About this file

This document is a Statement of Special Inspections for the Hillcrest Middle School project located at 530 Daniels Farm Road, Trumbull, Connecticut, prepared by the Town of Trumbull with structural engineer of record Michael Horton Associates, Inc. The Statement of Special Inspections outlines the comprehensive quality assurance and material testing services required for the construction of the new middle school facility. The special inspection program encompasses multiple building systems including soils and foundations, cast-in-place concrete, masonry, structural steel, cold-formed steel framing, spray fire-resistant materials, precast concrete, exterior insulation and finish systems, mechanical and electrical systems, and architectural systems. The Special Inspection Coordinator will be responsible for maintaining records of all inspections and furnishing interim monthly reports and a final report to the Building Official and Structural Engineer of Record prior to issuance of a Certificate of Use and Occupancy. Interim reports shall be submitted on a monthly basis or per an attached schedule, with the inspection program document prepared by design professional Paul J. Sheehan, P.E., dated April 1, 2026.

The inspection and testing agencies identified include Inspector Alfred Jones from Langan (ajones@langan.com, 617-824-9122, located at 555 Long Wharf Drive, New Haven, Connecticut 06511), with the Special Inspection Coordinator and additional testing agencies designated as "TBD" pending further assignment. All inspectors and testing agencies must be engaged by the Owner or Owner's Agent rather than by the Contractor or subcontractors performing the work, with any conflicts of interest requiring disclosure to the Building Official prior to commencing work. Inspector qualifications are subject to Building Official approval and must meet specific certification requirements including ACI certifications for concrete work, AWS certifications for welding inspection, ICC certifications for masonry and structural steel, and NICET certifications for soils and concrete technicians. The project has been designated as Seismic Design Category B and Wind Exposure Category C with a basic wind speed of 130 mph (3-second gust), though quality assurance plans are not required for seismic or wind resistance as steel systems are not specifically detailed for these requirements.
