1.0 Contract Title. Chapel Religious Multiservice Provider

2.0 Scope of Responsibilities. The Contractor shall provide qualified individuals to support F. E.

Warren AFB Chapel religious services and programs as specified in this Performance Work Statement (PWS) in each religious position section. There will be seven positions consisting of the following: Catholic Religious Education Coordinator, Protestant Religious Education Coordinator, Catholic Service Music Coordinator, Catholic Service Musician, Protestant Contemporary Service Music Coordinator, Protestant Contemporary Service Musician, and Protestant Traditional Service Musician. The Contractor shall ensure individuals meet and maintain all required qualifications for their respective position, effectively communicates with the Contracting Officer Representative to ensure individuals who fill the positions are suitable to the needs of the religious service or program for which they are performing services. The Contractor shall ensure all individuals understand their respective responsibilities outlined in this PWS.

3.0 Education and Experience Requirement. The Contractor and all individuals assigned to specified position(s) must:

3.1 Be at least 18 years of age and hold a high school diploma.

3.2 Have at least 2 years of experience and familiarity with responsibilities outlined in their respective positions.

3.3 Be able to effectively communicate verbally and in writing.

3.4 Be able to work cooperatively in an interfaith setting with chaplains, staff and parishioners of varied and diverse religious backgrounds.

4.0 Absences. The Contractor shall provide a qualified subcontractor when absent or otherwise unable to perform for the scheduled service and shall notify the Contracting Officer Representative one week in advance of the name and the dates the subcontractor will perform.

Sub-contracted work should not exceed 30 days during the period of the contract.

4.1 Absences of Individuals Assigned to Religious Positions. With the exception of emergencies or sudden illnesses, the Contractor shall provide a qualified individual to perform services when an individual assigned to a specified religious position is absent or otherwise unable to perform for the scheduled service and shall notify the Contracting Officer Representative one week in advance of the contact information of the fill-in individual as well as dates he/she will perform. Religious Education Coordinators shall be absent for no more than two weeks during the academic year, with the exception of emergencies.

5.0 Government Provided Equipment.

5.1 The Wing Chaplain will provide all facilities and equipment required for the performance of each individual serving in their religious position.

5.1.1 Religious Education Coordinators. The Wing Chaplain will provide each position with office space to include a desk, chair, computer with access to a printer, filing cabinets, bookcases, phone, expendable office supplies and other items deemed

Chapel Religious Multiservice Provider Page 4 necessary to perform services described in this PWS. Provided equipment are to be used only in the facilities in which the individual will perform services.

5.1.2 Musicians and Music Coordinators. The Wing Chaplain will provide each position with music materials, instruments such as piano, organ, keyboard and/or guitar, and other equipment deemed necessary to perform services described in this PWS.

6.0 Period of Performance. The contract will be in effect for 12 months with one (12-month) option period.

7.0 Payment. The Contractor will submit monthly invoices to the Wide Area Work Flow (WAWF) IAW FAR Clause 252.232-7006 WAWF Payment Instructions.

7.1 Religious Education Coordinators. One (1) unit is equivalent to one (1) week of no more than 25 hours per week. There are 52 units allotted during the 12-month period of the contract.

7.2 Musicians and Music Coordinators.

7.2.1 Musicians: A rehearsal or performance is equivalent to one (1) unit. There are 52 regular rehearsal units, 52 regular performance units and 10 special events rehearsal/performances units during the 12-month period of the contract. The 10 special event units can be used as either a special rehearsal or special performance during the 12-month period of the contract.

7.2.2 Music Coordinators: A rehearsal or performance is equivalent to one (1) unit. There are 52 regular rehearsal units, 52 regular performance units and 10 special events rehearsal/performances units during the 12-month period of the contract. The 10 special event units can be used as either a special rehearsal or special performance during the 12-month period of the contract.

7.3 The Contractor is accountable for paying any Social Security payments, Federal or Local Income Taxes, and State Unemployment Insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract.

Health care insurance will be the responsibility of the contractor and the F. E. Warren Chaplain Corps will not be liable for any injury or accident associated with this service contract.

