Grant applicants must complete and submit this certification as part of their response to the RFP. The document certifies that the applicant organization has not been suspended or debarred from doing business with the State of Minnesota or the federal government, as required under the Office of Grants Management Policy 08-04 regarding Grant Contract Agreements and Grant Award Notifications. The certification requires the applicant to print their name, provide a signature, state their title, and include the submission date. The RFP covers funding allocated by the 2025 Minnesota Legislature for waste reduction, reuse, recycling, and composting grants in Greater Minnesota under Minn. Stat. 115A.565, with the funding period running from July 1, 2026, through June 30, 2029, and all projects must be completed by June 30, 2029 with no extensions available.

Applicants must warrant that their organization meets the suspension and debarment requirements by signing this certification. The submission of inaccurate or misleading information on this certification may result in disqualification from the grant award, and may subject the applicant and their organization to suspension or debarment proceedings as well as other legal remedies available to the State of Minnesota. The grants are intended to fund projects focusing on best practices for waste reduction, reuse, recycling, or composting collection systems as defined under Minn. Stat. 115A.03.
