About this file
This document is an Emergency Purchase Extension Approval form from the State of Illinois Chief Procurement Office for General Services, involving the EPA's Environmental Protection Agency Bureau of Water and Serv Pro of Peoria. The emergency contract pertains to the restoration of water-damaged paper documents following a water leak. The original emergency contract was initiated on March 3, 2025, with an initial term ending June 2, 2025, and is now seeking an extension from June 3, 2025, to August 2, 2025. The scope of work involves cleaning, drying, separating, and restoring approximately 80 total file drawers across three document types: 42 BOW File Drawers, 31 BOL File Drawers, and 7 BOA File Drawers.
The original contract amount was $60,000, and the extension request is for $0, with the vendor already in possession of the documents and near completion of the restoration process. The extension is justified by the vendor's need for additional time to finish drying and restoring the remaining documents. The agency argues that switching vendors could compromise document preservation and potentially cause molding during a transition period. The emergency purchase extension was approved on May 29, 2025, with signatures from the Agency Procurement Officer (Andrew Shackelford), State Procurement Officer (Robert Brice), and Chief Procurement Officer (Ellen Holzman Daley).