8.0 Religious Position and Title. Catholic Religious Education Coordinator

8.1 Scope of Responsibilities. The Catholic Religious Education (RE) Coordinator shall develop, execute and manage the F. E. Warren Catholic RE program. The RE Coordinator shall coordinate and train catechists for Archdiocese of the Military Services (AMS) certification and to prepare and coordinate the entire youth and adult catechetical programs including sacramental preparation. The RE Coordinator is the Catholic RE Volunteer Supervisor and works with the Chaplain Corps Volunteer Program Manager to execute the Catholic RE Volunteer Program.

8.2 Specific Education and Experience Requirements. The Catholic RE Coordinator must:

8.2.1 Have at least 2 years of experience and familiarity with Catholic programs.

8.2.2 Possess relevant training and education (ministry-related field), have experience working within a volunteer-based Catholic RE program.

8.2.3 Have the ability to communicate effectively, efficiently organize, and manage diverse program elements.

8.2.4 Be proficient with Microsoft Office and Adobe software programs.

8.2.5 Have current AMS basic minimum certification for RE Coordinator or fulfill the

AMS basic minimum certification requirements and obtain the certification within 6 months of the assignment of the position.

8.2.6 Furnish resume including academic and work-related references (i.e., Catholic churches, schools, etc.).

8.2.7 Obtain a favorable criminal history background check in accordance with DoDI 1402.05, Background Checks on Individuals in Department of Defense Child Development and Youth Programs. This includes but is not limited to the following: Personal/Security Questionnaire, National Security Agency Check Questionnaire, Installation Records Check, and fingerprinting.

8.3 General Responsibilities. The service shall be performed in accordance with standard religious education practices and this PWS. The Catholic RE Coordinator shall:

8.3.1 Be familiar with and provide services in accordance with DoD, Air Force, Air Force Chaplain Corps, and F. E. Warren policy, guidance, and instruction governing provision of services.

8.3.2 Act as the point of contact for all Catholic religious education requirements and maintain all files with all pertinent information for all religious education activities.

8.3.3 Schedule, plan and conduct special religious education programs. Special programs include but are not limited to a RE volunteer appreciation event, RE end of year activity, or RE holiday event.

8.3.4 Work with the Wing Chaplain, Deputy Wing Chaplain and the Catholic Priest in implementing the Catholic RE program.

8.3.5 Maintain office hours in the provided office space at least 3 days per week during Monday through Friday, between the hours of 0730-1630, totaling 10 hours per week.

8.4 Specific Responsibilities. The Catholic RE Coordinator shall:

8.4.1 Maintain religious education enrollment and weekly attendance reports.

Consolidate weekly attendance reports using local or Air Force form for Individual Religious Education Enrollment and Registration Record. Contact the parents/guardians of the student(s) absent for three or more consecutive classes.

8.4.2 Serve as the Catholic RE Volunteer Supervisor. Develop a clear description of the duties and scope of responsibility for each volunteer position. Is supervised by the Chaplain Corps Volunteer Program Manager.

8.4.2.1 Utilize DD Form 2793, Volunteer Agreement for Appropriated Fund Activities & Non Appropriated Fund Instrumentalities to note a brief description of duties.

Alternatively, may include the description of duties as an Attachment to DD Form 2793.

8.4.2.1.1 Complete Part 1 of DD Form 2793.

8.4.2.1.2 Present the volunteer with a copy of their completed DD Form 2793 with

Part IV, and provide the finalized document to the Volunteer Program Manager.

8.4.3 Provide initial and annual orientation and training for RE program volunteers.

Initial orientation and training must include, at a minimum:

8.4.3.1 Assigned Duties.

8.4.3.2 Scope of Responsibilities.

8.4.3.3 Written information regarding their immediate supervisor, means of contacting their supervisor, conflict resolution procedures, and assistance procedures.

8.4.3.4 Procedures to document the number of hours of voluntary service.

8.4.3.5 Types of services provided.

8.4.3.6 Policies and procedures for obtaining reimbursement for incidental expenses.

8.4.3.7 Opportunities for recognition.

8.4.3.8 Child Care Criminal History Background Check requirements.

8.4.3.9 Line-of-Site Supervision requirements.

8.4.3.10 Self-reporting requirements.

8.4.3.11 Privileged Communication Non-Disclosure Agreement.

8.4.3.12 Safety Requirements.

8.4.4 Vehicle Training. If volunteer duties include driving a vehicle, the volunteer must possess a valid driver’s license. Volunteers are subject to screening through the National Driver Registry and must comply with applicable training requirements.

