About this file

This is Addendum No. 01 to a Centralized Request for Quote (CRFQ) issued by the West Virginia Department of Administration Purchasing Division for the West Virginia Conservation Agency. The solicitation, numbered CRFQ AGR27-07, seeks debris removal from Opequon Creek in Berkeley County, West Virginia, and streambank repair due to flood damage. A mandatory pre-bid meeting is scheduled for August 14, 2026, at 11:00 AM. Vendor questions are due by August 19, 2026, at 2:00 PM. The bid opening is scheduled for August 26, 2026, at 1:30 PM. The addendum modifies the original solicitation by attaching required forms but does not change any previously established dates or deadlines.

The addendum requires all bidders to submit four forms with their proposals: the Construction Bid Submission Review Form (WV-75), Drug Free Workplace Conformance Affidavit (WV-73), Certified Drug-Free Workplace Report Coversheet (WV-72), and a Bid Bond. The bid bond must be in an amount equal to a minimum of 5 percent of the total bid and must be executed by a surety licensed in West Virginia. Failure to attend the mandatory pre-bid meeting, sign the bid, supply a valid bid bond, acknowledge receipt of addenda, or meet any other mandatory requirement will result in immediate bid disqualification. All provisions of the original solicitation remain in full force and effect except as modified by this addendum. Vendors are instructed to submit an Addendum Acknowledgment with their bids to expedite document processing.
