About this file

This document is a Child Specific Recruitment (CSR) Closure Report Guidelines form issued by the Arizona Department of Child Safety (DCS), specifically document DCS-1227D dated April 2020. The guidelines outline the procedures and requirements for closing child adoption and guardianship cases, with a focus on comprehensive documentation and recruitment efforts.

The document provides detailed criteria for case closure, including mandatory actions such as ensuring children have professional photos on the Children's Heart Gallery for at least 6 months, responding to all family inquiries, exploring barriers with foster families, conducting file investigations of biological and significant relationships, and addressing youth's potential reluctance to be adopted. Specific scenarios that may trigger case closure include prolonged family-like placements, imminent aging out of the system, out-of-state moves, early caregiver identification, and existing connections to support services like Fostering Sustainable Connections or Casey Family Services. The guidelines require thorough documentation and potentially require approval from the Assistant Director of Foster Care for case closures not related to guardianship or adoption.