8.4.5 Recurring Training. All volunteers will receive recurring, annual training concerning the volunteer requirements listed in AFI 52-105, Chaplain Corps Resourcing. Volunteer supervisor will note the training on a sign-in sheet.

Supervisors will provide a copy of the sign-in sheet to the Volunteer Program Manager for storage in the volunteer’s record.

8.4.6 Provide annual feedback to volunteers on performance.

8.4.7 Report each volunteer’s service hours to the Volunteer Program Manager at least once per month.

8.5 Ensure that all volunteers in the religious education program have undergone mandatory background records checks as directed by DoDI 1402.05, Background Checks on Individuals in Department of Defense Child Development and Youth Programs.

8.6 Review professional religious education curricula and disseminate relevant materials to the teaching staff.

8.7 Attend the F. E. Warren AFB Chapel weekly staff meeting and report on the status of Catholic RE.

8.8 Attend the F. E. Warren AFB Chapel scheduled Catholic Community Advisory Council meetings.

8.9 Evaluate, select and procure Vacation Bible School (VBS) curriculum, recruit and train the VBS teachers and other support staff, publicize the event, coordinate supply needs, plan and accomplish VBS in cooperation with the local diocese and chapel staff.

8.10 Prepare publicity concerning the Catholic RE program for the F. E. Warren Chapel in base publications and off-base newspapers, etc. Proposed publicity must be coordinated in turn through the Deputy Wing Chaplain and the Wing Chaplain prior to submission to the appropriate agency.

8.11 Prepare and submit a Catholic RE Chapel Tithes and Offerings Fund (CTOF) and Appropriated Fund (APF) annual budget not later than 1 May for the upcoming fiscal year.

8.11.1 Coordinate logistical needs for Catholic RE. Be familiar with CTOF AFI 52-105, Chaplain Corps Resourcing. Submit requests through the CTOF Account Manager using AF Form 4356 and/or through the APF Approving Official using the Form 43.

8.11.2 Select and submit the RE curriculum order with approval from the Wing Chaplain, Deputy Wing and the Catholic Priest. Ensure Wing Chaplain (or Deputy Wing Chaplain in his/her absence) verifies approval by signing the appropriate purchase request form(s) prior to submitting the form for purchase.

8.12 Process F. E. Warren AFB Chapel Facility Request form (local form) to schedule the use of chapel facilities for all activities within the facilities. Ensure that all facilities used are properly maintained, cleaned and secured upon departure.

8.12.1 Ensure the RE facility is opened at least one-half hour before RE classes begin and maintain a presence until classes are over and all children have departed the building with their guardian(s).

8.13 Develop a working knowledge of sacramental policies required by the AMS to ensure proper Catholic RE protocols are maintained. In the absence of the Catholic Priest will ensure the required submissions with AMS are completed as necessary.

8.14 In the absence of the Catholic Priest, provide RE statistical data to the Wing Chaplain or Deputy Wing Chaplain by the 3rd of the month.

9.0 Religious Position and Title. Protestant Religious Education Coordinator

9.1 Scope of Responsibilities. The Protestant Religious Education (RE) Coordinator shall develop, execute and manage the F. E. Warren Protestant RE program for children, youth and adults of the Protestant community. The RE Coordinator is the Protestant RE

Volunteer Supervisor and works with the Chaplain Corps Volunteer Program Manager to execute the Protestant RE Volunteer Program.

9.2 Education and Experience Requirement. The RE Coordinator must:

9.2.1 Have at least 2 years of experience and familiarity with church programs.

9.2.2 Possess relevant training and education (ministry related field), have experience working within a volunteer-based Protestant religious education.

9.2.3 Have the ability to communicate effectively, efficiently organize, and manage diverse program elements.

9.2.4 Be proficient with Microsoft Office and Adobe software programs.

9.2.5 Furnish resume including academic and work related references (i.e. military chapels, churches, schools).

9.2.6 Obtain a favorable criminal history background check in accordance with DoDI

9.5 Select and submit the religious education curriculum order for the following academic year after determining curriculum needs in turn through the Protestant RE Chaplain, Deputy Wing Chaplain and the Wing Chaplain.

9.5.1 Attend the F. E. Warren AFB Chapel weekly staff meeting and report on the status of Protestant Religious Education.

