About this file

This is an Exhibit A document establishing data collection, reporting, and evaluation requirements for Community-Based Organizations (CBOs) receiving Proposition 47 Cohort 5 funds through the Tuolumne County Probation Department in California. The Tuolumne County Probation Department is soliciting proposals from qualified organizations to provide case management, risk/needs assessments, outpatient substance use disorder (SUD) treatment, employment and education programming, and community navigation services. Services are designed to serve justice-involved individuals eligible under Proposition 36 who present with chronic substance use, co-occurring mental health disorders, and repeated criminal justice involvement. The selected Contractor will be responsible for timely assessment, appropriate level-of-care placement, and ongoing treatment services in alignment with participant needs while collaborating with the Superior Court, Probation Department, Behavioral Health, and community stakeholders.

All subcontracted CBOs must comply with comprehensive data collection and reporting requirements established by the Board of State and Community Corrections (BSCC) and the Proposition 47 Cohort 5 Request for Proposals. Contractors are required to collect and maintain participant-level data including enrollment demographics, service delivery information, and participant outcomes at minimum. Data must be submitted to the Lead Agency no less than quarterly to support Quarterly Progress Reports (QPRs), Local Evaluation Plan (LEP) reporting, and reimbursement documentation. The grant operates on a reimbursement basis, and failure to provide timely documentation may delay payment. Contractors must maintain secure data systems in compliance with HIPAA and 42 CFR Part 2, retain all financial and programmatic records according to BSCC Grant Administration Guide requirements, and participate in data review meetings and evaluation activities. Non-compliance with data collection and reporting requirements may result in withholding of reimbursement, corrective action, contract modification, or termination.
