About this file

This document is a Government-Wide Debarment and Suspension (Nonprocurement) certification form from Invitation No. 590-26, executed by Vital Assurance Ltd. The form is designed to verify that contractors and subcontractors entering into covered transactions are not excluded or disqualified from participating in federal, state, or local government transactions. The certification requires the contractor to confirm their eligibility by checking the System for Award Management (SAM) exclusions and attesting to various legal and ethical standards regarding their business practices.

The document does not specify the specific goods or services under contract, but it outlines comprehensive compliance requirements for lower-tier contracts exceeding $25,000 or requiring federal official consent. The certification requires Vital Assurance Ltd to ensure that its principals and first-tier subrecipients have not been debarred, suspended, or otherwise disqualified from federal transactions, and that they have not been convicted of fraud, antitrust violations, or other criminal offenses within the past three years. The signed certification by Ryan McKinley is dated May 19, 2025, indicating the document's validity and the company's commitment to maintaining ethical standards in government contracting.