9.5.2 Attend the F. E. Warren AFB scheduled Protestant Advisory Council meetings.

9.5.7 Write and maintain after action reports for all major Protestant RE events.

9.5.8 Provide RE statistical data for the Air Force Chaplain Corps Activity Reporting

System (AFCCARS) by the 3rd of the month to the Active Duty RE Chaplain.

Compile weekly, monthly and annual statistics related to the program(s) using reporting tools as required.

10.0 Religious Position and Title. Catholic Service Music Coordinator

10.1 Scope of Responsibilities. The Music Coordinator shall provide, under the supervision of the service Chaplain, direction of the Catholic music program to include direction of congregational singing, choir/band recruitment, rehearsal, performance, and special music presentations.

10.2 Education and Experience Requirement. The Music Coordinator must:

10.2.1 Be an accomplished musician with solo and accompaniment experience in a range of music styles and traditions that include classical and traditional music.

Experience in church music, education, and background in vocal and instrumental music instruction, and Catholic liturgical music is desirable.

10.2.2 Have 2 years of experience leading Catholic style worship music including experience with recruiting, instructing and conducting volunteer vocalists.

10.2.3 Have experience providing musical ministry under direction and oversight of a presiding Chaplain or hired/contracted Priest and coordinating music with long and short term pastoral ministry goals.

10.2.4 Furnish resume including academic and work related references (i.e. military chapels, churches).

10.3 General Responsibilities. The Music Coordinator shall:

10.3.1 Coordinate with the service Chaplain for the selection of proper music for the week's services to ensure that selections of music and musicians are appropriate to the theme, season of the church year, and/or purpose of each service.

10.3.2 Provide piano, organ and/or keyboard accompaniment for the Catholic service, lead the parish singing, and provide special music through performance or coordinating guest performers.

10.3.3 Be able to work cooperatively in an interfaith setting with chaplains, staff and parishioners of varied and diverse religious backgrounds.

10.3.4 Recruit volunteers (vocalist and musicians).

10.3.5 Participates in interview process for potential contractor-assigned musicians and provides input to the service Chaplain.

10.4 Specific Responsibilities. The Music Coordinator shall:

10.4.1 Conduct rehearsals and performances preferably in an F. E. Warren Chapel facility except when authorized by the service Chaplain or the Deputy/Wing Chaplain and agreed upon by all service band/choir members.

10.4.2 Lead a rehearsal each week in advance of a scheduled performance. Rehearsals will last at least one (1) hour, but no more than two (2) hours, and include the Catholic Service Musician and volunteer band/choir members.

10.4.3 Lead and attend weekly performances for the Catholic service. Performance will last at least one (1) hour, but no more than two (2) hours. This unit shall include a 15-minute pre-service warm-up session no later than 0845 or time designated by the service Chaplain. The warm up is not a unit for pay.

10.4.4 Ensure that all set up (printed materials, stands, sound equipment, instruments, multimedia presentations, etc.) are completed at least 15 minutes before the start of each rehearsal and service. Ensure that all slides, music and video using copyright protected music are clearly marked with the Chapel's Church Copyright License (CCLI) number.

10.4.5 Wear attire appropriate for a family worship event while performing units.

10.4.6 Setup and take down and store all necessary items rehearsals and performances.

Ensure that the areas used will be properly secured upon completion of rehearsals and performances. Ensure that the areas used will be properly secured upon completion of the worship service.

10.4.7 Inform the service Chaplain if the instruments need tuning or repair.

10.4.8 Submit any request for music orders through the service Chaplain.

10.4.9 Coordinate with the service Chaplain for special services (e.g., Christmas Eve or funeral) before performing at the special services. Primary focus of this position is to provide music for the Catholic Service. Priority of resources (personnel-and Chapel- owned equipment) shall be to the worship service. The Music Director will receive notice of a special event performance no less than 30 days in advance and will have right of refusal for short-notice events (e.g., memorial service music). Such events will constitute one (1) unit of performance.

10.4.10 Process F. E. Warren AFB Chapel Facility Request form (local form) to schedule the use of chapel facilities for all Catholic music activities within the facilities. Ensure that all facilities used are properly maintained and secured upon departure.

11.0 Religious Position and Title. Catholic Service Musician

11.2.1 Be an accomplished musician with solo and accompaniment experience in a range of music styles and traditions that include classical and traditional music.

