About this file
This document is Exhibit C from the Arizona Department of Child Safety's Office of Procurement, detailing the Unusual/Significant Incident Report Protocol for ADJC (Arizona Department of Juvenile Corrections) Service Providers. The protocol applies to juveniles under ADJC jurisdiction receiving services funded by ADJC, DCS (Department of Child Safety), T/RBHA (Tribal/Regional Behavioral Health Authority), inter-state, or private pay sources.
The document outlines comprehensive reporting requirements for service providers when incidents occur involving ADJC juveniles. Service providers must make verbal reports within two hours of an incident during business hours (8:00 AM - 5:00 PM Monday-Friday) to the assigned Parole Officer and Community Services Office, or to the After Hours Administrator during non-business hours. Written reports must be submitted within 24 hours via fax or email, including detailed information such as the juvenile's name, incident description, environmental factors, and follow-up actions. The protocol defines 17 types of reportable incidents, ranging from serious injuries and restraint use to law enforcement contacts, substance use, and other potentially newsworthy or sensitive events.
