PERFORMANCE WORK STATEMENT
DEAD BIRDS & BIRD WASTE REMOVAL
Altus AFB
Combined PWS for Hangar 285, Hangar 435, Hangar 509, Hangar 514 and Hangar 517
1.0 Description of Services. Contractor is responsible for all personnel, supplies, materials, tools, labor and equipment to perform the cleaning of aircraft hangars: Hangar 285; Hangar 435; Hangar 509; Hangar 514; and Hangar 517, Altus AFB OK.
1.1 The listed Hangars above will be cleaned according to the schedule listed below. If a holiday falls on the scheduled day, the contractor must make arrangements to come the day before the holiday.
1.1.2. Period of Performance is February through January:
1.2. Contractor must remove and dispose of off-base, all bird waste, debris, and dead birds IAW all Federal and State regulations. The contractor will clean the designated areas only during the days that have been established by this contract.
1.3. Aircraft Hangar Descriptions and Cleaning Schedule:
1.3.1. Bldg 285:
1. . Hangar 285 is to be cleaned once a month on the Third Thursday and Friday except during the months April through October when cleaning will be accomplished on the First and Third Thursday and Friday of each month. Cleaning instructions listed below.
a. Clean the full-length of north and south hangar door tracks extending 20 feet interior and exterior of the threshold and the interior and exterior hangar door pockets located on the northwest, northeast, southeast, and southwest corners of hangar.
b. During the months of May and September clean the north and south main hangar doors from top to bottom all ledges, horizontal surfaces and remove visible debris to include cob webs from vertical surfaces and interior.
c. During the month of September clean the tops of the mezzanines located above the Inspection Flight offices, Tool Room and the Hydraulic Shop
d. During the months of May and September vacuum the tops of the 45 degree slopes on the northwest (adjacent to the left wing of the C-17 inspection bay) and southwest (directly in front of the KC-135 nose spot of the KC-135 inspection bay) side of the C-17 second floor outside the office area.
e. During the months of January and July clean the space behind the C-17 office area east stairwell, adjacent to the CE electrical and air handling area located on the east wall of the hanger. Also clean CE utility area adjacent to CTK caged storage in the NE corner of the hangar
1.3.2. Bldg 435:
1. Hangar 435 is to be cleaned once a month on the Third Thursday and Friday except
First and Third Thursday and Friday of each month. Cleaning instructions listed below
a. Clean perimeter floor surface from north and south walls out to first yellow line on floor.
b. Clean hangar door tracks.
c. Clean all horizontal surfaces in the doors and remove all visible debris from vertical surfaces on door interior to top of doors.
d. Clean and wash the drains on both ends of hangar.
e. Tops and ledges on sides of all six heaters exercising due diligence in care of heater.
1.3.3. Bldg 509:
1. Hangar 509 is to be cleaned once a month on the Third Thursday and Friday except
a. Clean bottom ledges on east and west side walls, and all ledges on hangar doors bottom.
b. Clean tracks for full length on north door.
c. Clean the perimeter floor east and west sides; clean 23 feet out beginning at the walls around hangar frame structures and fixtures.
1.3.4. Bldg 514:
1. Hangar 514 is to be cleaned once a month on the Third Thursday and Friday except
a. Clean tracks on east side of hangar for full length.
b. Clean 3 feet inside/outside of door closures area.
c. Clean the bottom ledges on north and south sidewalls, and all ledges on hangar doors.
d. Clean 5 feet inside/outside of roll up door closure area west side of hangar
1.3.5. Bldg 517:
1. Hangar 517 is to be cleaned once a month on the Third Thursday and Friday except
e. Clean tracks on east side of hangar for full length.
f. Clean 3 feet inside/outside of door closures area.
g. Clean the bottom ledges on north and south sidewalls, and all ledges on hangar doors.
h. Clean the perimeter floor north and south sides clean 23 feet out beginning at the walls around hangar frame structures and fixtures.
2.0. Deficiencies. The preferred method of resolving contract deficiencies is to allow for re-performance to correct the deficiency. The contractor will be given the opportunity to correct substandard performance on a day as agreed upon by the Facility Manager/Supervisors. If the substandard performance is not corrected within the specified time lines or the contractor refuses to re-perform the service the unsatisfactory service is then determined to be deficient.
2.1. Deducts: Deducts are calculated using the following formula:
Hangar 285. If an area is determined to be deficient by the inspector, 14.3% may be deducted from the contract price for each item of non-performance.
Hangar 435. If an area is determined to be deficient by the inspector, 25% may be deducted from the contract price for each item of non-performance.
Hangar 509. If an area is determined to be deficient by the inspector, 33.3% may be deducted from the contract price for each item of non-performance.
Hangar 514. If an area is determined to be deficient by the inspector, 25% may be deducted from the contract price for each item of non-performance.
Hangar 517. If an area is determined to be deficient by the inspector, 25% may be deducted from the contract price for each item of non-performance.
2.2. If the contractor is kept from cleaning any of the designated areas, the contractor will not be held responsible for the cleaning of the area, and will receive the monies dedicated for that purpose.
3.0. General Contract Management.
3.1. Employees must be eligible for clearance badges for on-base access.
3.2. Contractor must check in with the POC, Facility Manager/Supervisors for each hangar prior to performance. Coordinate with supervisors to move equipment, work benches and cabinets from inside walls to clean area properly. The inspector must be contacted before the contractor leaves the area, to make sure all areas have been cleaned satisfactorily. POC for Hangar 285 is Louis Baird at 580-649-1396; POC for Hangar 435 is Eugene Tull 580-649-1991; POC for Hangar 509. POC for Hangar 514 is Michael Runyon at 580-481-7678; and 517 is Chuck Bolton at 580-481-7964; alternate Dean Westover at 580-481-6716.
4.0. Pass and Identification Items. The Contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements:
a. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, and last name, social security number, date of birth, address, driver’s license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus
AFB.
b. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting Office within 30 days of leaving.
c. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract.
Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.
d. Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.
5.0. Antiterrorism Clause:
5.1. Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base.

6.0. IP-Security Clause:

7.0. Environmental Statement:
7.1. Contractors must comply with all environmental requirements to include completion of initial Environmental Management Systems (EMS) awareness level training within 10 days of contract award or with a new contract employee beginning work on the Air Force Installation and the appropriate documentation of training completion is to be maintained.

All chemicals brought onto Altus AFB must be reviewed through the Environmental, Safety and Occupational Health Offices prior to use on the installation.

8.0. Contractor personnel shall present a neat and clean appearance and be easily recognized as contractor employees. The contractor shall provide contractor personnel working at Altus AFB with a method of identifying the company name, employee’s first and last name, and job title. This may be accomplished by a name badge or a company shirt. Employees must wear identification at all times.
9.0. Contractor personnel shall present professional conduct at all times while on Altus AFB. Loud volumes and use of profanity are strictly prohibited.
