PERFORMANCE WORK STATEMENT

FOR

INSPECTION / MAINTENANCE / REPAIR / TESTING / RECHARGING

PORTABLE AND WHEELED FIRE EXTINGUISHERS

ON PETERSON AFB COLORADO

3 February 2017

SECTION 1

DESCRIPTION OF SERVICES

1.1 Scope of Work: Furnish all personnel, equipment, tools, materials, supplies, transportations and labor necessary to perform inspection, maintenance, and repair and recharge portable and wheeled fire extinguishers as defined in this Performance Work Statement (PWS). Conform to all federal, state, local requirements, the Air Force Occupational Safety and Health Standard (AFOSHSTD), Occupational Safety and Health Administration (OSHA) and applicable manufacture recommendations as required in the PWS. Work must be performed in accordance with (IAW) the current National Fire Protection Association (NFPA) 10 standards, “Portable Fire Extinguishers, for the inspection and maintenance of all types of portable and wheeled fire extinguishers that are on Peterson Air Force Base (AFB), Colorado. The United States Air Force shall provide all Halon 1211 for Flightline Fire Extinguishers.

1.2 Operational Requirements and/or Specific Tasks:

1.2.1 Scheduling. The Contractor shall coordinate with the Fire Department for a list of buildings and a list of points of contact. The contractor shall be notified by the Fire Department before commencing any inspection/ maintenance. At this time the contractor shall review with the Fire Department, the schedule of inspections for all portable and wheeled fire extinguishers at the facilities. The contractor shall be available on base a minimum of one day every other week to service previously identified unserviceable extinguishers. Contractor shall be escorted into controlled areas, if necessary, by facility/building managers.

1.2.2 Inspections. Inspect and perform all annual, 5-year, 6-year maintenance, and 12-year hydro testing, refilling and recharging of all portable and wheeled-type fire extinguishers IAW Section V-Technical Exhibit (TE) 1 of this PWS and the fire department coordinated list. Tag each extinguisher showing at a minimum: name of company and technician inspecting, date of inspection, and any repairs that were required. Contractor is not responsible for inspections of extinguishers mounted on vehicles, equipment, or aircraft. If any extinguishers are missing or appear to have been discharged, immediately notify the Fire Department at (719) 556-7354 or the Fire Department at (719) 556-6574.

1.3 Corrective Maintenance and Repair: Fire Extinguishers need to be operational at all times. Corrective maintenance returns a fire extinguisher to full operating capacity as described below. The contractor shall pick-up any extinguishers that have been identified by Peterson Fire Department as needing repair/recharge within 48 hours of notification. PARTS REPAIR. Contractor shall provide all parts and materials for repairs up to a cap of $1,500.00 annually.

1.3.1 Annual Fire Extinguishers. The contractor shall inspect IAW NFPA 10 and repair as necessary all extinguishers listed in Section V-TE-1of this PWS.

1.3.2 For unserviceable extinguishers, the contractor shall verify any unserviceable extinguisher with the fire department. The contractor shall dispose of any extinguisher deemed unserviceable by the fire department IAW NFPA10, Section V, TE-2. The contractor is not responsible for permanent replacement of extinguishers that are unserviceable due to age, corrosion, etc.

1.3.3 The 5-year and 6-year Fire Extinguishers. The contractor shall perform overhaul maintenance, repair, and recharging IAW the manufacturer’s recommendations for each type of extinguisher IAW Section V- TE-1. Each 5-year and 6-year maintenance shall be recorded on a record sticker, tag or plate, which shall be affixed on the exterior of the extinguisher shell. The 5-year and 6-year maintenance of each extinguisher, shall include the replacement of parts such as, but is not limited to, service collars, o-rings, valve stems and inspection tags.

