About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162, for the El Molino Avenue Demonstration Project. The form requires prospective bidders to complete a questionnaire under penalty of perjury disclosing whether the bidder, any officer of the bidder, or any employee with a proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate either "Yes" or "No" in response to this inquiry.

If a bidder answers affirmatively to the disqualification question, the form requires the bidder to explain the circumstances of such disqualification and attach supporting documentation as an exhibit. This disclosure requirement serves as a prequalification mechanism to ensure that only bidders in good standing and without relevant violations are permitted to participate in the City of Pasadena's procurement process for the El Molino Avenue Demonstration Project, which involves designing quick-build bicycle infrastructure improvements along a 3.8-mile corridor.
