About this file

This document is Amendment No. 5 to a mandatory use contract between the State of Ohio, specifically the Department of Administrative Services (DAS) and the Ohio Department of Health (ODH), and Health Monitoring Systems Inc., a contractor located in Pittsburgh, Pennsylvania. The contract is for a Real-Time Syndromic Surveillance Program, with Contract Number CSP903722, originally issued following a Request for Proposal (RFP) No. CSP901121.

The amendment renews the contract for an additional 12 months, extending the term from 09/01/24 through 08/31/25, and introduces a new Ohio Implementation Proposal for Inpatient Data. The cost summary now includes several new line items related to inpatient data collection, such as Inpatient Data Project Administration at $100,000 per year, Inpatient Data Connection and Correction at $7,500 per facility, Validation of Inpatient Data Capabilities at $2,500 per facility, Inpatient NSSP Service at $25,000 per year, and Inpatient Ohio Data Relay Service at $25,000 per year. The contractor will continue to provide cyber liability coverage, with a temporary allowance of $2M and a requirement to move to $5M coverage. All other prices, terms, and conditions remain unchanged from the previous amendment.
