About this file
This Statement of Work details preventative maintenance and repair services for a Reverse Osmosis/Deionization (RO/DI) water system required by the Texas Department of State Health Services (DSHS) Laboratory located at Dr. Bob Glaze Laboratory Services Section Building in Austin, Texas. The scope of work encompasses routine preventative maintenance performed on a monthly, quarterly, biennial, and annual basis, including service visits with system data point recording, RO pre-filter changes, replacement of final filters, UV lamp replacement, quartz sleeve replacement, o-ring replacement, resistivity meter calibration, and carbon rebed every two years. The contractor must maintain all equipment and tools necessary to perform services and respond to emergency or urgent situations within eight hours of notification, 365 days per year. The contract requires contractors to establish standard stocked parts availability within twenty-four hours and to complete all sanitization work the same day work begins. Annual sanitizing must be scheduled with at least 30 days' advanced notice and performed on a weekend using either Minncare HD chemical disinfectant or portable ozone generators. Service reports documenting the description of service, parts used, date, technician name, and system status must be provided per visit.
Pricing is structured in three categories: Category A covers scheduled routine and preventative maintenance invoiced monthly with a unit price specified for the preventative maintenance agreement; Category B establishes hourly labor rates for non-scheduled maintenance and repair services, distinguishing between regular hours (7:00 A.M. to 5:00 P.M. Monday through Saturday) and overtime hours (Sundays, nationally recognized holidays, and hours outside normal working hours), with separate rates for standard repairs and emergency repair services; and Category C allows a percentage mark-up to cost for all related materials and associated components, with the contractor required to invoice at cost using sales receipts or invoiced prices. The contractor must provide all parts meeting manufacturer specifications compatible with existing equipment, and substitutions require prior DSHS Laboratory representative approval. Travel time for routine maintenance is not reimbursable, and expedited shipping charges require prior approval from the DSHS Laboratory Representative/Contract Manager. All work must comply with federal, state, and local codes, including OSHA requirements, the Americans with Disabilities Act, Texas Building Codes, and City of Austin Building Technical Codes.
