SUBSTATION MAINTENANCE

TECHNICAL RESPONSE QUESTIONNAIRE PART 2 (Factors 2, 3, and 4) Source Selection Sensitive (when filled-out) IAW FAR 2.101 and FAR 3.104

Instructions: Please read each factor carefully and provide your response/submission accordingly. NO RESUMES PLEASE. [Reference Addendum to FAR 52.212-1, Paragraph C.1.(b)]

Factor 2: Position Descriptions. Provide job titles (NOT names of individuals/resumes), description of job duties, minimum experience, and training qualifications required for each labor category that your company proposes to use to fulfill the maintenance requirements on each substation component (as required by Paragraph 8.3 of the PWS).

To Be Considered Technically Acceptable for Factor 2: The contractor’s responses indicate that for each staff member whom the contractor proposes to assign to perform maintenance on each substation component (as required by Paragraph 8.3 of the PWS), that staff membermust: (1) possesses a minimum of five (5) years of substation maintenance experience for that PWS Paragraph 8.3 substation component within the last five (5) years preceding the release of this request for proposal; (2) has completed specific substation maintenance training for that PWS Paragraph 8.3 substation component; and (3) has completed NFPA 70 safety training.

Note: You may add or alter the number of “Job #” descriptions below as needed. If additional pages are necessary the attached pages shall clearly identify the Job#, the PWS Paragraph 8.3 Component, and the Job Title to which they are related.

Factor 3: Access to degreed Electrical Engineer with a Professional License as an Electrical Engineer registered in the State of California and a minimum of five (5) years of substation maintenance experience within the last five (5) years preceding the release of this request for proposal. Provide name and years of experience below and attach a copy of the license with this completed questionnaire.

To Be Considered Technically Acceptable for Factor 3: The contractor’s response indicates the contractor has access to a degreed Electrical Engineer with a Professional License registered in the State of California and who has a minimum of five (5) years of substation maintenance experience within the last five (5) years preceding the release of this request for proposal in accordance with the PWS, Paragraph 8.3, Contactor Personnel Requirements. Further, the contractor must provide a copy of a valid, active, and current license.

Factor 4: Teaming Arrangements Information. If your company is not performing ALL the work, you must explain the contemplated teaming arrangement. This includes, but is not limited to, the following types of teaming relationships: prime/sub-contractor, mentor/protégé, joint venture, etc. If a teaming arrangement is not applicable and ALL the work will be performed by your company as the prime contractor, please indicate N/A as your response.

To Be Considered Technically Acceptable for Factor 4: The contractor’s response provides sufficient information to determine whether or not the contactor contemplates a teaming arrangement and, if so, indicates the name of each teaming partner (i.e. subcontractor), identifies the tasks and the percentage of work to be performed by each party (i.e. prime and subcontractor), and provides sufficient information for the Government to determine that the offeror as a whole (i.e. prime/sub combined) has the ability to perform the requirements of this solicitation.
