PAST PERFORMANCE QUESTIONNAIRE (PPQ)

The Department of Education (ED) has issued Request for Proposals (RFP) number ED-ESE-10-R-0017 for Records Exchange Advice Communication and Technical Support (REACTS) services.

One of the RFP evaluation factors is the offerors’ past performance in similar projects. We depend on information received from agencies/companies such as yours, which have had first- hand experience with an offeror, for the evaluation of the offeror's performance on those contracts.

Areas of interest of the ED are summarized in the enclosed Past Performance Questionnaire (PPQ). The requirement is considered VERY URGENT to the ED. We respectfully request your response no later than April 09, 2010 (3:00 PM EDT). This schedule will allow ED sufficient time to analyze the data prior to the start of evaluations.

Please send your response electronically (your scanned signature is preferred) to:Steve.gagnon@ed.gov; or fax to 202-540-9215; or mail to:

Steven Gagnon U.S. Department of Education PCP/OCFO Room 7154 550 12th Street S.W.

Washington, DC 20202

Please call (202) 540-9215 if you have any questions. Your completed Questionnaire will become a part of the official Source Selection records.

Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of the source selection.

Stephanie Girard Contracting Officer
