About this file
This is a Common Carrier Attestation form required by Pinellas County, Florida, in accordance with Florida Statute 908.111 and 92.525. The attestation applies to contractors providing services for the Veterinary Drugs and Supplies contract opportunity in Florida. The document requires authorized contractor representatives to certify under penalties of perjury that they will not willfully provide transportation services for unauthorized aliens into the state, except to facilitate detention, removal, or departure from Florida or the United States. Contractors must complete the form by providing their name, signature, date, Federal Work Authorization User Identification Number, and the specific Pinellas County contract name and number.
The attestation serves as a compliance mechanism and contractual obligation for all vendors involved in the veterinary drugs and supplies procurement. Contractors must be defined as "common carriers"—individuals, firms, or corporations that transport persons or commodities for hire as a regular business. Pinellas County reserves the right to terminate the associated agreement for cause if any contractor is found in violation of this attestation. Execution of this document is mandatory for contract award and performance.
