About this file

This is a Contractor Questionnaire designed to evaluate potential suppliers for the Defense Logistics Agency's (DLA) Sources Sought Announcement (SSA) for the SICPS Weapon System Installation Kit (MK-3035/VRC). The questionnaire requests comprehensive information about contractor capabilities across multiple dimensions of manufacturing, sourcing, and quality assurance.

Contractors must provide company background and core competencies, along with point of contact information including CAGE and DUNS numbers. The questionnaire addresses past performance on the subject kit or similar products, requiring contract numbers and references for the most recent work. Respondents must demonstrate capability to produce the kit in three quantity ranges: up to ten units, ten to fifty units, and fifty to one hundred units, while identifying any associated challenges. The questionnaire requires detailed descriptions of sourcing and manufacturing approaches for electronic components (such as cables), mechanical components (such as mounting brackets), and commercial-off-the-shelf items identified in the technical data package (TDP). Contractors must identify any problematic components to source or manufacture and describe their experience handling export and distribution-restricted DoD technical data, managing product complexity comparable to the subject kit, and implementing obsolescence mitigation strategies. Additional requirements include addressing delivery lead times, capability to meet DoD delivery schedules, experience with vendor supply chain management, preferences regarding fixed quantity purchase contracts, and quality assurance procedures performed either in-house or through subcontractors. The government will conditionally require a Government Acceptance of Delivery process for sources lacking prior provision of the kit within five years, though waivers may be granted for similar form/fit/function kits.
