About this file
This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162, for the Safety Division Training consultant opportunity. The form requires prospective bidders to disclose, under penalty of perjury, whether the bidder, any officer of the bidder, or any employee with proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation. Bidders must indicate yes or no to this inquiry, and if the answer is affirmative, must provide a detailed explanation of the circumstances and attach supporting documentation.
This disclosure requirement is a standard compliance measure designed to ensure that only qualified and legally compliant contractors are eligible to bid on the City of Pasadena's Cal-OSHA safety training services. The form serves as a gatekeeping mechanism to protect the integrity of the procurement process and verify that bidders meet the baseline requirement of not having been previously disqualified from government contracting due to legal or safety violations.