Experience in church music, education, and background in vocal and instrumental music instruction, hymnology, and Catholic liturgical music is desirable.

11.2.2 Have at least 2 years of experience in Catholic music.

11.2.3 Furnish resume including academic and work related references (i.e. military chapels, churches).

11.3 General Responsibilities. The Musician shall:

11.3.1 Perform music selections for the Catholic worship service and special music for the performance and/or coordinating guest performers.

11.3.2 Be able to work cooperatively in an interfaith setting with chaplains, staff and parishioners of varied and diverse religious backgrounds.

11.3.3 Coordinate with the Catholic Service Music Coordinator for the selection of proper music for the week's services to ensure that musical selections are appropriate to the theme, season of the church year, and/or purpose of each service. The music selection is to be determined by the Music Coordinator.

11.4 Specific Responsibilities. The Musician shall:

11.4.1 Conduct rehearsals and performances in an F. E. Warren Chapel facility except when authorized by the service Chaplain or the Wing Chaplain and agreed upon by all members.

11.4.2 Attend a rehearsal each week in advance of a scheduled worship service.

Rehearsals will last at least one (1) hour, but no more than two (2) hours.

11.4.3 Attend music worship for the Catholic worship service. Performances will last at least one (1) hour, but no more than two (2) hours. This unit shall include a 15-minute pre-service warm-up session no later than 0900 or time designated by the service Chaplain. The warm up is not a unit for pay.

11.4.4 Ensure that all set up (printed materials, stands, sound equipment, instruments, multimedia presentations, etc.) are completed at least 15 minutes before the start of each rehearsal and service. Ensure that all slides, music and video using copyright protected music are clearly marked with the Chapel's Church Copyright License (CCLI) number.

11.4.5 Wear attire appropriate for a family worship event while performing units.

11.4.6 Handle any/all music equipment with proper care.

11.4.7 Setup and take down and store all necessary items for their rehearsal and performances. The Musician will ensure that the areas used will be properly secured upon completion of rehearsals and performances. The Musician will ensure that the areas used will be properly secured upon completion of the worship service.

11.4.8 Inform the Music Coordinator if the instruments need tuning or repaired.

11.5 Coordinate with the Music Coordinator for special services (e.g., Christmas Eve or funeral) before performing at the special services. Primary focus of this position is to provide music for the Catholic Service. Priority of resources (personnel- and Chapel-owned equipment) shall be to the worship service. The Musician will receive notice of a special event performance no less than 30 days in advance and will have right of refusal for short-notice events (e.g., memorial service music). Such events will constitute one

(1) unit of performance.

11.6 Coordinate, with the Music Coordinator, the F. E. Warren AFB Chapel Facility Request Form (local form) to schedule the use of chapel facilities for all activities within the facilities. Ensure that all facilities used are properly maintained and secured upon departure.

12.0 Religious Position and Title. Protestant Contemporary Service Music Coordinator

12.1 Scope of Responsibilities. The Music Coordinator shall provide, under the direction of the service Chaplain, direction of the Protestant Contemporary Service music program to include direction of congregational singing, choir/band recruitment, rehearsal, performance, and special music presentations.

12.2 Education and Experience Requirement. The Music Coordinator must:

12.2.1 Be an accomplished musician with solo and accompaniment experience in a broad range of music styles and traditions that include classical, contemporary, gospel, and traditional music. Experience in church music, education, and background in vocal and instrumental music instruction, hymnology, traditional religious music, and contemporary religious music is desirable.

12.2.2 Have 2 years of experience leading contemporary Protestant style worship (similar to Hill song and Vineyard style) music including experience with recruiting, instructing and conducting volunteer contemporary praise bands and vocalists.

12.2.3 Have experience providing musical ministry under direction and oversight of a presiding Pastor and/or Chaplain and coordinating music with long and short term pastoral ministry goals.

12.2.4 Furnish resume including academic and work related references (i.e. military chapels, churches).

12.3 General Responsibilities. The Music Coordinator shall:

12.3.1 Coordinate with the service Chaplain for the selection of proper music for the week's services to ensure that musical selections are appropriate to the theme, season of the church year, and/or purpose of each service.

12.3.2 Recruit volunteers for the Praise Team (vocalist and musicians).

Rehearsals will last at least one (1) hour, but no more than two (2) hours.

13.4.3 Provide weekly performances for the Protestant Contemporary worship service.
