About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, pursuant to Public Contract Code Section 10162, for bidders responding to an on-call arborist and land surveying services opportunity. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer of the bidder, or any employee with proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violation of law or safety regulations. Bidders must indicate yes or no in response to this inquiry, and if affirmative, must provide a written explanation of the circumstances and attach supporting documentation.

No additional pricing terms, set-asides, contract duration, renewal options, or other financial details are specified in this disclosure form, as it serves solely as a compliance and qualification screening document. The form functions as a prerequisite requirement for bidders to demonstrate their eligibility and regulatory standing prior to proposal submission for the on-call arboricultural services (tree evaluation reports with a 10 business-day turnaround requirement) and land surveying services (Certificate of Compliance review and subdivision map compliance verification, also with a 10 business-day turnaround expectation).
