About this file
This is a Respondent Reference Form (Exhibit G) for Solicitation No. HHS0017704 issued by the Texas Health and Human Services (HHS) Procurement and Contracting Services department for the Quetech Annual Support Plan (Annual Server Hosting Service). The form requires all respondents to submit a minimum of three business references demonstrating their ability to perform the scope of work outlined in the invitation for bids. References must be from services provided within the two years preceding the submission and should include contract dates, customer points of contact with complete contact information (address, telephone number, and email address), and detailed descriptions of services rendered.
Respondents must clearly indicate whether work was performed as a prime contractor or subcontractor on each reference. In cases where work was performed as a subcontractor, respondents are required to provide a comprehensive description of the scope of subcontracted activities. The form provides space for up to four business references, with each entry requiring the business name, point of contact title, phone number, email address, and a detailed description of services provided. This documentation requirement ensures the evaluation committee can verify the respondent's qualifications and relevant experience prior to contract award.
