all injuries and damage involving contractor, Air Force civilian or military personnel to the QAP and the Wing Safety Office.
5.2 Cooperate fully and assist government personnel in any government investigation of the mishap, until released by the mishap investigative authority through the CO. Safety personnel shall have the right, at any time, to investigate and report all mishaps involving Air Force personnel and property. The CO and Contractor Safety Representative may accompany Air Force mishap investigation personnel but may not release information regarding the mishap without written approval of the AF Safety investigating official or senior installation commander.
5.3 Maintain records of contractor employee’s occupational injuries, illnesses, and other related activities as required.
SECURITY REQUIREMENTS
6.1 Contractor will be required to comply with all security measures while on Malmstrom AFB. Contractor will submit a list of employees requiring access to the base at least seven days prior to work start.
6.2 Activities of the Contractor and Contractor’s employees and sub-contractors and their employees while on base will be in accordance with base regulations, including those of the fire marshal as well as any security directives. This includes, but is not limited to, giving way to all emergency vehicles that are in response, properly wearing seat belts at all times, and refraining from use of cell phones while driving except when using a hands-free device.
7. HOUSEKEEPING AND CUSTODIAL SERVICES
The contractor must maintain the contractor assigned areas in a clean, safe, and sanitary condition. Remove trash from all the work areas and store all flammables in an approved flammable storage container and ensure the area is left in an orderly manner at the end of the duty day. All waste material shall be legally disposed of off-base by the contractor.
8. UTILITY OUTAGES
8.1. Any utilities affecting buildings other than building 500 that are programmed to be interrupted during work shall be scheduled at least 14 days in advance of the outage and at a time convenient for the government. The “Utility Outage Notice” will be completed by the Contractor and submitted to the QAE for approval. No interruptions shall be made until the outage notice is approved and returned to the Contractor. Notification forms that are to be filled out by the Contractor will be provided at the pre-performance conference. Utilities will include all overhead and underground utilities and road closures or partial closures.
8.2. Prior to work commencing, the contractor shall survey the site to familiarize themselves with the work to be done. The Contracting Administrator and QAE will contact appropriate base personnel so that existing underground utilities that are to remain in-place can be marked. At any point where the Contractor’s routes cross base utilities, the contractor must hand-dig or Hydro-Vac those locations. This must occur when his routes come within 3 feet of the existing utilities. If the contractor hits, or hits and damages any existing utilities, he must immediately notify CE operations support and the inspector, and the Contractor must cease work. Base personnel will be dispatched to inspect the situation. Any damages caused by his efforts shall be the financial responsibility of the Contractor. The contractor must repair the damage caused or obtain the services of personnel experienced in the repairs to the damaged utility.
9. CONTRACTOR VEHICLES AND EQUIPMENT
9.1. The Contractor shall provide all vehicles and equipment necessary to safely accomplish the contract work. Power equipment shall be equipped with safety and noise limiting devices. The equipment shall be in a safe and efficient operating condition. The Contractor will not park vehicles in the project area during non-working hours. Vehicles will not be allowed on lawns or sidewalks without prior written approval. All equipment will be clearly marked with the Company’s name. Seatbelts must be worn at all times while operating a vehicle on base. Additionally, the use of cellular phones without a hands-free operation is prohibited while operating a vehicle on base.
10. INSPECTION.
10.1. Operations Support will provide the QAP to verify work quality. The inspector will be the spokesman for compliance with base standards as well as the specifications and drawings. Controversies between the inspector and the Contractor will be resolved by the Contracting Officer. If, for some reason, a change in the primary or alternate inspector is required, the Contractor will be notified in writing by the Contracting Officer. The Contractor shall coordinate Contractor activities with CE Operations Support.
10.2. Pre-Final Inspection. The Contractor must notify the Contracting Officer and/or Administrator 24 hours in advance requesting a pre-final inspection.
10.3. Final Inspection. The Contractor will notify the project inspector of his estimated work completion date at least 10 days in advance, so that a final inspection can be held by base personnel. Any deficiencies found shall be rectified by the Contractor and the Contractor shall inform the project inspector when the deficiencies have been corrected. The project coordinator will re-inspect the completed work to verify the corrections.
