About this file

This document is an incident reporting guide created by the State of Arizona's Department of Child Safety, specifically from the Office of Procurement located in Phoenix. The document provides a comprehensive matrix detailing various types of incidents that require either an incident report submission or a call to the hotline, specifically for child safety and care settings.

The matrix covers 24 distinct incident types, including serious events such as individual deaths, suicide-related incidents, physical and sexual abuse allegations, medical emergencies, injuries, restraint usage, child arrests, runaways, and other critical incidents. The guide systematically indicates which incidents necessitate an incident report, a hotline call, or both, providing clear guidance for staff in child care and treatment facilities to ensure proper reporting and immediate response to potentially critical situations involving children under care.
