PROJECT DESCRIPTION
The Control Road Bridge Replacements Project is to design and construct replacements for seven deficient bridges and to improve one at-grade stream crossing along Control Road (also known as Forest Road 51 and National Forest System Road 64) in the Tonto National Forest, as listed below.
• Webber Creek Bridge, Milepost 5.7
• Bonita Creek Bridge, Milepost 13.8
• Perley Creek Bridge, Milepost 14.7
• Moore Creek Bridge, Milepost 15.4
• Lewis Creek Bridge, Milepost 16.8
• Ellison Creek Bridge, Milepost 17.4

PROCUREMENT PROCESS OVERVIEW
CFLHD is using a two-step procurement process to select a qualified D-B firm/team to deliver the Project.
The issuance of this RFQ is part of the first step to solicit information from interested Proposers in the form of an SOQ.
CFLHD anticipates the following procurement milestone dates. This schedule is subject to revision by addenda to this RFQ, to information in the RFP, and by addenda to the RFP.
Action Dates Phase 1– Request for Qualifications RFQ issued October 7, 2010 Deadline for submitting RFQ questions October 29, 2010 Deadline for CFLHD response to questions November 4, 2010 SOQs due November 10, 2010 Short-listed Proposers notified November 30, 2010* Phase 2 – Request for Proposals Draft RFP released for comments (to short-listed Proposers only) December 3, 2010 * Deadline for submitting draft RFP comments December 10, 2010* RFP issued (to short-listed Proposers only) Early January Deadline for submitting RFP questions Late January Deadline for CFLHD response to questions Late January Proposals Due Early February Notice of Award issued Early March Notice to Proceed issued Early April *Dates are subject to change
Phase 1 – Request for Qualifications
CFHLD is issuing this RFQ for interested and qualified D-B firms/teams to submit an SOQ, which CFLHD will evaluate consistent with the evaluation criteria.
CFLHD’s selection committee – the Evaluation Board– will review and evaluate the SOQs submitted by all responsive Proposers. The Evaluation Board will evaluate the SOQs based on how well each meets the evaluation criteria.
A “shortlist” of three to five of the most highly qualified Proposers will be selected and invited to participate in Phase 2 – Request for Proposals. Once the shortlist has been established, Proposers not shortlisted will be notified in a timely manner.
It is not the intent of CFLHD to receive specific design or engineering recommendations regarding the Project as part of the Phase 1 process. Proposers should limit their SOQ’s to the information required by this notice, and other information that may be relevant to the qualifications and experience of the Proposer.
No reimbursement will be made for any of Proposers’ costs associated with the preparation of SOQ’s.
Phase 2 – Request for Proposals
The short-listed D-B firms/teams will be sent a Request for Proposal (RFP), which will outline the remainder of the selection process requirements. The RFP will contain the requirements necessary for the short-listed D-B firms/teams to submit both a Technical Proposal and a Price Proposal.
After receipt of the Phase 2 Technical Proposal and Price Proposal, an Evaluation Board will conduct a "best value" analysis. Attachment 4 contains a draft of the criteria expected to be used in the evaluation process. It is subject to change as the RFP is finalized. The Evaluation Board will submit their recommendation to the Source Selection Authority. Award of one firm fixed price contract is anticipated.
A stipend will be awarded to each Phase 2 Proposer who provides a responsive, but unsuccessful, Proposal.

PROPOSAL EVALUATION CRITERIA (DRAFT)
These RFP evaluation criteria are being developed concurrently as the RFQ is being issued and as the environmental documentation is being developed and the Scope of Work finalized. These RFP evaluation criteria are subject to change as a result of advancing project development.
Phase 2 – Technical and Price Proposals
The Request for Proposal describes information to be submitted by the Proposers and provides a framework for evaluating the proposals relative to established technical criteria and price.
The purpose of the proposal phase is to evaluate the proposals submitted by the shortlisted proposers. The evaluation will focus on technical elements: characteristics of the proposed project, management of the Design-Build team, design and construction approach, quality management plan and delivery schedule, and the price submitted by the proposer.
Technical Proposal The proposers shall submit information describing their proposed project and methods they intend to use in designing and constructing it. The submittal shall include drawings, written descriptions and other materials that address the criteria below.
Technical Characteristics of Proposed Project The proposers shall submit drawings, details, technical information and narratives that describe the project they intend to design and construct in response to the requirements presented in the Request for Proposals.

The submittal shall include a written Quality Management Plan that describes the Quality Assurance and Quality Control activities that will be undertaken during design and construction.
Price Proposal The proposers shall submit a Price Proposal in a sealed package, separate from the Technical Proposal envelope. The successful Proposer’s Price will become the Contract Bid Price.
The price to be used for evaluation of the price proposal will be the Contract Bid Price only. Contract Administration Cost (cost per day incurred by CFLHD to administer the project) will not be considered for evaluation purposes.
Best Value Analysis and Award. After receipt of the Phase 2 Technical and Price Proposals, the CFLHD Evaluation Board will conduct a "best value" analysis. The Technical scores and Price will be combined to rank Proposals for final selection. The Government will use a tradeoff process for this source selection.
Award will be made to the responsible offeror whose offer conforms to the solicitation requirements and provides the best value to the Government considering the stated technical factors and price. The award of one firm fixed price contract is anticipated in spring 2011.
The Government intends to award a contract without discussions with offerors (except clarifications as described in FAR 15.306(a)); therefore, the offeror’s initial proposal should be clear and complete and contain the offeror’s best terms from a price standpoint. The Government reserves the right to conduct discussions if later deemed necessary. The Government further reserves the right to reject any or all offers if such action is in the Government’s best interest.
