About this file
This document is an Exhibit M DES ESG Program Performance Report issued by the Arizona Department of Economic Security (DES), specifically a quarterly and annual reporting template for Emergency Solutions Grant (ESG) funded homeless services programs. The document involves multiple Continuum of Care (CoC) regions including Maricopa, TPCH (Tucson Pima Collaborative), and Balance of State (BOS), with performance reporting requirements for various homeless service activities such as Emergency Shelter, Street Outreach, Rapid Re-Housing, and Homeless Prevention.
The performance report requires providers to track and report key metrics including persons served, estimated service rates, bed utilization, income exit rates, housing destination outcomes, data quality, length of participation, and percentage of grant award expended. The template is designed for quarterly (Q1-Q4) and annual reporting, with providers expected to extract data from their Homeless Management Information System (HMIS) or comparable databases. Performance goals are standardized across activities, with targets such as 60-90% for various outcome metrics, 95% data quality, and 25% grant expenditure per quarter, indicating a structured approach to monitoring and evaluating homeless service program effectiveness.
