About this file
This document is a Request for Proposal (RFP) Contract Amendment from the Arizona Department of Health Services (ADHS), Office of Procurement, for the Health Start Program. The contract (CTR050594) involves Cochise County Health and Social Services as the contractor, and is part of a broader set of contracts with multiple health and county organizations including Adelante Healthcare, Apache County, Coconino County, Native American Community Health Center, North Country Healthcare, Pima County, and others. The amendment is the sixth in the series and is associated with the Maternal, Infant and Early Childhood Home Visiting (MIECHV) federal grant.
The contract is a cost reimbursement type with a one-year term, extendable up to six years maximum, and is set to continue through September 30, 2025. The total contract amount for Cochise County is being increased to $1,846,550.00, with an additional $88,250.00 being added to the existing contract. The amendment is part of a procurement process to ensure uninterrupted health services, with ADHS currently working on a new Request for Grant Application (RFGA2025-013) to competitively award new contracts. The contract covers health services under the Health Start Program, partially funded by federal MIECHV funds, and is designed to support early childhood and maternal health initiatives across Arizona.

The Arizona Department of Health Services (ADHS) Bureau of Women and Children’s Health (BWCH) supports efforts to improve the health of Arizona’s women and children. Within BWCH, the Office of Children’s Health supports the increased focus within the state and nation on the importance of early childhood programs. The Office of Children’s Health administers the Health Start program as a promising approach model under the Maternal, Infant and Early Childhood Home Visiting (MIECHV) federal grant funded by Health Resources and Service Administration (HRSA). The MIECHV program is funded entirely through federal funds. The Health Start Program is funded partially through MIECHV funds.
Contracts CTR050592, CTR050593, CTR050594, CTR050595, CTR050596, CTR050597, CTR050598, CTR050599, CTR050600, CTR050601, CTR050602, CTR050603, CTR050604, and CTR050606 were awarded via Request for Proposal (RFP) BPM002372 and will reach their maximum end date on July 5, 2025. ADHS procurement is currently working with the program and conducting a new RFGA for these services. RFGA2025-013 was published on May 15, 2025 and we anticipate an award by September 30, 2025.
This CI determination is to extend the current contracts past the five (5) year maximum, through September 30, 2025 and add additional funding for this time period, to allow ADHS procurement time to conduct, evaluate, and award RFGA2025-013 As services cannot stop, it is advantageous to the State to approve this Competition Impracticable request.
Select applicable option in accordance with A.A.C R2-7-E303(A):
Fifteen (15) contracts awarded under RFP BPM002372 are set to meet their max end date on July 5, 2025. ADHS procurement is currently conducting a new RFGA for these services, which is anticipated to be awarded by September 30, 2025. This CI Determination is to extend two (2) months past the five (5) year maximum end date to continue until the new RFGA is awarded, whichever comes first. Pricing is determined fair and reasonable as the cost will remain the same fixed price as in the current Contracts. ADHS Procurement is working with the Program to solicit services to ensure new contracts are competitively awarded and promote competition but must ensure services are uninterrupted.

Memorandum | Office of Procurement
Date: July 29, 2025
To: Memo to Procurement File
From: Lucas Terry, Senior Procurement Specialist
Regarding: CTR050594, Health Start Program – Cochise County Health and Social Services
Subject: CTR050594, Amendment Six (6) – Extension beyond five (5) years
CTR050592 Health Start Program was established through a Request for Proposal (RFP) and became effective on July 6, 2020 for an initial one (1) year period, with the option to renew annually, not to exceed five (5) years.
On May 1, 2025 a procurement request was submitted for Amendment Six (6) to extend the Contract to September 30, 2025. Amendment Six (6) was sent to Contractor on June 24, 2025 for signature. The Contractor returned the signed amendment on July 23, 2025, after the Contract expired.
As the program’s intent was to extend the contract through September 30, 2025, it is in the best interest of the State to reinstate this contract effective July 6, 2025 through September 30, 2025.
