About this file
This is a bid quote sheet for the New York State Office of Mental Health (OMH) Clinical Laboratories at the Nathan S. Kline Institute (NKI) for Psychiatric Research, requesting hazardous waste removal services for a three-year contract period. The sheet itemizes three primary waste categories: hazardous/chemical waste, universal waste (lamps), and electronic waste. Hazardous waste items include non-halogenated solvents (1,000 pounds annually), halogenated organic solvents (1,000 pounds annually), formalin solutions (420 pounds annually), chromic and sulfuric acid waste (85 pounds annually), various laboratory chemicals and pharmaceutical wastes, and waste containers including fiber drums, DOT drums, and labels. Universal waste items include 1,500 fluorescent lamps annually, PCB and non-PCB ballasts (100 and 500 pounds respectively), lead and alkaline batteries (500 and 300 pounds respectively), and waste paint/aerosol cans (1,000 pounds annually). Electronic waste consists of 25 computers and 5 microwaves annually. All items are scheduled for pickup twice annually, with the exception of one expandable fiber drum on a single annual pickup. Bidders must submit pricing for each item across three contract years, with costs calculated in cost per unit of measurement format.
All quoted prices must include applicable fees with no additional charges for freight, labor, or gas permitted on invoices. The hazardous waste pickup schedule shall occur once every six months, with yearly rates billed in two equal 50 percent installments. The previous contract, awarded to ACV Environmental Services, Inc. (contract T202643), totaled $39,434.85 for the period May 1, 2025 to April 30, 2026. This procurement includes minority and women business enterprise (MWBE) goals of 30 percent and a service-disabled veteran-owned business (SDVOB) goal of 6 percent. Bidders must certify compliance with prevailing wage rates, electronic payment system requirements, and New York State payroll restrictions for personnel providing services. Bidders are required to submit a hard copy of the completed bid quote sheet by the bid due date and provide an Excel version for pricing review verification.
