About this file
This document is a Contract Amendment Summary from the Arizona Department of Transportation (ADOT) for Amendment Number One (1) to a contract with Reyes and Sons for CTR077315, a Landscaping and Tree Trimming Services contract. The amendment focuses primarily on updating and replacing Section 21 of the contract, which details comprehensive insurance requirements for the contractor.
The amendment specifies detailed insurance coverage requirements for Reyes and Sons, including Commercial General Liability with aggregate limits of $2,000,000, Business Automobile Liability with a $1,000,000 combined single limit, and Workers' Compensation and Employers' Liability coverage. The new insurance provisions outline specific endorsement requirements, minimum coverage standards, notice of cancellation procedures, insurer acceptability criteria, and verification of coverage protocols.
