About this file
This is an Additional Provisions exhibit for a Purchase Order issued by the Colorado Department of Early Childhood (CDEC) for early intervention evaluation services. The document establishes the operational framework for service delivery under a fixed-price Purchase Order, requiring the Grantee to provide services according to the Statement of Work (Exhibit A) with all task descriptions, timetables, and responsibilities outlined therein. The Grantee is responsible for achieving specified goals and objectives unless written modifications are provided by the State. Any executed subcontracts must be provided to CDEC upon completion, and the Grantee must participate in CDEC-sponsored meetings related to the Purchase Order.
Payment is structured on a fixed-price basis submitted through the OnBase Invoice Submission Process no less than monthly, with invoices due no later than 30 days following the last day of the month and compressed timelines at fiscal year-end (invoices for services prior to June 30th must be submitted by July 7th, with final invoices due by September 14th). The Purchase Order amount is not to exceed the specified budget amount, and any costs exceeding this amount will not be paid by CDEC; Grantees seeking additional funds must request them 60 days prior to projected depletion and obtain written approval through an official modification. The Grantee must maintain source documentation supporting all payment requests and comply with mandatory reporting requirements for child abuse and neglect, critical incident reporting within 48 hours to the CDEC Program Manager, accessibility standards per 24-85-101 C.R.S. and WCAG Level AA compliance, and prohibitions on purchasing gift cards with grant funds. The State reserves the right to offset funds based on discovery of overpayment or improper use.
