About this file

The document is a "Disclosure of Investment Activities in Iran" form required for state and local contract opportunities in New Jersey, specifically related to a potential recycling used tires contract with the City of Newark. The form is a legal certification that requires bidders or proposers to attest, under penalty of perjury, whether they or their parent entities, subsidiaries, or affiliates have any investment activities in Iran, as listed on the New Jersey Department of Treasury's Chapter 25 list.

The form provides two certification options: first, the bidder can certify that neither they nor their related entities are on the Chapter 25 list, or second, they can acknowledge being listed and provide detailed information about their Iranian investment activities. The document emphasizes the potential consequences of false statements, including criminal prosecution, contract voiding, and potential debarment. While the form is associated with a City of Newark recycling used tires contract scheduled for bid opening on Thursday, April 17, 2025, at 10:15 am, the form itself is a standard compliance document required for participating in the bidding process, focusing on disclosing any prohibited international investment activities.