1.3.4 The 12-year Halon 1211 Fire Extinguishers. The contractor shall maintain and repair all hydrostatic testing, reclamation, and transferring of 150 lb Halon 1211 extinguishers to and from the flight line as listed on Section V, TE-1. A hydrostatic test of the extinguishers as specified in the manufacture’s recommendations. The 12-year maintenance of each extinguisher, shall include the replacement of parts such as, but is not limited to, service collars, o-rings, valve stems and inspection tags.

1.3.4.1 Recharging 5-year and 6-year Fire Extinguishers and 12-year Halon extinguishers:

1.3.4.1.1 The amount of recharge agent shall be verified by weighing. The recharge gross weight shall be the same as the gross weight that is marked on the extinguisher label.

1.3.4.1.2 For 5-year and 6-year maintenance or 12-year hydrostatic testing, extinguishers shall be emptied. The dry chemical may be reused provided a closed system is used for emptying and the agent is stored in a sealed container to prevent contamination. A temporary extinguisher (of same type) shall be provided by the contractor when any extinguisher is removed from the facility/area for service. Record tests and frequency of tests IAW NFPA 10, Section V, TE-2, shall be completed IAW Paragraph 1.5 of the PWS.

1.3.4.1.3 Halon 1211 is classified as a Class I Ozone Depleting Chemical. The Government shall provide, as government-furnished material (GFM), all halon required for extinguisher maintenance. The contractor is not allowed to provide halon from its own stock. This contract does not authorize the purchase of halon to charge Air Force fire extinguishers. All halon provided to the contractor in extinguishers or as GFM for extinguisher maintenance shall remain the property of the U.S. Government. Furthermore, this contract does not authorize the contractor to dispose of or claim any “government-owned” Halon 1211.

1.3.4.1.3.1. On a regular basis (at a minimum with each invoice) and at the conclusion of the performance of the contract, the contractor shall provide reports that demonstrate the tracking of halon to the fire department.

1.3.4.1.3.2. The contractor shall return to the Fire Department all unused or excess halon. The contractor shall not dispose of any U.S. government-owned halon.

1.4 Record Keeping and Documentation. The contractor shall maintain the monthly Inventory Record on all extinguishers, provided by the government or other automated product that meets the requirement listed in 1.4.1. The inventory record consist of a computer-generated listing of: facility showing each extinguisher type, size, location, serial number and last inspection date. The contractor shall maintain continuous update of this Inventory Record to include the due date of the next 5-year and 6-year maintenance, and the 12-year hydrostatic tests of each extinguisher and the correct count of the extinguisher at each location. A copy of this inventory record shall be sent via emailed to the 21st Space Wing Fire Department fire department, the same day as invoicing in Wide Area Work Flow (WAWF).

1.4.1. The Inventory Record shall list all halon fire extinguishers for which the contractor is responsible for and shall be readily available for audit or query. The contractor’s maintenance schedule shall be based upon the Inventory Record. The Inventory Record shall be a columnar report, in a comma-delimited text file, electronic spreadsheet file, or other computer-readable electronic format, and list the following minimum data for each extinguisher.

•Type of extinguisher (to include manufacturer’s name, if necessary for distinction)
•Serial number or tracking number of the extinguisher
•Date added to contractor’s responsible inventory or date of initial fill
•Date of last monthly inspection
•Date of last annual service maintenance
•Date of last six-year complete maintenance
•Date of last hydrostatic testing

· Undergoing major overhaul/refurbishment

• Date prepared for salvage or turned-in to government control *

* A date in this field indicates that the extinguisher has been removed from the contractor’s responsible inventory.

1.5 Reporting. The contractor shall provide two periodic reports (the Service Report and the Halon Report) to the fire department.

1.5.1 The Service Report, which is included in the inventory records, shall accompany the invoice and shall itemize all services, the contractor performed during the month. This report shall list the following data for each extinguisher serviced during the reporting/invoice period.

