About this file
This document is a Procurement File Memorandum from the Arizona Department of Health Services (ADHS), located in Phoenix, Arizona. The memorandum was prepared by Lucas Terry, a Procurement Specialist, and approved by Gina Corwin, the Chief Procurement Officer. The document relates to Contract CTR050594 for the BWCH Health Start Program in Cochise County.
The contract (CTR050592) was originally awarded on July 6, 2021, through Request for Proposal BPM002372, with an initial one-year term and options to renew annually, not to exceed five years. The Program submitted a procurement request to extend the contract on May 17, 2023. Amendment Three was signed by the supplier on July 25, 2023, and executed on July 30, 2023, which was after the original contract expiration date of July 6, 2023. Despite the delayed execution, the document indicates that the services are still needed, and it is in the best interest of the state to reinstate the contract for the remaining term.
