About this file
This is a data template and reporting document for the Proposition 47 Cohort 5 Grant program administered by the County of Tuolumne, California, in coordination with the Board of State Community Corrections (BSCC). The template serves as the primary data collection and reporting mechanism for tracking participants enrolled in programming services designed to serve justice-involved individuals with substance use and co-occurring mental health disorders, particularly those eligible under Proposition 36. The document functions as both a participant tracking spreadsheet and a comprehensive data dictionary, establishing standardized definitions, field formats, and reporting requirements for all data elements. Organizations selected to provide case management, risk/needs assessments, outpatient substance use disorder treatment, employment and education programming, and community navigation services must complete this template on a quarterly basis for a minimum of 13 quarters, tracking up to approximately 100 or more individual participants per reporting period.
The template requires grantees to report extensive demographic, clinical, and service delivery data for each participant, including enrollment and exit information, employment and housing status at program entry and completion, mental health and substance use treatment modalities, and quarterly assessments of participant motivation to change. Data reporting must be submitted quarterly for all enrolled participants, with one-time intervention participants tracked separately. The Grantee Code assigned by BSCC (a two-letter identifier such as "DZ" for Tuolumne County) must precede all participant identification numbers to ensure no duplication across grantees. Participants are tracked across 13 consecutive quarters of service delivery, with mandatory reporting fields for all active participants and completion status required only when participants successfully complete program requirements. The template includes dropdown menus and standardized response options for consistency across all grantee reporting, ensuring data integrity and comparability across the Proposition 47 Cohort 5 initiative statewide.