· Date

· Tracking number or serial number of the extinguisher (from inventory record)

· Type of Service (e.g. initial fill, annual service maintenance, six-year complete maintenance, hydrostatic testing, corrective maintenance/recharging, refurbishment, transfer to government, and salvage preparation)

· Quantity of halon (in pounds) recovered from extinguisher prior to servicing (or quantity remaining in extinguisher following salvage preparation and/or transfer of the extinguisher to government control)

· Quantity of halon (in pounds) used to recharge extinguisher

· Amount returned of halon (in pounds) returned to [state to whom]

1.5.2 The Halon Report shall be accomplished twice a year (30 June and 31 December) and forward to the fire department. The Halon report shall provide the following summary data points in a text file, electronic spreadsheet file, or other computer-readable electronic format:

· Date

· Total quantity (in pounds) of GFM halon received by the contractor from the government during the reporting/invoice period

· Total quantity (in pounds) of GFM halon under the contractor’s control as “bench stock” at the end of the reporting/invoice period

· Total quantity (in pounds) of unserviceable GFM halon accumulated in recovery cylinders under the contractor’s control, awaiting turn-in to the government at the end of the reporting/invoice period

· Total quantity (in pounds) of halon turned-in by the contractor to the government during the reporting/invoice period-30 June or 31 December of each year.

SECTION II

SERVICE SUMMARY

The Service Summary is a list of performance objectives and performance thresholds that represents the minimum acceptable level of performance. The performance objectives and thresholds shall be regularly verified by the fire department. Each performance objective represents a significant task as identified by the Government at the time of contract award. These performance objectives and performance thresholds represent only the significant task of this contract and do not excuse the Contractor from performance of other responsibilities identified in this PWS and Section V, Technical Exhibits (TE). Additionally, performance objectives may be added or deleted and performance thresholds raised or lowered during the course of the contract by a modification to the contract.

Fire Extinguishers are properly inspected and serviced according to frequency recommended by Manufacturer and/or NFPA 10.

Fire Extinguishers are properly tested and recharged is performed IAW manufacturer’s recommendation and NFPA 10 standard.

SECTION III

GOVERNMENT-FURNISHED PROPERTY AND SERVICES

3.1 Government-Furnished Property (GFP). The government will furnish all Fire Extinguishers, Halon, an area to work in, electricity, water, and sewage service necessary to accomplish the work IAW this contract. The contractor shall adhere to all base level utility conservation practices. The contractor shall be responsible for operating under conditions that prevent waste of utilities.

SECTION IV

GENERAL INFORMATION

4.1 Hours of Operation: Perform the inspection, pick-up or delivery of extinguishers during the hours of 7:00 a.m. – 4:00 p.m, Monday through Friday, according to the pre-coordinated schedule, mentioned under Section 1, paragraph 1.2.1.

The contractor is not required to provide service on the following days:

4.2 Hazardous Waste/Materials:

4.2.1 Hazardous Waste. All hazardous waste generated from contract activities shall be disposed of according to all federal and state regulations. The contractor shall make correct waste determinations for each waste stream. The contractor shall ensure proper waste packing, labeling, hazardous waste manifesting, and transport from accumulation site to ultimate disposal. Ensure the waste goes to a properly permitted treatment/disposal facility and that all manifests and any other required documentation are available to the Peterson AFB Environmental Flight before submittal of invoice. Pay all costs associated with hazardous waste management and disposal. Any and all fines/penalties assessed by state, local or federal agencies for hazardous waste laws violated shall be paid by the contractor. Contact the 21st Space Wing (SW) Environmental Flight immediately at (719) 556-7328 upon hazardous waste generation.

