Respondents must provide unit rates for two primary service categories: Demolition (including administration, supervisor, laborer, equipment, regulated and non-regulated material trucking, and asbestos-containing material and construction/demolition material disposal) and Backfill, Grade, and Site Finalization (including administration, supervisor, laborer, equipment, backfill material, and topfill material). All highlighted cells on the rate schedule must be completed with corresponding dollar values per unit type.

Respondents must enter rates as a single mean (average) per unit of measure with no ranges, qualifications, alternate unit types, or blank fields permitted.