4.2.2 Hazardous Materials. In the accomplishment of this contract, non-flammable, environmentally friendly, biodegradable products IAW Air Force Instruction (AFI) AFI 32-7086, Hazardous Materials Management, Chapter 2.5.5, found in Section V-TE-2 must be used. Submit AF Form 3952; “Chemical/ Hazardous Material Request Authorization” and the manufacturer’s “Material Safety Data Sheet (MSDS)”, to the base HAZMART, Building 652, for all chemical used on Peterson AFB within two (2) weeks of contract award and within two (2) weeks after any new chemicals are purchased for use under this contract. No chemicals shall be used on Peterson AFB prior to obtaining this approval. No chemicals shall be stored on Peterson AFB. The contractor shall contain, handle, and cleanup all spills from this operation. Cost to dispose of spilled material and any other waste items resulting from a spill is the responsibility of the contractor. All spills, leaks and inadvertent release of hazardous materials shall immediately be reported to the Fire Department COR at (719) 556-6574 or 556-7354.

4.3 Contractor Qualification/Experience: For the life of the contract, a trained and certified person(s) who have undergone the factory training and certification for the specific type and brand of portable and wheeled-type fire extinguishers being to reliably perform inspections; or certification by an organization acceptable to the authority having jurisdiction; or registration, license or certification by the state or local authority having jurisdiction; and IAW NFPA 10, to maintain, service, and recharge all portable and wheeled fire extinguishers. Provide certificate(s) upon request. The certified person(s) shall have available the appropriate manufacture’s service manual(s), the proper types of tools, recharge materials, lubricants, and manufacturer’s recommended replacement parts or parts specifically listed for use in the fire extinguisher and IAW with NFPA 10.

4.3.1 Contractor Employees. Do not employ persons for work on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security or operational mission of the installation and its population.

4.3.2 Present a neat appearance and be easily recognized as Contractor employees. Identification of all employees shall be accomplished by wearing badges and by Contractor-provided distinctive clothing bearing the name of the Contractor. A Government-issued identification badge shall be worn visibly at all times.

4.4 Security Requirements for Peterson AFB: Comply with all Peterson AFB security requirements imposed by the installation commander at all times while on the premises to include not using cell phones while operating a vehicle on base. Obtain necessary badge(s) required for access to Peterson AFB.

4.4.1 Badge Request. Request, on company stationery including address, telephone number, and contract number a list of employees, with the full legal name, social security number, date of birth and place of birth requiring access to Peterson AFB. Each contractor employee must obtain the necessary badge within the 10 day period. When a badge is requested for a new employee an updated employee list must be sent to the Contracting Office.

4.4.1.1 Contractor employees shall apply for individual identification badges at the Peterson AFB Visitor’s Control Center; Building 1334 upon employment after access letter is on file with security forces. Badges are processed Monday through Friday from 6:00 a.m. to 2:30 p.m. No appointment is necessary. Upon completion or termination of the contract or termination of a contractor employee, surrender identification badge(s), to their contract manager who in-turn shall surrender the badge(s) to the Contracting Office Security Forces the following workday.

4.4.2 Vehicle Requirements Proper state vehicle registration, proof of insurance and a valid driver’s license are required to drive on Peterson AFB. All commercial vehicles must enter through the East Gate (Marksheffel Road), the primary search area for all commercial delivery vehicles.

4.5 Quality Assurance: The government shall evaluate the contractor’s performance under this contract through on-site inspections and customer complaints. Fire inspectors shall routinely perform quality checks during their scheduled and unscheduled building inspections.

4.6 Performance of Services During Crisis Declared by the National Command Authority or Overseas Combatant Commander. In accordance with Department of Defense (DoD) Instruction (DoDI) 3020.37, paragraph E2.1.1., (See Section V-TE-2) a crisis situation is defined as: Any emergency so declared by the National Command Authority or the overseas Combatant Commander, whether or not U.S. Armed Forced are involved, minimally encompassing civil unrest or insurrection, civil war, civil disorder, terrorism, hostilities buildup, wartime conditions, disasters or international conflict presenting a serious threat to DoD interest. It is determined that the contractor is not required to perform services under this contract during a crisis unless otherwise directed by an authorized government representative.

4.7 "The Contractor shall report all Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the 21st Contracting Squadron, PAFB, CO 80914 via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk"
